Percutaneous bone-anchored hearing aid.

Patients born with severe dysmorphology involving the ears usually have hearing derangements as well as other areas of the craniofacial skeleton affected. To correct the functional problem, the usual and customary treatment is augmentation of the hearing with a bone conducting hearing device. The patients have to wear these devices with an external band. The new advances in osseointegrated implant allowed us today to utilize the same technology in the patient with the application of a bone anchored hearing device. The advantages are related to the obviation of the need to use a hearing band ac

The time course of hearing aid benefit.

OBJECTIVE: The present study sought to determine whether benefit derived from hearing aid use increases with hearing aid experience. This question is of considerable interest to both researchers and clinicians. Several previous investigations recently published have obtained mixed results. DESIGN: Control conditions were provided to address the potential influences of practice effects and changes in preferred hearing aid gain. The experimental group consisted of 13 hearing-impaired listeners about to be fit with their first hearing-aid. The control group consisted of 13 hearing aid users with

The history and development of the implantable hearing aid.

Implantable hearing aids have an interesting and colorful history. These devices have usually been investigational, but this has changed. Currently, several implantable devices are available for ongoing clinical trials in humans. They have promise in providing improved hearing in certain cases of conductive, mixed, and sensorineural hearing loss.

[New aspects of hearing aid fitting].

Improved methods to diagnose a hearing loss and the enormous progress in the development of digital and programmable hearing aids have been helpful in promoting the social integration of patients, even of those affected by a profound deafness. Besides the hearing loss, recruitment effects and alteration of sound can also be compensated by the new hearing aids. However, one precondition is that the hearing aid fitting is carried out professionally, with plenty of time, and including the new hearing aid fitting methods. The new techniques developed in our hospital to optimize hearing aid fitting

Physical handling of hearing aids by the elderly.

In an attempt to overcome handling problems with hearing aids in elderly patients, we have examined the prevalence of anticipated handling problems in this group and their ability to cope with different types of ear moulds and hearing aids. Increased total prevalence of anticipated problems was found only in the 85+ age group, although anticipated handling problems increased markedly in the 75+ age group. Patients over 75 years found it significantly easier to handle postaural aids with modified skeleton moulds than temporary tips or standard skeletons. They also coped less well with in-the-ea

[The importance of hearing-aid testing by the otorhinolaryngologist].

In a period of 16 months, 312 patients suffering from impaired hearing were supplied with hearing aids after undergoing conventional hearing tests conducted by acousticians. During the hearing follow-up tests, it was found that 111 patients (35.5%) had been optimally supplied with hearing aids and that for 98 hard-of-hearing persons (31.4%) merely fine adjustments had to be made on the hearing aids. In some cases a second bore had to be drilled into the ear mould. We had to completely refit 103 patients (33.0%) with hearing aids, since the previously fitted devices no longer complied with the

A comparison of custom in-the-ear hearing aids.

Custom-built in-the-ear hearing aids from five manufacturers were submitted to an extensive battery of audiologic tests to compare the usefulness of each unit for a moderate-severe sensorineural hearing loss as well as to examine differences between monaural versus binaural fittings. Using a ranking procedure, the hearing aids from one manufacturer were determined to be the best for this 30-year-old patient. Substantial differences were observed in each test condition. The variability of speech discrimination measures, however, was not definitive in determining the hearing aid which was best,

[The effectiveness of hearing aids in elderly people].

The analysis of factors which influence on the hearing aid efficacy in elderly with presbycusis is presented in the article. 125 patients from 60 to 90 years old with moderate to severe hearing loss, hearing aid users, were examined. The audiology examination included standard methods as well as tests evaluating central auditory pathways processing. According to the results of speech audiometry in free field low efficacy of hearing aid is revealed in 21 % patients. The main factor that decreases hearing aid benefit is the presence of central auditory processing disorders, the additional one is

[The evaluation of hearing aid effect in presbycusis].

Objective:To evaluate the effect of hearing aid in presbycusis. Method:Using the method of international outcome inventory for hearing aids(IOI-HA)and medium acoustic intensity(65 dBSPL) word recognition score(WRS), to evaluate the effect of hearing aid in moderate and severe presbycusis. Result:After the hearing aid of moderate presbycusis, The improved value of monosyllabic words and recognition rate in quite and noise statement(SNR=5) were 31.15%, 23.21%, 44.11%.However, improved values in severe presbycusis were 37.51%, 48.47%,50.17%, before and after hearing aid, the difference of the imp

Surgery for the bone-anchored hearing aid.

INTRODUCTION: This review covers the surgery for the bone-anchored hearing aid (Baha(®)). PREOPERATIVE WORKUP: A review of the indications and preoperative diagnostics shows that best results are generally obtained in patients with conductive or mixed hearing loss rehabilitation when surgery is not applicable or has failed and in patients that suffer from single-sided deafness. An audiogram must confirm that the bone conduction hearing is within the inclusion criteria. A computed tomography scan is performed in cases of malformation to assure sufficient bone thickness at the site of screw impl

Biological basis of hearing-aid design.

We show that we can accurately model the auditory-nerve discharge patterns in response to sounds as complex as speech and ask how we may exploit this knowledge to test new strategies for hearing-aid signal processing. We describe the auditory-nerve representations of vowels in normal and noise-damaged ears. The normal representations are predicted well by a cochlear signal processing model originally developed by Carney (Carney, L. H. J. Acoust. Soc. Am. 93:401-417, 1993). Basilar-membrane tuning is represented by a time-varying narrow-band filter. Outer hair cell control of tuning is exerted

[Case report: swallowed hearing aid].

An 86-year-old man presented ambulatory with acute dysphagia. Radiologic examination and endoscopy revealed a swallowed postauricular hearing aid. The earmold of the hearing aid became visible in the hypopharynx after mucus and saliva were removed. It could be extracted without effort once the connecting tube was disconnected from the coupling device lodged in the upper esophageal sphincter. The hearing aid itself was impacted in the proximal esophagus and was extracted without any problems. The postoperative phase was uneventful with normal swallowing and discharge. Technical inspection revea

Destruction of hearing aid by lightning strike.

A 20-year-old woman was struck by lightning while seeking shelter under a tree. The discharge entered at the site of the victim's behind-the-ear type hearing aid, destroying it. She suffered cardiorespiratory arrest, burns, left ventricular failure, and myoglobinuria, yet recovered fully. It is widely known that carrying a metallic object such as an umbrella or golf club above one's head increases the risk of being struck. However, few people know that small metallic objects worn on the head are similarly dangerous. Persons caught outside in a lightning storm might decrease the probability of

Hearing aid selection by brainstem audiometry.

Hearing aid selection and adjustment by means of brainstem audiometry was tested in 29 hearing impaired children. A projection procedure is introduced which employs normal and pathological intensity-amplitude functions. This method enables determination of some basic properties of appropriate hearing aids, such as average gain, type of compression, compression factor and onset level. Hearing aid evaluation is discussed for two examples of conductive and sensorineural impairment.
