Atrial fibrillation-associated remodeling does not promote atrial thrombus formation in canine models.

BACKGROUND: The most important complication of atrial fibrillation (AF) is thromboembolic stroke. Although AF-related remodeling is considered important in atrial thrombogenesis, its role never has been directly tested. This study assessed effects of AF-related remodeling on the atrial thrombogenic milieu by using radiofrequency ablation (RFA) to create a quantifiable prothrombotic nidus. METHODS AND RESULTS: We studied normal control dogs (control, n=16) and 3 canine AF-models: (1) atrial tachycardia remodeling (ATR; n=16) induced by atrial tachypacing (400 bpm for 1 week, with atrioventricu

Transoesophageal echocardiography improves the diagnostic value of cardiac ultrasound in patients with carcinoid heart disease.

Transthoracic and transoesophageal cardiac echocardiography and Doppler investigations were performed in 31 consecutive patients with malignant midgut carcinoid tumours. The transoesophageal images allowed measurement of the thickness of the atrioventricular valve leaflets and the superficial wall layers on the cavity side of both atria. The mean thickness of the anterior tricuspid leaflet was significantly greater than that of the mitral valve--a difference not seen in a control group of age-matched patients without carcinoid tumours and with normal cardiac ultrasound findings. In addition, t

Cellular disorganization and extensive apoptosis in the developing heart of mice that lack cardiac muscle alpha-actin: apparent cause of perinatal death.

Mice that lack cardiac muscle alpha-actin die during the perinatal period. Approximately 56% of mice that are homozygous null (-/-) for a functional cardiac alpha-actin gene do not survive to term, and the remainder generally die within 2 wk of birth. We found that there were neither morphologic differences nor differences in the extent of apoptosis between the mutant and normal hearts on embryonic day (E) 12 and E14 of development. However, apoptosis was greater in the hearts of homozygous null mice on E17 and postnatal day 1 when compared with wild-type hearts. The antiapoptotic factor Bcl-x

B cells promote atrial fibrillation via autoantibodies.

Atrial fibrillation, the most frequent cardiac arrhythmia, causes heart failure and stroke. Here we describe that combining the typical risk factors of atrial fibrillation (hypertension, obesity and mitral valve regurgitation (HOMER)) activates adaptive immunity in wild-type mice, ultimately causing electrical remodeling of cardiomyocytes. In HOMER mice, dendritic cells expanded in the left atria and heart-draining lymph nodes, where we detected cardiomyocyte-derived proteins. Systemically expanding B cells, while exposed to interferon-α, produced autoantibodies that disrupted calcium handling

Competitive inhibition of phosphodiesterase activity by amrinone: its implication in the cardiac effect of the drug.

The effects of amrinone on partially purified bovine heart PDE and on resting tension of isolated guinea-pig left atria were investigated. Amrinone strongly inhibited the high affinity Mg2+ dependent PDE activity present in the 105000xg supernatant fraction of heart homogenate (IC50 = 60 microM), but only weakly blocked the Ca2+-CaM-dependent PDE (IC50 = 2 mM). The inhibition of high affinity PDE was competitive in nature. In guinea-pig left atria driven at 1 Hz, amrinone induced a concentration-dependent positive inotropic effect (EC50 = 200 microM). This action was not followed by a rise of

New-onset atrial fibrillation after coronary surgery and stroke risk: a nationwide cohort study.

BACKGROUND: New-onset postoperative atrial fibrillation (POAF) after coronary artery bypass grafting (CABG) increases ischaemic stroke risk, yet factors influencing this risk remain unclear. We sought to identify factors associated with 1-year ischaemic stroke risk, compare the CHA2DS2-VASc (Congestive heart failure, Hypertension, Age ≥75 years, Diabetes, previous Stroke/transient ischaemic attack (TIA), Vascular disease, Age 65-74 years, Sex category) and ATRIA (Anticoagulation and Risk Factors in Atrial Fibrillation) scores' predictive abilities for ischaemic stroke, and assess oral anticoag

Cardiac connections: the segmental approach to radiologic diagnosis in congenital heart disease.

The segmental approach to the diagnosis and classification of congenital heart disease, which emphasizes the importance of the connections (proximal-to-distal relationships) of cardiac chambers and great vessels, is presented. The radiologic identification of great vessels and cardiac chambers and their connections is described, considering, in turn, three major segments: the atria together with the systemic and pulmonary veins, the ventricles with their atrioventricular valves and the great arteries with their outflow tracts. Cardiac and great vascular connections can be described with greate

Professor of Cardiology, SGPGIMS, Lucknow, Uttar Pradesh.

Atrial fibrillation (AF) is the most common supraventricular tachycardia and its incidence increases with age. The pathophysiology of AF has been studied extensively and is a subject of continuing research. The primary pathologic change seen in AF is progressive fibrosis of the atria and hence structural remodeling, is the mainstay in many forms of AF. Dilation of the atria can be due to almost any structural abnormality of the heart which includes valvular heart disease, hypertension or congestive heart failure. Electrical remodeling promotes AF by acting on fundamental arrhythmia mechanism:

Cardiac pacing in intensive care.

Cardiac pacing techniques and equipment have developed dramatically in recent years. Bradycardias and tachycardias may be effectively treated by pacing. Bradyarrhythmias: It is generally accepted that pacing is indicated for a sustained symptomatic bradycardia. Prophylactic pacing for 'high-risk' bundle branch block in acute myocardial infarction is more controversial. A new era in cardiology has been introduced with the advent of 'physiological pacing', i.e. pacing of the heart with the maintenance of atrioventricular synchrony and varying the heart rate according to the body's metabolic lead

Unusual giant right atrium in rheumatic mitral stenosis and tricuspid insufficiency.

Dilation and hypertrophy of the atria occur in patients with valvular heart disease especially in mitral regurgitation, mitral stenosis or tricuspid abnormalities. In sub-saharan Africa, rheumatic fever is still the leading cause of valvular heart disease. We report a case of an unusual giant right atrium in context of rheumatic stenosis and severe tricuspid regurgitation in a 58-year-old woman.

Ultrastructural observations of the fetal rat heart during prolonged gestation.

The ultrastructure of myocardial cells was examined from fetal rat hearts during experimentally induced prolonged gestation (GD 23, 24 and 25), and compared to those from term fetuses (GD 22) and newborn animals (N1, N2, N3). Gestation was prolonged by daily subcutaneous injections of 5 mg progesterone to gravid females on days 20 through 24. The 4 chambers of the heart were assessed individually for structural differences by morphometric analysis. Comparison between the 2 atria at each stage revealed no differences as was the case between the 2 ventricles. At term and throughout the newborn p

Single Ventricle: Radiology Primer for Preprocedural Evaluation.

Single ventricle is an uncommon congenital abnormality characterized by a single anatomic or functional ventricle that cannot sustain two separate serial circulations. Imaging plays an important role in classifying a single ventricle, quantitating ventricular and valvular function, detecting associated cardiovascular anomalies, and identifying critical factors for management. Echocardiography is the first-line imaging modality. MRI and CT are useful when echocardiography is indeterminate or suboptimal, especially for evaluation of vascular anatomy. Morphologically, two atria can connect to a s

Ventrally emigrating neural tube cells contribute to the normal development of heart and great vessels.

We investigated the contributions of a recently described population of neural tube cells, which participates in the development of a variety of tissues, to the development of the heart and great vessels. These cells, termed as the ventrally emigrating neural tube (VENT) cells, originate in the ventral part of the hindbrain neural tube, emigrate at the site of attachment of the cranial nerves, and populate their respective target tissues. VENT cells of the caudal hindbrain neural tube at the level of the vagus nerve, which were previously reported to migrate into the heart, were tagged with re

Quantitative description of the regional mechanics of the left atria by electroanatomical mapping.

The left atrium is a complex chamber, which plays an integral role in the maintenance of physiologic hemodynamic and electrical stability of the heart and is involved in many disease states, most commonly atrial fibrillation. Preserving regions of the left atrium that contribute the greatest to atrial mechanical function during curative strategies for atrial fibrillation are important. We present here a new application of the CARTO electroanatomical mapping system in the assessment of the left atria mechanical function. Electroanatomical data were collected in course of the electrophysiologica

The effect of theophylline on amine-induced cardiac cyclic AMP and cardiac contractile force.

In the isolated electrically driven rat atria, theophylline (5 X 10(-4)M) produced a small but significant increase in cyclic AMP content which was prevented by reserpine pretreatment. Theophylline was also found to exert a direct contractile effect, unrelated to cyclic AMP, in atria obtained from reserpine-pretreated animals. The norepinephrine inotropic response was attenuated after 3 min, enhanced after 15 min, and abolished after 60 min of exposure to theophylline (5 X 10(-4) M). The maximum phenylephrine inotropic response was not significantly changed after 15 min of exposure to theophyl

Atrial natriuretic peptide in patients with heart failure and chronic atrial fibrillation: role of duration of atrial fibrillation.

The purpose of this study was to analyze the determinants of atrial natriuretic peptide level in patients with congestive heart failure and atrial fibrillation. In particular, the duration of atrial fibrillation was analyzed because atrial fibrillation per se might have a specific effect on atrial natriuretic peptide level. The study group consisted of 26 patients with mild to moderate stable congestive heart failure and chronic atrial fibrillation. Plasma samples of atrial natriuretic peptide were obtained at rest and were analyzed with radioimmunoassay. Median atrial natriuretic peptide leve

Efficacy of Sacubitril/Valsartan in a Patient With Heart Failure and Impaired Secretion of Atrial Natriuretic Peptide Due to Long-Standing Persistent Atrial Fibrillation.

Atrial natriuretic peptide (ANP) is a circulating hormone released from the atria in response to wall stretch and volume overload in the setting of heart failure. When atrial fibrillation (AF) becomes long-standing persistent, ANP secretion in response to volume overload is impaired due to degenerative changes of the atria. Here, we report a case of heart failure with preserved ejection fraction and impaired ANP secretion due to long-standing AF. N-terminal pro-brain natriuretic peptide (NT-proBNP) level was elevated (269 pg/mL), whereas the increase in ANP level was marginal (46.1 pg/mL), sug

Morphological perspectives of the heart of Rhinella marina: Contributions to comparative anatomy and evolutionary biology.

BACKGROUND: Rhinella marina, an anuran with established invasive populations in the Amazon biome, has a tricavitary heart with two atria and a single ventricle. Given its invasive tendencies and the management challenges it poses in certain areas, a detailed morphological study of this species is important for understanding its biology and for comparisons across taxa. METHODS: We examined ten adult individuals using mesoscopic techniques, light microscopy (LM), and scanning electron microscopy (SEM). RESULTS: The heart occupies the midline region of the body, dorsal to the sternum, between t

[Esophageal stimulation of the heart: importance of the artifact inhibition system impulse in the quality of the ECG tracing from the stimulating electrode in the esophagus].

The use of diagnostic possibilities of oesophageal stimulation of the heart depends on obtaining a readable and stable electrocardiogram from the stimulating electrode. The objective of the evaluating the importance of the electronic system eliminating the artefact of the stimulating impulse from the oesophageal tracing was to compare the quality of records obtained by the new and the traditional method. For stimulation an oesophageal cardiostimulator SP-5 manufactured by OBR, TEMED Zabrze was used. The atria and ventricles were stimulated by a fixed frequency and individual timed stimuli. In

[Value and methods of temporary stimulation of the heart to prevent dangerous arrhythmias].

The authors assessed the efficacy of the prevention of frequently or continuously relapsing supraventricular (7 cases) and ventricular (8 cases) tachycardia as well as ventricular fibrillation (16 patients) with the help of overdrive pacing. In 28 out of 31 patients (90.3%) the therapy was successful in preventing arrhythmia relapses including patients with the syndrome of an increased QT interval. Overdrive pacing of the atria was more effective as compared to that of the ventricles (93% versus 71.4%). The effective frequency of overdrive pacing of the atria was 40.5% lower as against ventric
