Regional perfusion with hemofiltration (chemofiltration) for the treatment of patients with regionally advanced cancer.

BACKGROUND: Regionally advanced cancer is a common, often unresolved problem. Effective local control with chemotherapy is limited by the toxicity following systemic administration. Chemofiltration (CF) is a form of regional perfusion that enables the administration of cytotoxic drugs into one body area while limiting systemic toxicity. The drug is infused into the artery supplying the involved area. The venous effluent of the same organ is pumped out into a hemofiltration unit at a high flow rate. The drug is then filtered away and the blood returned to systemic circulation. METHODS: Forty-o

[Hemofiltration in the adult respiratory distress syndrome in patients with diffuse suppurative peritonitis].

Hemofiltration (HF) potentials are presented in complex treatment of 12 patients with disseminated suppurative peritonitis in the phase of polyorgan failure with signs of adult respiratory distress syndrome (ARDS). It has been shown that HF reduces the amount of extravascular fluid in the lungs, increases right-to-left pulmonary shunt, eliminates mediators causing higher permeability of pulmonary vessels, and other medium-molecular toxins. Death was recorded in 6 out of 12 patients. In 3 of the dead patients HF had an obvious positive effect, and death was caused by acute hepatorenal failure.

[Use of continuous arteriovenous hemofiltration in diuretic-resistant hypervolemia and electrolyte disorders].

CAVH was carried out in 10 olig/anuric children with diuretic resistant hypervolemia. In addition, three children had hypercalcemia and one child hypernatremia. All but one needed artificial ventilation for pulmonary edema and positive inotropic cardiac support. Mean duration of CAVH for correction of fluid- and electrolyte imbalance was 51.5 hours (range 20-144 hours). The mean fluid removal of 1715 +/- 1479 (SD) ml decreased mean body weight from 21.4 +/- 18.9 (SD) kg to 19.2 +/- 17.0 (SD) kg. The electrolyte disorders were corrected within 18-24 hours. After correction of fluid overload and

Treatment of acute renal failure in newborns by continuous arterio-venous hemofiltration.

The treatment of acute renal failure (ARF) in the newborn with hemo- or peritoneal dialysis is technically difficult and may even be contraindicated. As in the adult, continuous arterio-venous hemofiltration (CAVH) may be an alternative therapy. We used CAVH in the treatment of four newborns with ARF of different etiology. Two brachial, one femoral and one umbilical arteries were cannulated as arterial access, while three jugular and one umbilical veins were used as venous return. An Amicon 0.005 m2 Polysulphon Hollow Fiber hemofilter was connected to the patient with shortened pediatric hemod

Charcoal hemofiltration for hepatic veno-occlusive disease after hematopoietic stem cell transplantation.

Hepatic veno-occlusive disease (HVOD) after hematopoietic stem cell transplantation (HSCT) results in considerable morbidity and mortality. No therapy has been shown to be uniformly effective. Several studies have highlighted the pivotal role of endothelial injury and the hemostatic system in the pathogenesis of HVOD. Charcoal hemofiltration has been shown to be effective for adsorbing circulating bilirubin and other protein-bound toxins and for supporting patients in hepatic failure. We describe two adult patients with severe, biopsy-proven HVOD (peak bilirubin levels, more than 50 mg/dl in b

Continuous arteriovenous hemofiltration countercurrent dialysis (CAVH-D) in acute respiratory failure (ARDS).

The balance between intravascular volume, oxygen transport, and arterial oxygenation is delicate in patients with adult respiratory distress syndrome (ARDS). Recently, we used continuous arteriovenous countercurrent hemodialysis (CAVH-D) in 14 nonoliguric patients who had severe ARDS. The cause of the ARDS was pancreatitis in 1 patient, trauma in 10 patients, and postoperative in 4 patients. All patients were edematous, in marked positive fluid balance but not intravascularly overloaded before institution of CAVH-D. Transfemoral CAVH-D was instituted and managed by the SICU staff. Patients und

Experience with renal failure during extracorporeal membrane oxygenation: treatment with continuous hemofiltration.

We use extracorporeal membrane oxygenation (ECMO) to treat respiratory and cardiac failure in children who are unresponsive to standard ventilator and pharmacologic management. All patients have cardiac and abdominal ultrasonography prior to ECMO to identify major structural anomalies and anatomically normal kidneys. Despite this, oliguric renal failure is seen in a number of patients. Acute renal failure (ARF) developed in two of the first 20 patients we placed on ECMO and both of these patients died. Six of the last 27 patients (22%) also developed ARF and were treated with continuous hemofi

Regional citrate anticoagulation in predilution continuous venovenous hemofiltration using prismocitrate 10/2 solution.

Regional citrate anticoagulation (RCA) for continuous renal replacement therapy (CRRT) is associated with a longer filter life and fewer bleeding events. Complexity of the regimen is the major hurdle preventing widespread application. This study describes a simple predilution continuous venovenous hemofiltration (CVVH) protocol utilizing a commercially prepared replacement solution containing citrate (Prismocitrate 10/2). Ten patients with acute renal failure were evaluated. The Prismaflex system was used for predilution CVVH, with Prismocitrate 10/2 running at 2500 mL/h as the main predilutio

Hemofiltration in severe high microvascular permeability pulmonary edema secondary to rickettsial spotted fever.

Two patients, affected by spotted fever, developed low pulmonary capillary wedge pressure (PCWP) pulmonary edema with severe hypoxemia. Conventional specific and supportive therapy, including mechanical ventilation, failed to induce significant respiratory and hemodynamic improvement which was dramatically reached by means of hemofiltration. Removal of circulating middle molecular weight peptides by the convective mass transfer, characteristic of hemofiltration, offers a new and effective therapeutic approach for the adult respiratory distress syndrome secondary to rickettsial diseases.

Continuous arteriovenous hemofiltration in the critically ill patient. Clinical use and operational characteristics.

Continuous arteriovenous hemofiltration is an extracorporeal technique for the treatment of fluid overload and electrolyte disturbances and for the removal of urea nitrogen. This technique is especially applicable in critically ill patients with hemodynamic instability. A special filter and modified hemodialysis blood lines can easily and rapidly be attached to a patient. No special blood access is needed. Fluids and solutes are removed from the patient by ultrafiltration. A net filtration pressure inside the filter causes an ultrafiltrate to form. The extracorporeal circuit can be kept in pla

Clinical analysis of AN69ST membrane continuous venous hemofiltration in the treatment of severe sepsis.

We aimed to investigate the clinical efficacy of blood purification technology based on cytokine adsorption in the treatment of sepsis. Sixty patients with sepsis were randomly divided into control (n = 30) and experimental (n = 30) groups. Both groups were given routine treatment and continuous venovenous hemofiltration, and on this basis, the experimental group received acrylonitrile/sodium methacrylate (AN69ST) blood purification. The levels of C-reactive protein, procalcitonin, white blood cell count, albumin, platelets, total bilirubin, creatinine, lactic acid, and APACE II score, as well

The therapeutic effect of high-volume hemofiltration on sepsis: a systematic review and meta-analysis.

BACKGROUND: Sepsis remains the leading cause of death in the intensive care unit (ICU), despite the treatment of sepsis has progressed. As a mode in continuous renal replacement therapy (CRRT), continuous veno-venous hemofiltration (CVVH) has been widely used in the treatment of sepsis. Whether high ultrafiltrate volume in CVVH is beneficial for sepsis survival remains controversial. We performed a systematic review and meta-analysis to evaluate the treatment effect of high-volume hemofiltration (HVHF) on sepsis. METHODS: A systematic search was conducted on the Medline, Embase, and Cochrane

Acute Boric Acid Poisoning With Chronic Renal Failure Successfully Treated With Continuous Hemofiltration.

We describe an 88-year-old female with chronic kidney disease (CKD) stage 3a (baseline serum creatinine, 1.1 mg/dL) who presented with vomiting and diarrhoea after accidentally ingesting 6 g of a boric acid solution intended for eye irrigation. On admission, her blood pressure was 95/49 mmHg, and physical examination revealed mild facial and chest erythema. As the ingested dose exceeded the toxic threshold and the patient exhibited potential for hemodynamic instability, continuous hemodiafiltration (CHDF) was initiated at 16 hours post-ingestion with the following parameters: blood flow rate,

Anticoagulant effect of low concentration plasma trisodium citrate in continuous veno-venous hemofiltration.

BACKGROUND: Effective anticoagulation is a prerequisite of continuous renal replacement therapy (CRRT). During CRRT, prolonged anticoagulant therapy should be administered to prevent extracorporeal clotting. The purpose of our study was to evaluate whether a low concentration of plasma trisodium citrate (TCA) was effective as an anticoagulation treatment for continuous veno-venous hemofiltration (CVVH). We also retrospectively analyzed the complications of regional citrate anticoagulation (RCA) in our hospital. METHODS: We conducted this single-center retrospective study at a tertiary academi
