Advanced hemodynamic monitoring of the patient with acute heart failure and cardiogenic shock.

PURPOSE OF REVIEW: To provide a practical, physiology-driven framework for advanced hemodynamic monitoring in patients with acute heart failure (AHF) and cardiogenic shock, highlighting and supporting clinicians in integrating monitoring tools into an individualized bedside approach. RECENT FINDINGS: Cardiogenic shock is increasingly recognized as a heterogeneous syndrome involving not only impaired cardiac output, but also systemic inflammation, vasoplegia, tissue hypoperfusion and multiorgan dysfunction. Recent studies have emphasized phenotype-based approaches for both diagnosis and treatm

Non-invasive prediction of massive transfusion during surgery using intraoperative hemodynamic monitoring data.

OBJECTIVE: Failure to receive prompt blood transfusion leads to severe complications if massive bleeding occurs during surgery. For the timely preparation of blood products, predicting the possibility of massive transfusion (MT) is essential to decrease morbidity and mortality. This study aimed to develop a model for predicting MT 10 min in advance using non-invasive bio-signal waveforms that change in real-time. METHODS: In this retrospective study, we developed a deep learning-based algorithm (DLA) to predict intraoperative MT within 10 min. MT was defined as the transfusion of 3 or more un

Hemodynamic monitoring using thoracic bioimpedance - an optimal solution for the treatment of hypertension.

Hypertension is a major issue of public health because of its increasing prevalence and multiple complications caused by failing to achieve an efficient blood pressure control. Considering hypertension as a hemodynamic disorder allows to prescribe a tailored therapy guided by individual hemodynamic parameters, therefore leading to an increased rate of control. We present the case of a 59 years old diabetic, dyslipidemic and obese male who, although treated with 5 classes of antihypertensive drugs had uncontrolled hypertension that caused left ventricular failure. Using the HOTMAN system of hem

Lung and cardiac ultrasound for hemodynamic monitoring of patients with severe pre-eclampsia.

OBJECTIVE: To evaluate lung and cardiac ultrasound for the assessment of fluid tolerance and fluid responsiveness before and after delivery in pregnant women with severe pre-eclampsia (PE). METHODS: This was a prospective observational study of singleton pregnant women with severe PE and healthy term controls. Lung ultrasound echo comet score (ECS), which denotes the amount of extravascular lung water, was obtained using the 28-rib interspaces technique. The echocardiographic E/e' ratio, measured by pulsed-wave and tissue Doppler, was used as a marker of diastolic left ventricular function. F

Hemodynamic monitoring: To calibrate or not to calibrate? Part 2--Non-calibrated techniques.

There is much evidence that fluid overload leads to adverse outcomes in perioperative and critically ill patients. Cardiac output monitoring can help us guiding initial and ongoing fluid resuscitation and can help us to assess whether a patient will be responsive to fluids when hypotensive. In recent years, many sophisticated devices that measure a variety of hemodynamic parameters have evolved on the market. We wanted to provide an overview of the different techniques available today, including their validation in different patient populations. In this second part of the review, we focus on n

Hemodynamic monitoring and pulse oximetry during percutaneous gastrostomy and jejunostomy: necessity or nuisance?

Hemoglobin desaturation and arrhythmias have been reported during diagnostic gastroscopy and colonoscopy. During endoscopic placement of percutaneous gastrostomy tubes (PEG), the gastroscope is inserted twice and the stomach is markedly distended, thus increasing the potential risk of complications. Also, the procedure is frequently performed in patients exhibiting underlying systemic disease or little respiratory reserve. Patients undergoing PEG or percutaneous endoscopic jejunostomy (PEJ) were studied prospectively. Blood pressure, pulse, pulse oximetry, and lead II ECG tracings were obtaine

[Uses and risks of hemodynamic monitoring by inflow catheter in acute myocardial infarct].

Using hemodynamic monitoring by flow-directed heart catheterization in acute myocardial infarction, left ventricular dysfunction can be analyzed and quantified. Differential therapy concerning substitution of volume and administration of drugs with influence on pre- and after-load is facilitated. Since the introduction of the Swan-Ganz flow-directed catheter the monitoring has been technically improved with the additional advantage of lower incidence of catheter-induced arrhythmias and the possibility to measure the pulmonary capillary wedge pressure. The Swan-Ganz thermodilution catheter furt

Monitoring of Hemodynamics With Right Heart Catheterization in Children With Pulmonary Arterial Hypertension.

Background Right heart catheterization (RHC) is a high-risk procedure in children with pulmonary arterial hypertension without clear guidelines for the indications and targets of invasive reassessment. Our objectives are to define the aims of repeated RHC and evaluate the correlation between noninvasive criteria and hemodynamic parameters. Methods and Results Clinical and hemodynamic characteristics from 71 incident treatment-naïve children (median age 6.2 years) with pulmonary arterial hypertension who had a baseline and reevaluation RHC were analyzed. Correlations between noninvasive predict

Hemodynamic Monitoring for the Evaluation and Treatment of Shock: What Is the Current State of the Art?

Hemodynamic monitoring has become a fundamental and ubiquitous, if not defining, aspect of critical care medicine practice. Modern monitoring techniques have changed significantly over the past few years and are now able to rapidly identify shock states earlier, define the etiology, and monitor the response to therapies. Many of these techniques are now minimally invasive or noninvasive. Basic hemodynamic monitoring and evaluation usually includes a focused physical examination and static hemodynamic vital signs: temperature, heart rate, respiratory rate, mean arterial pressure, and arterial h

[Intraoperative monitoring of cerebral function and carotid hemodynamics during carotid endarterectomy].

The authors investigated the hemodynamics and monitored the cerebral function to perform the carotid endarterectomy (CEA) safely. The hemodynamics were investigated by measurements of carotid arterial blood flow by an electromagnetic flow meter before and after CEA. And a doppler flow meter applied directly to the carotid arteries to analyze the flow parameters such as peak frequencies (PF), mean frequencies (MF), mode frequencies (Mo F) and % window. We used routinely our specially designed shunt system during surgery, by which stump pressure of the ICA could be measured easily. The cerebral

Usability Validation of an Integrated Hemodynamic and Pulmonary Monitoring System Using Eye-Tracking Analysis.

Background/Objectives: Hemodynamic monitoring is essential for guiding appropriate treatment by assessing cardiac output and volume status, as well as for preventing complications associated with excessive fluid administration. The EdgeFlow CW10 Plus is a device that extends conventional hemodynamic monitoring by incorporating pulmonary abnormality surveillance through B-line detection. This study aimed to evaluate whether the hemodynamic monitoring and pulmonary monitoring functions are well integrated, and verify the usability and efficiency of the system. Methods: A usability test was condu

[Functional hemodynamic monitoring should be emphasized in intensive care for burn and trauma patients].

Hemodynamic monitoring is a very important measure for critically ill patients with burn and trauma, and it should be carried out throughout the course of treatment. Functional hemodynamic monitoring consists of the assessment of the dynamic interactions of hemodynamic variables in response to a defined volume change. Accordingly, response of fluid volume can be assessed in a quantitative fashion by measuring variation of both arterial pulse pressure and left ventricular stroke volunime during positive pressure breathing, or the change in cardiac output response to passive leg raising maneuver

Invasive hemodynamic monitoring in obstetrics and gynecology. ACOG Technical Bulletin Number 175--December 1992.

Indications for invasive hemodynamic monitoring in obstetrics and gynecology are much the same as in any other area of medicine and surgery. The clinical decision of whether to employ monitoring cannot be made according to absolute criteria but must be made on an individual basis. Obviously, not every patient will benefit from or need invasive hemodynamic monitoring; proper patient selection is important, as is the availability of skilled physicians, nurses, and ancillary support personnel. The information collected at the patient's bedside can be used to both guide and evaluate therapeutic ma

Continuous monitoring of cerebral oxygenation in acute brain injury: assessment of cerebral hemodynamic reserve.

A new index of cerebral hemodynamics, cerebral hemodynamic reserve (CHR), was evaluated in 12 comatose adults with severe, acute, traumatic, diffuse swelling of the brain, who underwent continuous monitoring with a fiberoptic catheter of the saturation difference in arteriojugular oxyhemoglobin. CHR was assessed as the ratio of changes in global cerebral oxygen extraction to changes in cerebral perfusion pressure (CPP) as a result of spontaneous increases in intracranial pressure (ICP). During the course of hyperventilation (Pco2 in the range of 20 mm Hg) for ICP control below 20 mm Hg, 34 obs
