Embracing computer modeling to address pandemic influenza in the 21st century.

Reacting to a pandemic influenza outbreak will require the mass distribution of vaccine, when available, which will require county health departments to set up and operate one or more mass vaccination clinics, also known as points of dispensing (PODs). Carefully planning these PODs before an event occurs is a difficult but important job. First, this article describes a tool--the Clinic Planning Model Generator computer program--designed to help public health agencies evaluate and make adjustments to their POD plans. The Clinic Planning Model Generator was built on data from a smallpox exercise

Some international activities in environmental health monitoring and surveillance.

Environmental health monitoring and surveillance include activities such as collection of information on the production and use of chemicals; preparation of inventories of waste discharges; measurement of physical, chemical and biological agents in air, water and food, at work place and at home; epidemiological investigations, and collection and analysis of environmental, and health statistical data. There are two main objectives of these activities: estimation of human exposure to potentially harmful environmental factors and timely detection of adverse health effects; and the assessment of e

A Psychometric Analysis of the Health Literate Health Care Organization-10 Item Questionnaire.

The concept of a Health Literate Healthcare Organization (HLHO) is a relatively new approach to health literacy that moves the focus from the individual patient to the overarching health care system. The HLHO-10 questionnaire was developed internationally to assess the 10 Attributes of HLHOs as described by participants of the Institute of Medicine Roundtable on Health Literacy. The purpose of this study was to establish reliability and validity of the HLHO-10 among a sample of United States hospitals. Reliability and validity were established through assessing the factor structure for the HLH

Coalitions as Prevention Infrastructure for Youth Substance Use: Coordinated, Local Relevance, and Sustainability.

CONTEXT: Youth substance use remains a preventable public health issue, yet substantial proportions of US adolescents continue to report substance use. Despite advances in prevention, the substance use landscape continues to evolve, and disparities persist, underscoring the need for strategies that address the social, environmental, and structural factors influencing youth substance use. OBJECTIVE: To outline the conceptual rationale for community partnerships in youth substance use prevention, summarize evidence supporting coalition- and partnership-based prevention systems, and demonstrate,

A Systematic Evaluation of Barriers and Facilitators to the Provision of Services for Justice-Involved Women.

The community health delivery system (CHDS) are vital agencies to the success of integration and the provision of services to improve the health and well-being of justice-involved women. Many agencies face barriers and challenges in providing services to vulnerable populations, such as justice-involved women, and, as a result, often offer individual rather than coordinator care. Thus, it is necessary to explore CHDS systemic barriers and challenges to identify opportunities for coordinated care. We conducted semi-structured interviews with 26 CHDS directors or designees to identify systemic ba

Patient safety: a shared responsibility.

Patient safety is an essential and vital component of quality nursing care. However, the nation's health care system is prone to errors, and can be detrimental to safe patient care, as a result of basic systems flaws. A variety of stakeholders (society in general; patients; individual nurses; nursing educators, administrators, and researchers; physicians; governments and legislative bodies; professional associations; and accrediting agencies) are responsible for ensuring that patient care is safely delivered and that no harm occurs to patients. The responsibility of these stakeholders in addre

Continuity of mental health care to youth in the juvenile justice network.

A sizable number of youth at risk for psychiatric disabilities enter the juvenile justice system each year without sufficient screening mechanisms or treatment systems. In 1977 the children and youth division of a mental health center began a program of evaluation, treatment, and consultation for juveniles in the subsystem of probation, secure and nonsecure detention centers, urban group homes, and other postadjudication residential facilities. One of the major benefits--a central and coordinated response to youth identified as having significant emotional problems--stems from the opportunitie

Family planning clinic services in the United States: patterns and trends in the late 1990s.

CONTEXT: Publicly funded family planning clinics are a vital source of contraceptive and reproductive health care for millions of U.S. women. It is important periodically to assess the number and type of clinics and the number of contraceptive clients they serve. METHODS: Service data were requested for agencies and clinics providing publicly funded family planning services in the United States in 1997. The numbers of agencies, clinics and female contraceptive clients were tabulated according to various characteristics and were compared with similar data for 1994. Finally, county data were ta

Scarcity in the Covid-19 Pandemic.

As we write, U.S. cities and states with extensive community transmission of Covid-19 are in harm's way-not only because of the disease itself but also because of prior and current failures to act. During the 2009 influenza pandemic, public health agencies and hospitals developed but never adequately implemented preparedness plans. Focused on efficiency in a competitive market, health systems had few incentives to maintain stockpiles of essential medical equipment. Just-in-time economic models resulted in storage of only those supplies needed then. At the same time, global purchasing in search

Epidemiology of ocular trauma in limited-resource settings: a narrative review.

Ocular trauma disproportionately impacts low- and middle-income countries (LMICs) and contributes significantly to blindness and disability in these settings. While numerous publications address the epidemiology of ocular trauma in limited-resource settings, there are no systematic reviews, meta-analyses, or large-scale review articles investigating this topic further. In this article, the authors summarize, compare, and contrast the extant literature on ocular trauma in LMICs. With this synthesis of the available data, the article aims to identify commonalities and potential targets for syste

The effects of the (fentanyl-fueled) drug overdose epidemic on medicolegal death investigation in the United States.

The evolving opioid epidemic in the United States, fueled by illicit fentanyl, has greatly increased deaths from illicit drug use. These nonnatural deaths require formal death investigation. The National Association of Medical Examiners states in its Forensic Autopsy Performance Standards that autopsy remains a necessary component for proper investigation of suspected acute overdose deaths. If a death investigation office lacks adequate resources to investigate all deaths under its jurisdiction while meeting expected standards, then that office may be forced to consider altering its protocols

Formulation and Delivery Systems of Cocaine and Methamphetamine Scent Mimics.

Narcotic scent mimics, which are chemicals that emit a perceivable scent similar to that of the illicit drug but do not possess the health and addiction risks of the drug, have applications ranging from canine training in law enforcement agencies to antinarcotic public education. However, low-cost and easily accessible formulation and delivery systems for narcotic scent mimics have thus far been lacking. In this work, we assess the potential of synthetic (metal-organic framework, MOF) and natural (cotton) materials as carrier and delivery agents for pseudoscent chemicals mimicking cocaine and

Understanding barriers and facilitators of inter-organizational dynamics in addressing substance use disorder among pregnant and parenting women.

BACKGROUND: Pregnant and parenting women with substance use disorders (SUDs) face complex and overlapping challenges, including substance use, legal issues, housing instability, and trauma. Effective interorganizational collaboration is critical but often hindered by fragmented care and resource limitations. This study explores the key barriers and facilitators that impact collaborative efforts among healthcare providers, government agencies, and community organizations in addressing SUD among pregnant and parenting women. METHODS: This qualitative study was conducted in New York State betwee

Our public safety system is a perfect storm.

Public safety results from an effective interaction of three separate systems - public health, mental health, and policing. In too many communities today, the crisis in our mental health system creates the perfect storm. Solving the issues of silo-based care necessitates creating an oversight data management structure supporting cross-sector data integration on all levels ensuring that both operational and technical frameworks exist to maintain the security of client data. Safer communities isn't just about being sophisticated, technologically advanced but using an intensifying laser-focus ana

Leveraging Lessons Learned from Yellow Fever and Polio Immunization Campaigns during COVID-19 Pandemic, Ghana, 2021.

Ghana is a yellow fever-endemic country and experienced a vaccine-derived polio outbreak in July 2019. A reactive polio vaccination campaign was conducted in September 2019 and preventive yellow fever campaign in November 2020. On March 12, 2020, Ghana confirmed its first COVID-19 cases. During February-August 2021, Ghana received 1,515,450 COVID-19 vaccines through the COVID-19 Vaccines Global Access initiative and other donor agencies. We describe how systems and infrastructure used for polio and yellow fever vaccine deployment and the lessons learned in those campaigns were used to deploy C
