Tribute to a triad: history of splenic anatomy, physiology, and surgery--part 1.

The spleen is an enigmatic organ with a peculiar anatomy and physiology. Though our understanding of this organ has improved vastly over the years, the spleen continues to produce problems for the surgeon, the hematologist, and the patient. The history of the spleen is full of fables and myths, but it is also full of realities. In the Talmud, the Midrash, and the writings of Hippocrates, Plato, Aristotle, Galen, and several other giants of the past, one can find a lot of Delphian and Byzantine ambiguities. At that time, splenectomy was the art of surgery for many splenic diseases. From antiqui

[Antony's fire (gangrenous ergotism) and medieval iconography].

Ergotism was known as Holy Fire or St Antony's Fire in the Middle Ages, because of the burning sensations and limb gangrene it entailed. It was a frequent disorder, caused by eating rye flour contaminated by the fungus Claviceps purpurea. The Hospitable Order of St Antony was founded near Vienne in France, and counted 400 establishments in Europe by 1777. Ergotism is the subject of an abundant iconography, including statues and paintings. Woodcuts show the temptations of St Antony, with strange and diabolic scenes, and individuals with gangrenous limbs. Germanic woodcuts of the XVth century sh

Continuity or change in the transition to Islam? A diachronic assessment of agricultural production at Old Dongola, Northern Sudan (14th-18th centuries CE).

In this paper, we present an archaeobotanical study from Old Dongola, northern Sudan, examining agricultural dynamics and subsistence strategies from the Late Christian to the Islamic period. Old Dongola, established in the 5th century, served as the capital of the Kingdom of Makuria, one of the three medieval Nubian kingdoms. Following the collapse of Makuria in the 14th century, the city underwent significant cultural and social transformations, marked by the migration of Arab tribes and the rise of new power structures. From the 14th to the 18th century, Old Dongola served as the seat of a

Evolving concepts of heart failure: cooling furnace, malfunctioning pump, enlarging muscle--Part I.

Understanding of the causes of dyspnea and anasarca, the cardinal features of heart failure, has changed dramatically since Greco-Roman times, when sputum and pleural effusions were thought to originate in the brain, and the heart was believed to heat and distribute the vital spirit. It was not until the seventeenth century, when Harvey demonstrated that the heart was a pump and autopsy descriptions revealed valve abnormalities that interfered with the circulation, that it became possible to identify the role of heart disease in causing shortness of breath and edema. Morgagni's recognition, to

A stone never cut for: a new interpretation of The Cure of Folly by Jheronimus Bosch.

From the late 15th century onwards, cutting for stones in patients' heads to cure mental illness was depicted in European art. The earliest and most famous painting of this kind is The Cure of Folly, painted by Jheronimus Bosch around the turn of the 15th to the 16th century. In general, these depictions are thought to relate to surgical as well as mental health practices of that time. However, it can be questioned whether barber surgeons or quacks cut for stones in patients' heads, and if they did not, what the meaning of Bosch's and other artists' paintings might be. Paintings from the 15th

Eight sages over five centuries share oxygen's discovery.

During the last century, historians have discovered that between the 13th and 18th centuries, at least six sages discovered that the air we breathe contains something that we need and use. Ibn al-Nafis (1213-1288) in Cairo and Michael Servetus (1511-1553) in France accurately described the pulmonary circulation and its effect on blood color. Michael Sendivogius (1566-1636) in Poland called a part of air "the food of life" and identified it as the gas made by heating saltpetre. John Mayow (1641-1679) in Oxford found that one-fifth of air was a special gas he called "spiritus nitro aereus." Carl

The occurrence of rabies in pre-Columbian Central America: an historical search.

Rabies is considered one of the oldest infectious diseases known to humans. However, the first written reports on rabies cases in the Americas did not appear until the first decade of the 18th century from Mexico. In an attempt to clarify if the disease was already present in pre-Columbian times, we searched for evidence in the Maya and Aztec cultures. Other sources of information were early manuscripts written by the conquistadors and early explorers. We did not identify any unequivocal direct evidence that the disease rabies was known in pre-Columbian Central America but sufficient circumsta

The myth of the medical breakthrough: smallpox, vaccination, and Jenner reconsidered.

A discussion of the particulars leading to the eradication of smallpox is pertinent to both investigators and the public as the clamor for more "breakthroughs" intensifies. The rational allocation of biomedical research funds is increasingly threatened by disease-advocacy groups and congressional earmarking. An overly simplistic view of how advances truly occur promises only to stunt the growth of researchers and research areas not capable of immediate great breakthroughs. The authors review the contributions of Jenner and his countless predecessors to give a more accurate account of how "over

Origin and evolution of syphilis: drifting myth.

The venereal form of treponematosis, caused by the spirochete Treponema pallidum, plagued every major city in the preantibiotic era. "Civilization means syphilization," was an idea touted by Richard von Krafft-Ebing in the late 19th, and early 20th centuries that the effects of modern life make men more susceptible to syphilis and other diseases. Christopher Columbus was thought of as an importer of syphilis to Europe. Because his serendipitous voyages to the New World initiated the process of Spanish colonization, which foreshadowed general European colonization of the New World, it is diffic

The birth of plastic surgery: the story of nasal reconstruction from the Edwin Smith Papyrus to the twenty-first century.

BACKGROUND: The nose is the central and most prominent feature on the human face; and on its shape, size, and appearance depends the relative facial beauty of the person. The objective of this article was to give a succinct and interesting account of the development of nasal reconstruction from antiquity to the present day. METHODS: The authors present the story of nasal reconstruction, including those contributions not often cited in the English literature using articles sourced from MEDLINE, ancient manuscripts, original quotes, techniques, and illustrations. RESULTS: The story of rhinopla

Protective face masks through centuries, from XVII century plague doctors to current health care professionals managing the COVID-19 pandemic.

The adoption of items similar to face masks by human beings dates back to the remote past. With specific regard to the use of face protections for medical purposes, from the beginning of the XVII century onwards in Europe physicians in charge of curing patients with plague wore a complicated, and subsequently typical, costume. The face mask included eye sockets of glass and leather headdresses with long, pointed beaks. These beaks were filled with scented spices, aromatic substances and perfumes to filter out the plague and to mask "bad air", which was considered to be the vehicle of the disea
