Fetal heart rate responses to anemia in Rh isoimmunization.

On 52 occasions 24 Rh immunized women were monitored with a nonstress test (NST) prior to fetal blood sampling. Cardiotocographic characteristics were recorded for each NST. Fetal blood was analysed for hemoglobin and hematocrit. Fetal hemoglobin and hematocrit were positively correlated to long-term variability, acceleration amplitude and negatively correlated to deceleration amplitude (linear regression analysis; p less than 0.05). Decelerations were almost without exception associated with low concentrations of hemoglobin and hematocrit. In fetuses of 32 weeks' gestation or more, a loss of

Abnormal antepartum fetal heart rate patterns and subsequent handicap.

In this paper data on abnormal antepartum FHR patterns are related to the state of fetal oxygenation, fetal brain abnormalities and to neurological outcome. It is concluded that in IUGR fetuses changes in heart rate (and movement) patterns are late signs of impairment. Antepartum heart rate decelerations are usually the first of the abnormalities detected and are associated with fetal hypoxaemia. A fixed or flat FHR pattern might be indicative of congenital malformations of the brain or of prenatally acquired encephalopathy. Several studies have shown that antepartum FHR abnormalities (usually

Spontaneous prolonged contractions during antenatal fetal heart rate monitoring.

Uterine contractions of more than 2 minutes' duration occurred spontaneously in 1.4% of 6,399 antenatal fetal heart rate (FHR) recordings. All recordings were made in the Department of Obstetrics and Gynaecology at Tygerberg Hospital. The patients with spontaneous prolonged contractions (SPC) were compared with a randomly selected control group. Patients who had an SPC during antenatal FHR recording were not at a higher risk of abruptio placentae, preterm labour or unexpected intra-uterine death. Of the infants born to the mothers in the study group, 65.5% were male. There were more primigravi

Fetal heart rate tracings in fetuses with congenital malformations.

The fetal heart rate (FHR) tracings of 73 fetuses with gross congenital malformations were surveyed. Fifty-four percent of the tracings were pathologic. The most common pathologic tracing patterns were baseline bradycardia (30%), variable decelerations (25%), baseline tachycardia (22.5%) and decreased variability (20%). There was no characteristic tracing pattern for the whole group or any specific subgroup of malformations. The highest rates of pathologic FHR tracings were found in the groups with multiple malformations (83.3%), chromosomal aberrations (81.8%) and central nervous system lesio

Fixed fetal heart rate pattern after intrauterine accidental decerebration.

A fixed fetal heart rate (FHR) pattern, with a normal frequency without accelerations and decelerations can be observed in early pregnancy during the inactive episodes of the rest-activity cycle, in association with certain drugs administered to the mother and in some cases of anencephaly. In the present case report a fetus is described in which a persistent fixed FHR pattern was indicative of decerebration in utero. The latter occurred after a serious maternal respiratory and cardiovascular collapse. The discrepancy between maternal and fetal outcome and the pathophysiologic backgrounds under

Second stage fetal heart rate abnormalities and neonatal acidosis.

Of 403 newborns with either fetal heart rate (FHR) bradycardia or tachycardia during the last ten minutes of the second stage of labor, 20% had an umbilical cord pH of less than 7.20 at birth compared with only 4% of newborns who had a normal FHR during the last ten minutes of labor (P less than .0001). Thirty (18%) of 165 neonates with mild bradycardia and 33 (27%) of 121 neonates with moderate to marked bradycardia had cord pH values of less than 7.20 at birth (P less than .0001). Of the 117 neonates with tachycardia during the last ten minutes of the second stage, 17 (15%) had a pH of less

Characteristic fetal heart rate changes in severe rhesus isoimmunization.

A case of pregnancy complicated by severe rhesus isoimmunization is described. The finding of a sequence of fetal heart pattern changes on daily cardiotocography offers a simple method of detecting the severely compromised fetus, which appears to be related to the underlying pathological process, and is more sensitive than other tests of fetal well-being currently used.

Prospective study of fetal heart rate and rhythm patterns.

The fetal heart beat, detected by ultrasound, was recorded for five minutes from 934 antenatal women--266 at 30 to 35 weeks' gestation and 718 at 36 to 41 weeks. Episodes of bradycardia < 100/minute ranging from 5-30 seconds' duration occurred in 3 (1.1%) of the former and in 9 (1.3%) of the latter group. Tachycardia > 180/minute ranging from 30-90 seconds' duration occurred in 1 (0.4%) of the former and in 4 (0.6%) of the latter. Premature beats were not detected between 30 and 35 weeks, but occurred in 12 (1.7%) of 720 at 36 to 41 weeks' gestation. 50 subjects were monitored at both 30 and 3

Sinusoidal fetal heart rate patterns in severe Rh disease.

Four patients with severe Rh isoimmunization are presented in whom sinusoidal fetal heart rate patterns were recorded. In 2 cases in which the pattern was seen intermittently in the postintrauterine transfusion period, the perinatal outcome was good. In the 2 cases in which the pattern was seen spontaneously and continuously, the outcome was poor, confirming the ominous and possibly agonal significance of the latter pattern.

[Influence of umbilical cord entanglement on fetal heart rate].

Umbilical cord entanglement was found in 34% of 555 women in labour. One loop of cord around the neck of the fetus was seen in 29% of the women and two or more loops in 5%. Cardiotocograms of these patients were examined for abnormal heart rate patterns. Cord involvement did not influence the incidence of basal bradycardia or tachycardia, early or late decelerations, loss of beat-to-beat variation or acceleration patterns. Variable decelerations, however, were seen in 23% of the patients with cord entanglement and in 7% without cord involvement. This difference is statistically significant (P

[The effect of maternal exercise on the fetal heart rate].

The effect of maternal exercise (the two-step test of Master) on the fetus was studied in 28 pregnant women. These subjects consisted of 5 cardiac cases, 5 cases with IUGR, 5 cases with preeclampsia and 13 normal pregnant women. Overt deceleration of the FHR was found after exercise only in 3 cardiac cases whose FHR tracings had been normal before. Slight changes such as a small rise or a small fall in FHR after exercise were seen in 4 of 7 cases who had shown decelerations in FHR before, and in a normal case. Fetal tachycardia appeared in 2 normal cases immediately before exercise; in one cas

Ambulatory Fetal Heart Rate and Rhythm Monitoring for Pregnancies at Risk for Fetal Arrhythmias: Perspectives from the Fetal Heart Society.

Fetal arrhythmias, including tachycardias, bradycardias and irregular rhythms, may require close surveillance and management to prevent fetal heart failure, premature delivery and stillbirth. With early detection and appropriate management of pathological fetal rhythms, 90-95% of fetuses with even high-risk arrhythmias are live-born and can be delivered at term gestation. Fetal heart rate monitoring (FHRM) is a feasible method for ambulatory detection of new-onset or recurrent arrhythmias. When indications for FHRM are met, prescribed monitoring, guided and supported by the healthcare team, ca

Abnormal fetal heart rate patterns caused by pathophysiologic processes other than fetal acidemia.

Fetal acidemia is a common final pathway to fetal death, and in many cases, to fetal central nervous system injury. However, certain fetal pathophysiological processes are associated with significant category II or category III fetal heart rate changes before the development of or in the absence of fetal acidemia. The most frequent of these processes include fetal infection and/or inflammation, anemia, fetal congenital heart disease, and fetal central nervous system injury. In the presence of significant category II or category III fetal heart rate patterns, clinicians should consider the poss

Clinical associations of fetal heart rate accelerations as derived from transabdominal fetal electrocardiograms.

OBJECTIVE: To determine the value of quantifying accelerations of the fetal heart rate (FHR), as collected non-invasively during pregnancy, as a proxy for fetal movements. STUDY DESIGN: The study consists of a prospective collection of research material with retrospective analyses of the collected fetal electrocardiograms (ECGs), done in a homogeneous population in a low socioeconomic residential area of Cape Town, South Africa, as part of the Safe Passage Study. Recruitment and follow-up were done from August 2007 to August 2016. Maternal and fetal ECGs were collected non-invasively at vario

Intermittent auscultation (IA) of fetal heart rate in labour for fetal well-being.

BACKGROUND: The goal of fetal monitoring in labour is the early detection of a hypoxic baby. There are a variety of tools and methods available for intermittent auscultation (IA) of the fetal heart rate (FHR). Low- and middle-income countries usually have only access to a Pinard/Laënnec or the use of a hand-held Doppler device. Currently, there is no robust evidence to guide clinical practice on the most effective IA tool to use, timing intervals and length of listening to the fetal heart for women during established labour. OBJECTIVES: To evaluate the effectiveness of different tools for IA

Association of non-reassuring fetal heart rate and fetal acidosis with placental histopathology.

OBJECTIVE: To investigate the association between different placental lesions and non-reassuring fetal heart rate (NRFHR) pattern and fetal acidosis in labor. STUDY DESIGN: Placentas from 213 women who underwent cesarean section because of NRFHR with or without fetal acidosis (pH < 7.2) were classified by histopathologic findings: consistent with maternal circulation abnormalities i.e., namely, marginal or retroplacental hemorrhage (M0), maternal underperfusion, vascular (M1) or villous changes (M2), and those consistent with fetal thrombo-occlusive disease due to vascular (F1) or villous (F2

Checkmark fetal heart rate pattern associated with severe fetal hypoxia. A case report.

A 31-year-old nulliparous woman developed vaginal bleeding after spontaneous rupture of the membranes at 40 weeks of gestation. Fetal heart rate monitoring showed checkmark and sinusoidal patterns. A 3,281 g male neonate was delivered by emergency cesarean section, with Apgar scores of 1 and 3 at 1 and 5 min, respectively. The neonate exhibited severe acidosis with arterial blood gas pH of 6.96, and was diagnosed to have severe hypoxic-ischemic encephalopathy. An umbilical artery branch was noted in the membrane near the cervical os and found to be torn, representing hemorrhagic vasa previa. S
