Pediatric Injury Transfer Patterns During the COVID-19 Pandemic: An Interrupted time Series Analysis.

INTRODUCTION: With the expected surge of adult patients with COVID-19, the Children's Hospital Association recommended a tiered approach to divert children to pediatric centers. Our objective was understanding changes in interfacility transfer to Pediatric Trauma Centers (PTCs) during the first 6 mo of the pandemic. METHODS: Children aged < 18 y injured between January 1, 2016 and September 30, 2020, who met National Trauma Databank inclusion criteria from 9 PTCs were included. An interrupted time-series analysis was used to estimate an expected number of transferred patients compared to obse

Emergency department evaluation and management of nonaccidental trauma in pediatric patients.

Children who have suffered physical abuse may present to the healthcare setting multiple times before a diagnosis is made. Emergency clinicians must be able to recognize sentinel and severe signs of nonaccidental trauma and pursue an appropriate evaluation to prevent further injury. This issue offers evidence-based recommendations for the identification and management of nonaccidental trauma in children. Key historical and physical examination findings that should trigger an evaluation for physical abuse are reviewed. Recommendations are given for obtaining diagnostic studies and consulting wi

Vaginal microbiota of American Indian women and associations with measures of psychosocial stress.

Molecular-bacterial vaginosis (BV) is characterized by low levels of vaginal Lactobacillus species and is associated with higher risk of sexually transmitted infections (STI). Perceived psychosocial stress is associated with increased severity and persistence of infections, including STIs. American Indians have the highest rates of stress and high rates of STIs. The prevalence of molecular-BV among American Indian women is unknown. We sought to evaluate measures of psychosocial stress, such as historic loss (a multigenerational factor involving slavery, forced removal from one's land, legally

Core symptoms of PTSD across four millennia: a phenomenological and nosographic analysis - from ancient Mesopotamian texts to modern psychiatric classifications.

Post-traumatic stress disorder (PTSD) represents a transcultural recurrent response to extreme events, documented across approximately four millennia of human history. Although formally codified in Diagnostic and Statistical Manual of Mental Disorders (DSM)-III (1980), descriptions of post-traumatic symptomatology already appear in Mesopotamian cuneiform texts (2027-2003 BCE) and continue through classical literature and modern clinical observation. This historical-critical analysis assesses the phenomenological continuity of trauma-related symptoms across epochs and cultures, reconstructing t

Compound risk: History of traumatic stress predicts posttraumatic stress disorder symptoms and severity in sudden cardiac arrest survivors.

BACKGROUND: Sudden cardiac arrest (SCA) survivors can develop posttraumatic stress disorder (PTSD) which is associated with worse clinical outcomes. The purpose of this study was to evaluate the prevalence and predictors of PTSD in a large sample of SCA survivors. Prior history of psychological trauma and the effects of repeated trauma exposure on subsequent PTSD and symptom severity after SCA were also explored. METHODS: A retrospective, cross-sectional study of 188 SCA survivors from the Sudden Cardiac Arrest Association patient registry completed an online questionnaire that included measu

The role of anxiety and self-compassion in the link between trauma and alcohol use among American Indians.

Historical trauma and ongoing systemic inequities contribute to elevated levels of anxiety and problematic alcohol use among American Indians. The current study examines the relationship between recent trauma and problematic alcohol use, testing anxiety as a mediator and self-compassion as a moderator. A sample of 727 American Indian adults completed measures of recent traumatic experiences, anxiety, problematic alcohol use, and self-compassion. Results indicated that anxiety mediated the relationship between trauma and alcohol use, suggesting that individuals with greater recent trauma exposu

Peri-operative strategy in resuscitation of unstable injured surgical patients: a primer.

BACKGROUND: Trauma remains a leading cause of death, both for individuals under 40 in North America, and globally, where it contributes to ~10% of deaths annually. Thoughtful, timely, balanced resuscitation, especially in the peri-operative period for unstable injured surgical patients, is vital for optimizing outcomes. The advanced trauma life support protocol plays a pivotal role in early evaluation and management, emphasizing hemorrhage control and resuscitation strategies. OBJECTIVE: This narrative review provides a structured, evidence-based framework aimed at enhancing the educational e

Evaluation of a Novel Mechanical Venous Thromboembolism Compression Device in Trauma Patients: A Pilot Study.

BACKGROUND: Venous thromboembolism (VTE) is the fourth most common preventable hospital-acquired complication for hospitalized trauma patients. Mechanical prophylaxis, using sequential compression or intermittent pneumatic compression (IPC) devices, is recommended alongside pharmacologic prophylaxis for VTE prevention. However, compliance with device use is a barrier that reduces the effectiveness of mechanical prophylaxis. OBJECTIVE: This study aimed to determine whether using the Movement and Compressions (MAC) system compared with an IPC device impacts compliance with mechanical VTE prophy

Blending Aboriginal and Western healing methods to treat intergenerational trauma with substance use disorder in Aboriginal peoples who live in northeastern Ontario, Canada.

As with many Indigenous groups around the world, Aboriginal communities in Canada face significant challenges with trauma and substance use. The complexity of symptoms that accompany intergenerational trauma and substance use disorders represents major challenges in the treatment of both disorders. There appears to be an underutilization of substance use and mental health services, substantial client dropout rates, and an increase in HIV infections in Aboriginal communities in Canada. The aim of this paper is to explore and evaluate current literature on how traditional Aboriginal healing meth

[The roots of trauma: a review of the history of psychotrauma].

Perceptions of the importance of the role of psychological trauma in the origins of psychiatric problems have oscillated throughout the history of psychiatry. However, since the conception of post-traumatic stress disorder (PTSD), western societies have witnessed a marked expansion of the discourse of trauma in the interpretation of devastating human experiences like catastrophes, genocides, disasters, and epidemics. Through an integrative literature review, this article analyzes some of the historical and epistemological determinants behind the emergence of traumatic memory and the establishm

Evolution of traumatic narratives impact of the Holocaust on children of survivors.

Traumas' lessons are embedded in oral narratives of disasters that are transmitted over centuries and incorporated into historical memory; often they are woven into scripture and religious ritual; eventually they become encrypted in the collective unconscious. The story of the Holocaust functions like a map of the world for survivors' children, whose minds it both constrains and overwhelms, impacting psychological development and construction of reality. The focus in this paper is on composites of three Holocaust survivors and their daughters, who exemplify traumatic narratives' evolution as t

Fluid Resuscitation of the Adult Trauma Patient: Where Have We Been and Where Are We Going?

The resuscitation of an adult trauma patient has been researched and written about for the past century. Throughout those discussions, 2 major controversies persist when discussing resuscitation methods: (1) the ideal choice of fluid type to use during the initial resuscitation period, and (2) the ideal fluid volume to infuse during the initial resuscitation period. This article presents a brief historical perspective of fluids used during a trauma resuscitation, along with the latest research findings as they relate to the 2 stated issues.

Psychological testing as a diagnostic and therapeutic tool in the treatment of traumatized Latin American and African refugees.

The use of psychological assessment, an underutilized tool, in connection with posttraumatic stress disorder (PTSD) is presented. Identification of PTSD in refugees from Latin America and Africa is usually difficult because it is compounded by the trauma of migration. Issues regarding diagnosis and treatment are discussed, and case examples are provided to illustrate specific clinical concerns. Disclosure of historical information to the clinician and validation of a history of trauma are addressed through the testing process and projective data patterns.

Evaluation of the applicability, efficacy, and safety of a thromboembolic event prophylaxis guideline designed for quality improvement of the traumatically injured patient.

BACKGROUND: Thromboembolic events (TE) such as deep venous thrombosis (DVT) and pulmonary embolism (PE) are common after trauma. Our Trauma Practice Management Committee developed an evidence-based DVT/PE prophylaxis guideline using a modified Delphi approach to standardize care and reduce TE rates. Our objective was to evaluate the applicability, efficacy, and safety of this guideline in the traumatized patient, especially those admitted first to the intensive care unit (ICU). METHODS: We developed a risk-stratified DVT/PE prophylaxis guideline incorporating specific injuries, pertinent hist

Management of the complex duodenal injury.

INTRODUCTION: Complex duodenal trauma is a rare injury with an incidence of 1-4.7% of all abdominal trauma. Historically, varied approaches have been used in the management of these complex injuries and the prevention of complications. This is a review of the current management methodology of complex duodenal injury. METHODS: A review of the medical literature to include the past and current management of duodenal trauma was performed. Google scholar (1970-2022) and PubMed (1970-2022) were searched using the keywords: complex duodenal trauma, surgical management, and duodenal complications.

Hallux Rigidus: Relevant Anatomy and Pathophysiology.

Hallux rigidus is a painful condition of the great toe characterized by restriction of the metatarsophalangeal joint arc of motion and progressive osteophyte formation. Precise cause of hallux rigidus remains under debate. Anatomic variations and historical, clinical, and radiographic findings have been implicated in the development and progression of hallux rigidus. Radiologic findings associated with hallux rigidus include metatarsal head osteochondral defects, altered metatarsal head morphology, and an elevated hallux interphalangeus angle measure. Associated historical findings include a p

Traumatic pasts and the historical imagination: Symptoms of loss, postcolonial suffering, and counter-memories among African migrants.

This work aims to rethink the relationship between anthropology and cultural psychiatry from a historical perspective, through reflections on the dynamics of forgetting and remembering in the context of migration. While migrants' symptoms often bear cultural hallmarks of suffering, they also reveal images of a traumatic history, which resurface in moments of danger, uncertainty, and crisis. I claim these symptoms are allegories of a dispossessed past, and can be interpreted as counter-memories, as "palimpsests" of an eclipsed script. Trauma symptoms keep returning to a collective past, and thu
