Effectiveness of an mHealth system on access to eye health services in Kenya: a cluster-randomised controlled trial.

BACKGROUND: There is limited access to eye health services in many low-income and middle-income populations. We aimed to assess the effectiveness in increasing service utilisation of the Peek Community Eye Health (Peek CEH) system, a smartphone-based referral system comprising decision support algorithms (Peek Community Screening app), SMS reminders, and real-time reporting. METHODS: In this cluster-randomised controlled trial of eye health in Kenya, community unit clusters were defined as one health centre and its catchment population. Clusters were randomly allocated (1:1) to receive Peek C

Increasing Access to Medications for Opioid Use Disorder and Complementary and Integrative Health Services in Primary Care.

OBJECTIVES: Evidence-based therapies for opioid use disorder (OUD) and chronic pain, such as medications for OUD (MOUD) and complementary and integrative health (CIH; e.g., acupuncture and meditation) therapies, exist. However, their adoption has been slow, particularly in primary care, due to numerous implementation challenges. We sought to expand the use of MOUD and CIH within primary care by using an evidence-based quality improvement (EBQI) implementation strategy. METHODS: We used EBQI to engage two facilities in the Veterans Health Administration (VHA) from June 2018 to September 2019.

Neighbourhood deprivation, health inequalities and service access by adults with intellectual disabilities: a cross-sectional study.

BACKGROUND: Adults with intellectual disabilities (IDs) experience health inequalities and are more likely to live in deprived areas. The aim of this study was to determine whether the extent of deprivation of the area a person lives in affects their access to services, hence contributing to health inequalities. METHOD: A cross-sectional study design was used. Interviews were conducted with all adults with IDs within a defined location (n = 1023), and their medical records were reviewed. The extent of area deprivation was defined by postcode, using Carstairs scores. RESULTS: Area deprivation

Access to maternal and perinatal health services: lessons from successful and less successful examples of improving access to safe delivery and care of the newborn.

The huge majority of the annual 6.3 million perinatal deaths and half a million maternal deaths take place in developing countries and are avoidable. However, most of the interventions aiming at reducing perinatal and maternal deaths need a health care system offering appropriate antenatal care and quality delivery care, including basic and comprehensive emergency obstetric care facilities. To promote the uptake of quality care, there are two possible approaches: influencing the demand and/or the supply of care. Five lessons emerged from experiences. First, it is difficult to obtain robust evi

Patterns of Social Determinants of Health and Publicly-Funded Service Access among Children Involved in Educational, Child Welfare, and Social Service Systems.

Social determinants of health (SDOH)-conditions in which children live, learn, and play-affect child health and well-being. Publicly funded services in education and child welfare systems are important resources to support child well-being, but cross-system coordination is rare. Leveraging integrated administrative data from 60,287 6th graders enrolled in public schools in Minnesota, we used latent class analysis (LCA) to examine patterns of cross-system SDOH, including educational services and involvement in child welfare. Five classes emerged. The largest class was characterized by a few mul

A Promising Route Towards Improvement of Homeless Young People's Access to Mental Health Services: The Creation and Evolution of an Outreach Service Network in Montréal.

Youth homelessness is a complex phenomenon as well as an important public health issue often compounded by mental illness of varying severity, in turn creating numerous deleterious consequences. While emergency health services usage remains high, access to mental health services is arduous and conventional interventions often fall short on providing integrated care and seldom lead to sustained positive outcomes for this group. From this observation, clinicians in Montreal, Canada, initiated collaborative meetings, eventually attended by a growing number of institutional and community stakehold

Bridging the knowledge gap: accessibility of SRHR information, services and competences of health facilities for adolescents and their care givers in rural Eastern Uganda.

INTRODUCTION: A quarter of Uganda's population is aged 10-19 years and majority live in rural areas. Access to sexual and reproductive health services and rights (SRHR) remains a challenge among this population, contributing to a high teenage pregnancy rate of 24.9% in rural Uganda. The study assessed the accessibility of SRHR information, services and competences of health facilities among adolescents and their care givers in Namutumba and Bugiri districts. METHODS: A cross-sectional study was conducted. 473 respondents were systematically and randomly recruited after informed consent and/ a

TELEHEALTH and digital health platforms in promoting access to sexual reproductive health self care among youth: A case of rocket health services in Uganda.

Sexual Reproductive Health (SRH) self-care offers a pathway for low income countries to advance towards Universal Health Coverage by empowering individuals, families, and communities to prioritize their SRH needs independently of healthcare providers. Facilitating access to SRH products is crucial for embracing self-care and digital health technologies hold promise for enhancing accessibility. This study explored the role played by rocket health digital platforms in enhancing accessibility to SRH self-care products among youth in Uganda. Employing a cross-sectional design with a mixed-method a

Roles of social workers and community health extension workers in enhancing access to maternal and child health services: reflection on a Nigerian intervention.

Nigeria introduced Maternal and Child Health (MCH) programs to improve access to skilled care in response to persistently poor health outcomes among women and children. While existing assessments often emphasize the role of healthcare workers, the contributions of social workers and Community Health Extension Workers (CHEWs) remain largely overlooked. This study explores the roles played by social workers and CHEWs in enhancing access to MCH services through an intervention project in Anambra State, Nigeria. An exploratory qualitative design was employed, involving interviews with five social

Gender disparities and barriers to access and use of essential health services in Ethiopia: Designing primary health care through gender lens.

Despite remarkable gains in maternal and child health (MCH) outcomes globally, the lack of access to and use of essential health services remains a challenge in low-income countries, like Ethiopia. To address health service access and use, the Ethiopian government has endorsed a fifteen-year "Optimizing Ethiopian Health Extension Program" strategy. This study explored gender barriers to access to and use of MCH services in the primary health care (PHC) setting in Ethiopia. An exploratory qualitative study was conducted in nine districts in Ethiopia. Data were collected using 87 key informants

Barriers to Accessing Sexual and Reproductive Health Services and Associated Mental Health Impacts Among Migrant Women in the UK: A Scoping Review.

Migrant, internally displaced, asylum-seeking, and refugee women in the United Kingdom experience ongoing risks of having their sexual and reproductive healthcare rights violated. This growing population also has limited access to sexual and reproductive health (SRH) services, which may be associated with psychological distress. This scoping review was conducted to identify barriers to accessing SRH services and reported mental health experiences in this population. Following the PRISMA-ScR guidelines and the Arksey & O’Malley methodology, searches were conducted in six bibliographic databases

Mental health of people with limited access to health services: a retrospective study of patients attending a humanitarian clinic network in Germany in 2021.

BACKGROUND: Germany has a statutory health insurance system. However, a substantial part of the population still suffers from limited access to regular health services. While humanitarian organizations are partially filling this gap, people with limited access show a high prevalence of mental disorders. This study investigates the prevalence, and social determinants of mental disorders in patients attending the clinics of a humanitarian health network in three major cities in Germany, as well as perceived barriers to healthcare access in this population. METHODS: We performed a descriptive, r

Migrants and Service Providers' Perspectives of Barriers to Accessing Mental Health Services in South Australia: A Case of African Migrants with a Refugee Background in South Australia.

International mobility has increased steadily in recent times, bringing along a myriad of health, social and health system challenges to migrants themselves and the host nations. Mental health issues have been identified as a significant problem among migrants, with poor accessibility and underutilisation of the available mental health services (MHSs) repeatedly reported, including in Australia. Using a qualitative inquiry and one-on-one in-depth interviews, this study explored perspectives of African migrants and service providers on barriers to accessing MHSs among African migrants in South

Access of non-specialist sexual health services by men who have sex with men: do they differ from those attending specialist services?

OBJECTIVES: Men who have sex with men (MSM) bear a disproportionate burden of STIs. While routine STI surveillance data suggest MSM regularly access specialist genitourinary medicine (GUM) clinics for their sexual healthcare, the extent to which MSM attend non-specialist sexual health services (SHSs) is unclear. METHODS: We used data from the GUM Clinic Activity Data Set (GUMCADv2), the national STI surveillance system, to compare the characteristics, service usage and STI outcomes of MSM accessing specialist and non-specialist (non-GUM) SHSs in England in 2014.
