BACKGROUND: Geriatric intertrochanteric fractures remain a major public health concern due to the considerable disability, morbidity, mortality, and health care costs associated with these injuries. The underlying poor bone quality and unstable nature of these fractures makes them difficult to treat. The main purpose of this study was to evaluate the outcome of hemiarthroplasty for unstable intertrochanteric hip fractures when compared to the traditional treatment options of open reduction internal fixation (ORIF).
METHODS: A retrospective 1:1 matched cohort of 150 patients who had intertroch

The purpose of this study was to evaluate a multimodal pain management program incorporating periarticular injections of liposomal bupivacaine after hemiarthroplasty treatment of femoral neck fractures. This retrospective study selected patients treated with periarticular injections of liposomal bupivacaine within the multimodal pain management program (LBUP) (n = 100) and a control group of patients treated without local infiltration (n = 78). Similar pain control was achieved between both groups from day 1 to day 4 postsurgery (min p = .392). Length of stay was significantly lower for LBUP p

BACKGROUND: Bipolar hemiarthroplasty for unstable intertrochanteric fractures in elderly patients is a viable option that can prevent the complications of an open reduction, such as nonunion and metal failure. This study evaluated the clinicoradiological results of cementless bipolar hemiarthroplasty for unstable intertrochanteric fractures in elderly patients.
METHODS: Forty hips were followed for more than 2 years after cementless bipolar hemiarthroplasty using a Porocoat® AML Hip System. The mean age was 78.8 years and the mean follow-up period was 40.5 months. The Harris hip score and pos

Infection following hemiarthroplasty of the hip is a devastating complication associated with significant morbidity. While two-stage revision total hip replacement (THR) is the gold standard for chronic periprosthetic joint infection (PJI) in total hip arthroplasty, literature focusing on infected hemiarthroplasty remains limited. This study evaluates the clinical, functional, and radiological outcomes of two-stage revision THR in patients with infected hemiarthroplasty. This retrospective level IV study, conducted in a single center, included 23 patients with confirmed PJI following hemiarthr

Femur neck fractures are among the most common presentations seen in trauma centers. Surgical fixation of the fracture versus hip joint arthroplasty is the standard modality of treatment. The appropriate treatment modality is decided based on the patient's fracture pattern, activity level prior to the injury, fitness for surgery, and availability of resources. Total or hemi hip arthroplasties are effective and well-optimized procedures. However, they do not come without the risk of complications, some of which can cause significant morbidity to the patient. We present a case of a vascular comp

AIMS: After hip arthroplasty surgery, hip precautions are often implemented to minimise the risk of dislocation, although existing evidence does not support their effectiveness. At our institution, we replaced post-operative precautions with a novel pose avoidance protocol in patients undergoing hip hemiarthroplasty for neck of femur fracture. The objectives of our study were to validate this new protocol by assessing the incidence of dislocation before and after its introduction and evaluating its impact on the length of hospital stay.
METHODS: Between 20th September 2021 and 19th March 2023

Introduction Metatarsophalangeal joint (MTPJ) arthropathy in the great toe causes considerable pain and debilitation, severely impacting quality of life. Traditional management options included conservative measures, steroid injections, and arthrodesis. New options include Cartiva, a synthetic cartilage hemiarthroplasty for the MTPJ. This prosthesis has evidence of improved pain and function without the sacrifice of joint movement seen with arthrodesis. However, the implant itself has mixed reviews. This study aims to identify the pre-, peri, and short-term post-surgical outcomes of Cartiva su

The optimal treatment of recent femoral neck fractures remains debated. The available options are internal fixation, hemiarthroplasty (HA) and total hip arthroplasty (THA). There is a consensus in favour of internal fixation in younger patients. In elderly individuals who are institutionalised and have limited physical activity, HA is usually performed when the joint line is intact. Whether HA or THA deserves preference in patients aged 60 years or over is unclear. In addition, there are two types of HA, unipolar and bipolar, and two types of THA, conventional and dual-mobility. Both HA types

A 70-year-old female patient, known case of hypertension and incomplete Left bundle branch block (LBBB), posted for bipolar hemiarthroplasty. A geriatric patient with LBBB and systemic hypertension is a fatal combination; it may precipitate into complete AV dissociation during anaesthesia and surgical stress. We are reporting this interesting case as our patient had developed complete Atrioventricular (AV) dissociation when scheduled earlier for surgery and got postponed. We had given combined spinal epidural anaesthesia and managed successfully without any complication.

Although a long-stemmed cemented hemiarthroplasty is frequently recommended for oncologic lesions of proximal femur, we have observed an alarming number of spontaneous stem fractures. The purpose of this retrospective study was to identify the associated risk factors for stem fractures in a study cohort of 60 (61 prostheses) during 1983-2007. At a mean follow-up of 41 months, 4/61 (6.6%) stems had fractured after a mean of 36 (12-92) months after surgery. All failed implants were Osteonics Omnifit (4/27; 14.8%) and multivariate analysis did not show any correlation with other studied variables

Primary articulation of bipolar hemiarthroplasty systems is at the femoral head-liner interface. The purpose of this study was to compare observed damage modes on 36 retrieved bipolar systems with implant, demographic, intraoperative, and radiographic data to elucidate the effects of component design, specifically locking mechanism, on clinical performance. Retrieved bipolar hip hemiarthroplasty systems of 3 different design types were obtained, disassembled, and evaluated macro- and microscopically for varying modes of wear, including abrasion, burnishing, embedding, scratching, and pitting.

BACKGROUND: Although not all elderly patients with femoral neck fractures are candidates for THA, active, mentally competent, independent patients achieve the most durable functional scores with THA compared with hemiarthroplasty. However, a relatively high frequency of early or late dislocation could reduce the potential benefits with THA.
QUESTIONS/PURPOSES: We asked whether the incidence of first-time, recurrent dislocation, and revision differed in patients with hip fractures having THA or hemiarthroplasty.
PATIENTS AND METHODS: We retrospectively reviewed 380 patients with hip fractures

INTRODUCTION: Hip fracture is a serious injury in adults, especially those aged over 60 years. The most common type of hip fracture (displaced intracapsular) is treated for the majority of patients with a partial hip replacement (hemiarthroplasty). The hemiarthroplasty implant can be fixed to the bone with or without bone cement. Cement is the current recommended technique but recently some risks have been identified, which could potentially be avoided by using uncemented implants. Controversy, therefore, remains about which type of hemiarthroplasty offers patients the best outcomes. This is th

PURPOSE: Advances in cancer treatment have prolonged survival, but the incidence of bone metastases has also increased. Shoulder arthroplasty for proximal humeral metastases involves major surgical trauma and uncertain functional outcomes in older patients. This study aimed to assess whether age influences prognosis and functional recovery after shoulder arthroplasty.
METHODS: Fifty-three patients with isolated proximal humeral metastases undergoing arthroplasty were retrospectively reviewed. Patients were divided into Group A (≥65 years, n = 25) and Group B (<65 years, n = 28). Surgical outc
