Effects of hypocarbia on the development of cystic periventricular leukomalacia in premature infants treated with high-frequency jet ventilation.

BACKGROUND AND OBJECTIVE: Previous data from our institution indicate that mechanically ventilated premature infants are at increased risk for cystic periventricular leukomalacia (CPVL), particularly if hypocapnia occurs. High-frequency jet ventilation (HFJV) may produce substantial hypocapnia. We sought to investigate whether hypocapnia during HFJV is associated with the development of CPVL. METHODS: Sixty-seven premature infants (mean gestational age, 27.2 weeks; mean birth weight, 1001 g) underwent HFJV for a mean of 44 (range, 8 to 70) hours during the first 3 days of life. All infants we

[The efficacy of HFJV (high frequency jet ventilation) for ARDS (adult respiratory distress syndrome) following total arch replacement].

A 40-year-old man was admitted to the hospital with shock due to acute aortic dissection (type A of Stanford classification). The emergent total arch replacement was performed with the retrograde cerebral perfusion uneventfully. At the 3rd day after the operation, bilateral infiltrates were recognized on the frontal chest radiograph. The oxygenation was gradually decreased to less than 200 mmHg of PaO2/FiO2 in proportion with the growth of infiltrates and increase of LDH in blood. Steroid therapy was not effective. So then HFJV was introduced to improvement of hypoxygenation at the 16th day af

Anaesthesia for extracorporeal shock-wave lithotripsy. A comparison of propofol and methohexitone infusions during high frequency jet ventilation.

A continuous infusion of propofol 2.0-2.5 mg/kg for induction followed by 9 mg/kg/hour for the first 30 minutes and 6 mg/kg/hour thereafter, was compared with methohexitone 1.5 mg/kg for induction followed by 4.8 mg/kg/hour thereafter for maintenance of anaesthesia in a randomised study of 40 patients who underwent extracorporeal shock-wave lithotripsy using high frequency jet ventilation (HFJV). Systolic blood pressure was significantly lower in the propofol group after induction of anaesthesia, tracheal intubation, placement in the semirecumbent position in the hoist, bath immersion and afte

Clinical efficacy of high frequency jet ventilation during extracorporeal shock wave lithotripsy of renal and ureteral calculi: a comparison with conventional mechanical ventilation.

The use of high frequency jet ventilation compared to conventional mechanical ventilation during general anesthesia for extracorporeal shock wave lithotripsy of renal or ureteral calculi can reduce stone movement. This decrease in stone movement theoretically lessens the total shock and energy requirements for stone fragmentation and perirenal tissue damage. To assess these theoretical advantages of high frequency jet ventilation, we studied patients undergoing extracorporeal shock wave lithotripsy to determine differences in stone movement during high frequency jet and conventional mechanical

Complications during interventional rigid bronchoscopy with general anaesthesia and high-frequency jet ventilation: a 5-year retrospective pilot study.

BACKGROUND: High-frequency jet ventilation (HFJV) is the standard technique for rigid bronchoscopy. Few studies have comprehensively examined the complications associated with this technique. Previous evidence suggests that an ASA 4 score and basal SpO2 less than 95% may increase the likelihood of periprocedural complications. OBJECTIVES: This retrospective study aimed to quantify and characterise the frequency and types of complications linked to HFJV, and to identify independent factors associated with their occurrence.DESIGN A retrospective pilot cohort study. SETTING: This monocentric pi

Nasal high-frequency jet ventilation (NHFJV) as a novel means of respiratory support in extremely low birth weight infants.

OBJECTIVE: Describe our experience of successfully using nasal high-frequency jet ventilation (NHFJV) in extremely low birth weight infants with respiratory failure. STUDY DESIGN: A retrospective review was conducted on 16 infants with birth weights <1000 g who received NHFJV from 2015 to 2019. Successful use was defined as avoiding intubation for at least 72 hours and demonstrating tolerance after being placed on NHFJV. RESULTS: Median gestational age was 24.5 weeks (IQR 24, 25), and weight at the start of NHFJV 1090 g (IQR 905, 1250). NHFJV was used successfully in 13/16 (81%) infants with

Predictive value of FRC and respiratory compliance on pulmonary gas exchange induced by high frequency jet ventilation in humans.

To determine if functional residual capacity (FRC), compliance of the respiratory system (C), or underlying pulmonary disease are predictive for the efficacy of high frequency jet ventilation (HFJV) on pulmonary gas exchange, we investigated six adult patients within 4 h of abdominal surgery and six patients with severe adult respiratory distress syndrome. Gas exchange during intermittent positive pressure ventilation (IPPV) was compared with that during HFJV at frequencies of 100 b.p.m. (HFJV100) and 200 b.p.m. (HFJV200), resulting in a minute ventilation of about 400 ml kg-1 with both ventil

Superimposed high-frequency jet ventilation used for endolaryngotracheal surgery in a child with congenital subglottic stenosis: a case report.

BACKGROUND: Airway management and anesthesia during endolaryngotracheal surgery in patients with obstructive airway diseases pose a major challenge for anesthesiologists, especially in pediatric patients. Children with obstructive airway disease often have a potentially difficult airway. Adequate airway assessment and preparation before anesthesia is essential. In the formulation of the entire anesthesia strategy, the choice of ventilation mode is the most critical. Superimposed high-frequency jet ventilation (SHFJV) is an enormous step forward in the progress of difficult surgery of the laryn

High-frequency jet ventilation is beneficial during shock wave lithotripsy utilizing a newer unit with a narrower focal zone.

INTRODUCTION: High-frequency jet ventilation (HFJV) during shock wave lithotripsy (SWL) has been reported using older lithotripsy units with larger focal zones. We investigated how HFJV affects the clinical parameters of SWL using a newer lithotripsy unit with a smaller focal zone. METHODS: We reviewed all patients who underwent SWL by a single surgeon (KVA) from July 2006 until December 2007 with the Siemens Lithostar Modularis (Siemens AG, Erlangen, Germany). Either HFJV or conventional anesthetic techniques were used based on the anesthesiologists' preference. Preoperative imaging was revi

Effects of driving pressure and respiratory rate on airway pressure and PaCO2 in rabbits during high-frequency jet ventilation.

The effects of driving pressure (DP) and respiratory rate (RR) on mean airway pressure (Paw), PaCO2, and other respiratory variables were studied during high-frequency jet ventilation (HFJV) in anesthetized rabbits. The animals were tracheotomized and paralyzed, and various combinations of RR (4, 8, 12, and 16 Hz) and DP (0.5, 1.0, and 2.0 kg/cm2) were applied randomly with an inspiratory/expiratory ratio of 1.0. HFJV was delivered via a 2.5-Fr catheter through a T-attachment, using 100% oxygen, and a hot-wire flowmeter was connected to the outlet of the T-attachment for measurement of the min

Conventional vs high-frequency jet ventilation in a piglet model of meconium aspiration: comparison of pulmonary and hemodynamic effects.

The pulmonary and cardiovascular effects of high-frequency jet (HFJV) and conventional (CV) ventilation were evaluated in a piglet model of meconium aspiration. A mixture of 20% human meconium and 0.9% saline solution was instilled deep into the trachea of 10 piglets, after which either HFJV or CV was administered for 4 hours. Arterial blood gases, cardiac output, mean pulmonary and systemic arterial pressures, pulmonary and systemic vascular resistances, and pulmonary mechanics were compared between groups. During the 4 hours of ventilation, PaO2 and PaCO2 were not statistically different bet

Influence of High-Frequency Low-Tidal Volume Versus Jet Ventilation on Acute and Long-Term Outcomes Following Pulmonary Vein Isolation.

INTRODUCTION: Use of high-frequency low-tidal volume (HFLTV) and high-frequency jet ventilation (HFJV) during pulmonary vein isolation (PVI) improves acute procedural success and long-term outcomes compared to conventional ventilation. However, the impact of HFLTV compared to HFJV on acute and long-term outcomes is unknown. METHODS: We prospectively identified 1039 patients who underwent first-time PVI or PVI with posterior wall isolation (PWI) at our institution between 2022 and 2024 with HFLTV or HFJV. Acute procedural and safety outcomes were analyzed. Twelve-month arrhythmia-free survival

High frequency jet ventilation through a supraglottic airway device: a case series of patients undergoing extra-corporeal shock wave lithotripsy.

High frequency jet ventilation has been shown to be beneficial during extra-corporeal shock wave lithotripsy as it reduces urinary calculus movement which increases lithotripsy efficiency with better utilisation of shockwave energy and less patient exposure to tissue trauma. In all reports, sub-glottic high frequency jet ventilation was delivered through a tracheal tube or a jet catheter requiring paralysis and direct laryngoscopy. In this study, a simple method using supraglottic jet ventilation through a laryngeal mask attached to a circle absorber anaesthetic breathing system is described.
