Age-related hearing impairment (ARHI) is commonly associated with decreased auditory temporal resolution caused by auditory neurodegeneration. Age-related deterioration in gap detection ability, resulting in poor temporal auditory processing, is often attributed to pathophysiological changes in both the peripheral and central auditory systems. This study aimed to investigate whether the gap detection ability declines in the early stages of ageing and to determine its usefulness in detecting peripheral and central auditory degeneration. The study used 1-month-old (1 M), 6-month-old (6 M) and 12

The classic view of sensorineural hearing loss (SNHL) is that the "primary" targets are hair cells, and that cochlear-nerve loss is "secondary" to hair cell degeneration. Our recent work in mouse and guinea pig has challenged that view. In noise-induced hearing loss, exposures causing only reversible threshold shifts (and no hair cell loss) nevertheless cause permanent loss of >50% of cochlear-nerve/hair-cell synapses. Similarly, in age-related hearing loss, degeneration of cochlear synapses precedes both hair cell loss and threshold elevation. This primary neural degeneration has remained hid

Synaptopathy is a common cochlear pathology involving damage, dysfunction, and loss of auditory-nerve synapses with sensory hair cells. Cochlear synaptopathy can occur following sound overexposure due to glutamate excitotoxicity. Synaptopathy is undetectable with the clinical audiogram and is widely hypothesized to cause "hidden hearing loss," i.e., impaired perception of speech and other complex sounds under challenging listening conditions. However, evidence that cochlear synaptopathy causes hidden hearing loss is mixed, and the specific aspects of auditory perception impacted by synaptopath

UNLABELLED: Noise can induce hearing loss. In particularly, noise can induce cochlear synapse degeneration leading to hidden hearing loss, which is the most common type of hearing disorders in the clinic. Currently, there is no pharmacological treatment, particularly, no post-exposure (i.e., therapeutic) treatment available in the clinic. Here, we report that systematic administration of K + channel blockers before or after noise exposure could significantly attenuate NIHL and synapse degeneration. After systematic administration of a general K-channel blocker tetraethylammonium (TEA), the ele

OBJECTIVES: Sensorineural hearing loss (SNH) is one of the most common forms of hearing loss. A special form of SNH is hidden hearing loss (HHL) with subjective normal hearing. Current research results indicate that these patients demonstrate a reduced wave I in the averaged signal of brainstem audiometry (ABR). Since the averaging technique is not susceptible to latency jitter and amplitude height variation, a single sweep analysis is required for a deeper insight in HHL.
MATERIAL AND METHODS: A total of 14 mice with significantly different calcium currents in the IHC at normal hearing thres

The synapses between inner hair cells (IHCs) and spiral ganglion neurons (SGNs) are the most vulnerable structures in the noise-exposed cochlea. Cochlear synaptopathy results from the disruption of these synapses following noise exposure and is considered the main cause of poor speech understanding in noisy environments, even when audiogram results are normal. Cochlear synaptopathy leads to the degeneration of SGNs if damaged IHC-SGN synapses are not promptly recovered. Oxidative stress plays a central role in the pathogenesis of cochlear synaptopathy. C-Phycocyanin (C-PC) has antioxidant and

A 38-year-old woman with fibrous dysplasia of the entire right half of the mandible and the right temporal bone is presented. She had a unilateral progressive conductive hearing loss and stenosis of the auditory canal complicated by a chronic external otitis, a hidden cholesteatoma, destruction of the ossicles and an impending facial palsy. Surgery was performed to create a new auditory canal, to eradicate the cholesteatoma and to decompress the facial nerve from the surrounding fibrous dysplasia.

A certain degree of noise can cause hearing problems without a permanent change in the hearing threshold, which is called hidden hearing loss and results from partial loss of auditory synapses. Photobiomodulation (PBM) enhances neural growth and connections in the peripheral nervous systems. In this study, we assessed whether PBM could rescue cochlear synaptopathy after acoustic overexposure in rat. PBM was performed for 7 days after noise exposure. The auditory brainstem responses (ABRs) were acquired before and after noise exposure using a tone and a paired-click stimulus. Auditory response

Cochlear nerve degeneration (CND), including the loss of synapses between inner hair cells and auditory nerve fibers (ANFs), has emerged as a likely contributor to "hidden hearing loss", a condition in which listeners experience speech-in-noise difficulties that cannot be fully explained by audiometric thresholds. This form of primary neural de-afferentation preferentially affects low- and medium-spontaneous rate (SR) fibers, which are critical for encoding acoustic features such as amplitude modulations, especially under challenging listening conditions such as noisy backgrounds. Although CND

Exposure to different types of noise for varying durations can lead to damage in different parts of the auditory pathway, such as synapse and hair cell loss, resulting in temporary or permanent threshold shifts. Noise-induced hearing loss (NIHL) involves intricate mechanisms in its pathogenesis, with oxidative stress and glutamate excitotoxicity considered important factors. Currently, there is a lack of effective drugs for NIHL. This study exhibited noise-induced hidden hearing loss (NIHHL) and noise-induced permanent hearing loss in CBA/CaJ mouse models and found that pre-administration of N

Hidden hearing loss, or cochlear neural degeneration (CND), disrupts suprathreshold auditory coding without affecting clinical thresholds, making it difficult to diagnose. We present an information-theoretic framework to evaluate speech stimuli that maximally reveal CND by quantifying mutual information (MI) loss between inner hair cell (IHC) receptor potentials and auditory nerve fiber (ANF) responses and acoustic input and ANF responses. Using a phenomenological auditory model, we simulated responses to 50 CVC words under clean, time-compressed, reverberant, and combined conditions across di

PURPOSE: In this study, the development of a quantitative measurement method to predict long-term auditory adaptation through the stimuli that have been modulated according to different short-term modulation types was aimed to form a psychoacoustic test battery. It might be used in the evaluation process of individuals with hidden hearing loss.
METHODS: The individuals participating in our study were separated into two groups: high-risk group (n = 39) and low-risk group (n = 30) according to the noise-exposure score. To all participants, auditory brainstem response (ABR), dichotically digit t

Tinnitus is a phantom auditory sensation in the absence of external sounds, while hyperacusis is an atypical sensitivity to external sounds that leads them to be perceived as abnormally loud or even painful. Both conditions may reflect the brain's over-compensation for reduced input from the ear. The present work differentiates between two compensation models: The additive central noise compensates for hearing loss and is likely to generate tinnitus, whereas the multiplicative central gain compensates for hidden hearing loss and is likely to generate hyperacusis. Importantly, both models predi

ATP6V1B2 encodes the V1B2 subunit in V-ATPase, a proton pump responsible for the acidification of lysosomes. Mutations in this gene cause DDOD syndrome, DOORS syndrome, and Zimmermann-Laband syndrome, which share overlapping feature of congenital sensorineural deafness, onychodystrophy, and different extents of intellectual disability without or with epilepsy. However, the underlying mechanisms remain unclear. To investigate the pathological role of mutant ATP6V1B2 in the auditory system, we evaluated auditory brainstem response, distortion product otoacoustic emissions, in a transgenic line o

Hyperacusis is a hearing disorder characterized by decreased sound tolerance. However, diagnostic methods remain debated to take into account both the subjective complaint and an objective measure of hearing impairment. This study aims to propose a quick screening method for hyperacusis using a visual analog scale, combined with hearing assessments to specify the physiological and emotional factors involved in this hearing discomfort. First, a threshold of 6/10 on the visual analogue scale led to a sensitivity of 1 and a specificity of 0.94, on the categorization of a sample of hyperacusis (N

Chronic tinnitus is a prevalent hearing disorder, and yet no successful treatments or objective diagnostic tests are currently available. The aim of this study was to investigate the relationship between the presence of tinnitus and the strength of the middle-ear-muscle reflex (MEMR) in humans with normal and near-normal hearing. Clicks were used as test stimuli to obtain a wideband measure of the effect of reflex activation on ear-canal sound pressure. The reflex was elicited using a contralateral broadband noise. The results show that the reflex strength is significantly reduced in individua

The prevalence of tinnitus is positively correlated with hearing loss, although, tinnitus can also present alongside clinically normal pure-tone thresholds. As standard pure tone audiograms (PTA) only sample at octave or inter-octaves, they potentially can miss lesions between the tested frequencies. Here we investigate if tinnitus patients with normal audiograms have hearing loss missed by standard PTA testing, referred as "missed hearing loss" in the paper. Hearing thresholds in 106 tinnitus patients who have a normal PTA were tested using fine frequency resolution (1/24 octave step) audiome

INTRODUCTION: The phenomenon recently described as "hidden hearing loss" was the subject of a meeting co-hosted by the Department of Defense Hearing Center of Excellence and MIT Lincoln Laboratory to consider the potential relevance of noise-related synaptopathic injury to military settings and performance, service-related injury scenarios, and military medical priorities. Participants included approximately 50 researchers and subject matter experts from academic, federal, and military laboratories. Here we present a synthesis of discussion topics and concerns, as well as specific research obj

The synapse between the inner hair cells (IHCs) and the spiral ganglion neurons (SGNs) in mammalian cochleae is characterized as having presynaptic ribbons and therefore is called ribbon synapse. The special molecular organization is reviewed in this chapter in association with the functional feature of this synapse in signal processing. This is followed by the review on noise-induced damage to this synapse with a focus on recent reports in animal models in which the effect of brief noise exposures is observed without causing significant permanent threshold shift (PTS). In this regard, the pot
