Idiopathic Sudden Sensorineural Hearing Loss in Different Ages: Prognosis of Patients With Initial Total Hearing Loss.

OBJECTIVE: This study aimed to analyze the hearing improvement and prognosis factors of idiopathic sudden sensorineural hearing loss (ISSNHL) in different ages with initial total hearing loss. METHODS: We reviewed the medical records of 5,711 hospitalized patients with ISSNHL from 2016 to 2021 in our center. All of the patients had been treated with uniform combination drug therapy. After excluding the patients with initial partial hearing loss and those diagnosed with clear etiology, 188 patients were enrolled in this study and divided into six age groups (18-30, 31-40, 41-50, 51-60, 61-70,

Audiometric comparison of Lassa fever hearing loss and idiopathic sudden hearing loss: evidence for viral cause.

A recently published prospective study on acute sensorineural deafness in Lassa fever among a West African population showed the audiometric pattern of a known virally induced hearing loss. Using the audiometric data from the patients with Lassa fever in that study, we analyzed and classified the initial hearing loss and final recovery into three groups by pure-tone average values and then did the same for 222 patients with idiopathic sudden hearing loss (SHL) in our study. Statistical analyses of the severity of initial hearing loss and the hearing recovery pattern indicate that the clinical

Catastrophic bilateral sudden sensorineural hearing loss following COVID-19.

Since December 2019, when it was first detected in Wuhan, the SARS-CoV-2 (COVID-19) has spread across the globe. The pandemic has had an unprecedented impact on the global population with >110 million confirmed cases and 2.5 million deaths. The most common initial symptoms of COVID-19 infection are cough, dyspnoea, fever, malaise and anosmia. Severe clinical manifestations include respiratory compromise, pneumonia, organ failure and death. There have been many other less common symptoms as a result of COVID-19 described in the literature, including significant rates of olfactory dysfunction. H

Association between Sudden Sensorineural Hearing Loss and Lyme Disease.

OBJECTIVES: Sudden sensorineural hearing loss (SSNHL) is defined as sensorineural hearing loss of 30 dB or more over at least three adjacent audiometric frequencies occurring within a 72-h period of time. One of the causes of SSNHL could be the progressive inflammatory state caused by an infection. The aim of this study was to assess the prevalence of SSNHL caused by various factors, most importantly those potentially related to Lyme disease. MATERIAL AND METHODS: The study includes a group of 86 patients between the ages of 20 and 70 who were hospitalized due to SSNHL between 2017 and 2018.

Rheopheresis in treatment of idiopathic sensorineural sudden hearing loss.

BACKROUND: Only few therapeutic options exist for patients with refractory sudden idiopathic sensorineural hearing loss (SISHL). Little is known about the efficacy of second-line therapies. Rheopheresis seems to be an effective therapeutic possibility. METHODS: Between 2012 and 2015, 106 patients with SISHL were enrolled in the study, of whom 52 were refractory to initial treatment. As salvage therapy, these patients were offered either 3 sessions of rheopheresis (33 pts) or intratympanic steroid treatment through MicroWick application (19 pts). Pure tone audiometry was performed at diagnosis

Idiopathic sudden sensorineural hearing loss: temporal bone histopathologic study.

We microscopically examined the temporal bones of 12 ears with idiopathic sudden sensorineural hearing loss (iSSNHL), 10 ears with presbycusis, 11 ears with normal hearing, and 8 unaffected contralateral ears of patients with iSSNHL. The degeneration of the spiral ligament, vascular stria, hair cells, dendrites, and apical spiral ganglion cells was greater in ears with iSSNHL than in the other groups. The apical ganglion cells were significantly more affected than the basal ganglion cells, and the spiral ganglion cell loss increased as a function of duration of iSSNHL. Cochlear ossification wa

[Study of trough type audiogram of sudden hearing loss].

We investigated 974 patients with sudden sensorineural hearing loss who consulted the otolaryngological service of Hamamatsu University Hospital and four affiliated hospitals from 1984 to 1992. Among them, we selected 569 new cases showing a pure tone average of 31 dB or more at 250 Hz, 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz on the affected side in the initial audiometry performed within 14 days after the onset of sudden sensorineural hearing loss. The 569 patients consisted of 13 with acoustic neuromas (SHLANs), 493 with idiopathic sudden sensorineural hearing loss (ISHLs) and 63 others. Ten of

Diatrizoate and dextran treatment of sudden sensorineural hearing loss.

Many treatments have been proposed to manage sudden sensorineural hearing loss. The University of Iowa has used a protocol of intravenous radiopaque contrast and volume expander for the last 10 years in the treatment of sudden sensorineural hearing loss. A retrospective analysis of these cases was performed to review the efficacy of this treatment with respect to the natural history of the disease, as reported in previous studies. Sixty-four percent of 39 cases showed audiometric improvement in their pure-tone averages, speech reception thresholds, and speech discriminations while receiving tr

Sudden hearing loss with delayed onset following head trauma.

Eight cases are reported with an identical history: head trauma with immediate profound deafness and vestibular dysfunction in one ear; then after a long latency, sudden hearing loss in the second ear, recurring and fluctuating with vestibular symptoms. Two cases were surgically explored. In one case 15 years after the trauma a perilymph fistula of the round window was found. In the other case a broad fracture of the basal coil of the cochlea was demonstrated, which had been asymptomatic for 18 years. In a retrospective study, 6 more cases with an identical course could be found. Latency betwe

Perilymph oxygenation in sudden and progressive sensorineural hearing loss.

The perilymphatic oxygen tension of the vestibular perilymph of 34 patients with different inner ear losses were measured with the polarographic method before and after inhalation with carbogen. Two patterns of disturbed perilymphatic oxygenation were observed: 1) Characterized by low initial values of perilymphatic oxygenation and normal response to carbogen inhalation was present in sudden deafness and sudden cochleovestibular loss of inner ear function. 2) Characterized by normal initial values of perilymphatic oxygenation and a low response to carbogen, was found in presence of a slowly pr

The double-membrane break syndrome in sudden hearing loss.

Some patients with sudden hearing loss actually have two membrane breaks, one at the oval or round window and one further inside the cochlea. One may heal spontaneously and the other not, or both, or neither. It is very likely impossible to detect this early from the threshold audiogram, because the intracochlear breaks allows endolymph and perilymph mixing which spreads over much of the anatomically normal clchlea causing diffuse loss of function. This theory is illustrated by three case histories.

[From sudden hearing loss to an inflammatory systemic disease].

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides are a group of rare autoimmune disorders characterized by necrotizing inflammation of small- to medium-sized blood vessels. They typically involve the upper and lower respiratory tracts and frequently the kidneys. However, they may also present with otologic manifestations, which can occasionally occur as an early or even the sole clinical symptom. The following case report outlines the course of the disease in a patient in whom bilateral hearing loss ultimately emerged as the leading manifestation of ANCA-associated vasculiti

GRβ Regulates Glucocorticoid Resistance in Sudden Sensorineural Hearing Loss.

BACKGROUND: In recent years, the incidence of sudden deafness has gradually increased, with a very limited understanding of its etiology and pathogenesis. Glucocorticoids are the first choice of the treatment, but some hormone-resistant patients are not sensitive to glucocorticoid therapy. The pathogenesis is not yet known. In this study, we aim to construct the HEI-OC1 cell line stably overexpressing Glucocorticoid Receptor Beta (GRβ), and identify its exact role in the cases of glucocorticoidresistant sudden deafness. METHODS: We used the endotoxin lipopolysaccharide-stimulated cochlear hai
