A shot at hepatitis prevention.

Recent trends suggest that the prevalence of hepatitis infection has declined in populations vaccinated against the hepatitis viruses. Still, with an estimated 20,000 new infections annually in the United States from human hepatitis A virus, as many as 100,000 annual cases of acute infection from hepatitis B virus, and more than 1 million individuals with chronic hepatitis B virus infection, there is room for further improvement. Although hepatitis A virus infection causes debilitating symptoms and illness, it is typically not a protracted illness, and it usually resolves over weeks to months

Hepatitis A - prevention in travellers.

BACKGROUND: Hepatitis A is the second most common vaccine preventable infection in travellers. Highly effective vaccines exist for its prevention for travellers from 12 months of age, including last minute travellers and those in special risk groups. OBJECTIVE: Information about hepatitis A infection, its epidemiology and existing vaccine options is presented for use in travel related consultations in general practice. DISCUSSION: Most travellers at risk of hepatitis A should be vaccinated, as the vaccine is a safe and effective means of prevention. Combination vaccines - hepatitis A/hepatit

Hepatitis E: a neglected threat.

Hepatitis E, responsible for explosive waterborne outbreaks and sporadic cases of acute hepatitis in developing countries, affects predominantly young adults and has a fatality rate as high as 25% in pregnant women. No effective treatment exists for hepatitis E; however, a vaccine using a baculovirus-expressed recombinant hepatitis E capsid protein was recently studied in Nepal. In this review, the progress made in hepatitis E research and the recently concluded vaccine trial of the recombinant protein vaccine are briefly discussed.

[Thyrotoxicosis hepatitis: a case report].

Abnormal liver function in thyroid disorders may be secondary to thyrotoxicosis or to autoimmune injury to the liver. We report the case of a 36-year-old female who developed jaundice and pruritus with mild cholestasis and moderately elevated transaminase levels. The diagnosis of Graves' disease was made shortly thereafter. Laboratory findings were: alanine and aspartate aminotransferase 219 (IU/I (N: 9-50) and 102 IU/I (N: 10-15) respectively, alkaline phosphatase 336 IU/I (N: 40-135), bilirubin 24 micromol/I (N: 2-23), and gamma-glutamyl transpeptidase 232 IU/I (N: 9-43). [Clinical forms of hepatitis A].

INTRODUCTION: Although its incidence has decreased during the last 20 years, hepatitis A is still the most common hepatitis. In most cases, hepatitis A is asymptomatic. When it is symptomatic, the course is benign in most cases. The aim of this review is to summarize current data regarding unusual clinical forms of hepatitis A. CURRENT KNOWLEDGE AND KEY POINTS: Hepatic insufficiency is the most severe complication of hepatitis A. It is more commonly observed in adult patients. In most cases, the outcome of hepatic insufficiency is rapidly favorable. In rare cases, hepatic insufficiency progre

Granulomatous hepatitis: a retrospective study.

51 cases of granulomatous hepatitis were seen among 1234 liver biopsies over a 10 year period. Tuberculosis was the commonest cause seen in 55 percent of cases. Other causes included leprosy, sarcoidosis, histoplasmosis, brucellosis, amoebic liver abscess, lymphoma and malignant granuloma. 12 percent of cases remained undiagnosed. Clinically these patients presented with pyrexia and hepatosplenomegaly. Jaundice was uncommon. Many showed elevated alkaline phosphatase levels, anaemia and raised ESR Granulomatous hepatitis of unknown aetiology with FUO was seen in 6 percent cases only.

Hepatitis C virus. A review.

Hepatitis C virus has been shown to be responsible for most cases of posttransfusion hepatitis, as well as for sporadic non-A, non-B viral hepatitis. Hepatitis C virus has also been implicated in the development of primary hepatocellular carcinoma, autoimmune hepatitis, and fulminant viral hepatitis. Although the role of the parenteral transmission of hepatitis C virus is well established, its route of transmission in cases of sporadic infection remains unclear. Sexual transmission is suspected but not confirmed. Recent work regarding treatment has shown interferon alfa to be effective, but th

Hepatitis C: a sleeping giant?

In the United States, non-A, non-B hepatitis accounts for 20-40% of acute viral hepatitis. Although it has traditionally been considered a transfusion-associated disease, non-A, non-B hepatitis is more likely to occur outside the transfusion setting. Surveillance data from the Centers for Disease Control show that in 1988 6% of patients with non-A, non-B hepatitis reported a history of blood transfusion, 46% parenteral drug use, 10% household or sexual exposure to a contact who had had hepatitis or exposure to multiple sex partners, 2% medical or dental employment involving frequent blood cont

[Recurrences of viral hepatitis A].

The aim of the study was to examine the frequency, severity, persistence and etiology of relapses occurring during the hepatitis A viral infection. Therefore, a prospective study of 910 patients suffering from hepatitis A (HA) was carried out. The clinical examination and determination of glutamyl pyruvic transaminase (GPT) in the serum every 7-14 days till recovery (usually during 6--8 months) were performed. HAV infection was confirmed by detecting anti-HAV IgM in the blood of all the examined by radioimmunoassay. In 876 (93.3%) patients HA had typical clinical features and a monophasic cour

Fibrosing cholestatic hepatitis in a kidney transplant recipient with hepatitis C virus.

Fibrosing cholestatic hepatitis (FCH) is a fatal disorder that presents as a progressive deterioration of liver function over a period of several weeks to several months. It is caused by the direct cytotoxic effect of the over-expression of viral antigens on hepatocytes in immunosuppressed patients. Our patient was a 59-year-old man with hepatitis C virus (HCV) infection of genotype 2a who had suffered from end-stage renal disease due to diabetic nephropathy and underwent kidney transplantation. His serum total bilirubin levels gradually increased to 20 mg/dl and liver atrophy progressed durin

Asymptomatic giant cell hepatitis: a subtype of post-infantile giant cell hepatitis?

Giant cell hepatitis in adults is considered a rapidly progressive and life-threatening disease, but there are few descriptions of a prolonged disease course. A 36-year-old Japanese man was referred to our hospital for further evaluation of abnormal liver function test results. Although asymptomatic, he had undergone follow-up for 9 years with these abnormalities. Because the cause of liver injury was not identified despite extensive noninvasive examinations, the patient underwent needle biopsy. He was finally diagnosed with post-infantile giant cell hepatitis (PIGCH) based on the presence of

Cerebral Malaria in a Patient with HIV, Hepatitis B, and Hepatitis C.

Cerebral malaria is one of the most common causes of non-traumatic encephalopathy. A 25-year-old man who is a known intravenous and oral drug abuser presented to our clinic with fever and sore throat for two days prior and an altered level of consciousness for one day. On examination, the patient was icteric, and his Glasgow coma scale score on arrival was 10/15; he had dilated pupils reactive to light and a positive corneal reflex. All cranial nerves were intact; however, signs of meningeal irritation were positive. Motor examination showed an increased tone and rigidity in all limbs, patella

Hepatitis E virus as a Cause of Acute Hepatitis in The Netherlands.

BACKGROUND: Recent studies indicate that 27% of Dutch blood donors have evidence of past infection with HEV. However, the low number of diagnosed HEV infections indicates either an asymptomatic course or under diagnosis. OBJECTIVES: We investigated whether HEV is a cause of acute hepatitis in Dutch patients and which diagnostic modality (serology or PCR) should be used for optimal detection. STUDY DESIGN: Serum samples were retrospectively selected from non-severely immuno-compromised patients from a university hospital population, suspected of having an infectious hepatitis. Criteria were:

A hepatitis B virus mutant associated with an epidemic of fulminant hepatitis.

BACKGROUND: A nosocomial outbreak of fulminant hepatitis B occurred in five patients in Haifa, Israel. Previous investigations identified the suspected source as a carrier of hepatitis B surface antigen who was positive for antibodies to hepatitis B e antigen and had chronic liver disease. We examined the strain of hepatitis B virus (HBV) that caused this epidemic, in order to identify specific mutations in the precore or core region. METHODS: The presence of HBV was identified by polymerase-chain-reaction amplification of viral DNA in serum from the source patient, the five patients with ful

Hepatitis in clinical practice. 2. Non-A non-B and delta hepatitis.

Non-A non-B (NANB) hepatitis has at least two distinct causative agents. NANB hepatitis can lead to chronic liver disease, cirrhosis, and hepatocellular carcinoma. No definitive policy exists to exclude this disease as a cause of posttransfusion hepatitis. Delta agent is epidemiologically linked to hepatitis B virus (HBV) infection, either as a coinfection or as a superinfection in chronic hepatitis B surface antigen carriers. Recognition of hepatitis due to delta agent will probably increase as testing for this agent becomes more available and its presence becomes more generally well known th
