This examination of nursing home bed supply estimates undersupply in each of the states for the purpose of identifying the states with the greatest undersupply of beds. New data on state nursing home bed supply for the period 1979-1982 are used. The study employs selected independent variables in two different types of analyses to estimate bed supply for each state. Where a state is found to have a bed shortage, state public policymakers may wish to employ policies that differ from those suitable for states with an adequate supply of beds. Because of limitations in the data, issues of oversupp

Examination of 163 nursing home residents revealed chronic brain syndrome of different amount in 84%. Demented patients with clinical evidence of arteriosclerosis had significantly higher values of emotional incontinence, dysphoria, depression and vegetative complaints than those without evidence of arteriosclerosis. Correlation of single psychopathologic parameters with other psychopathologic, neurologic and somatic variables resulted in very different correlation profiles for the following symptoms: emotional incontinence, apathy, dysphoria and euphoria. These changes in personality should b

BACKGROUND: Among older adults with frailty, evidence on the benefits and risks of discontinuing antihypertensive drugs is limited.
METHODS: In a multicenter, randomized, controlled trial conducted in France, we assigned, in a 1:1 ratio, nursing home residents 80 years of age or older who were receiving more than one antihypertensive drug and had a systolic blood pressure below 130 mm Hg to a protocol-driven strategy of progressive reduction of antihypertensive treatment (step-down group) or to receive usual care (usual-care group). Patients were to be followed for up to 4 years. The primary

OBJECTIVES: Assess the frequency of delirium during any acute event, its risk factors, and the duration of delirium in nursing home patients.
DESIGN: Prospective 2-month follow-up study.
SETTING AND PARTICIPANTS: 145 nursing home patients living in 3 Norwegian nursing homes.
METHODS: At baseline, known risk factors for delirium were obtained from medical records. During any acute events where the nurses decided that a physician had to be alerted, the Confusion Assessment Method was used to identify delirium on days 1, 2, 4, and 6 and thereafter weekly if delirium was present on day 6. Eucalyptol is one of the most popular volatile components. It is used in many essential oils for relieving sinus and lung congestion caused by a variety of conditions. This pilot study sought to analyze clinical evidence for the effect of the scent of eucalyptol on the cognitive function of elderly people. Seventy nursing-home residents with cognitive impairment were recruited. Three one-week experiments were performed: eucalyptol scent was diffused in bedrooms with a diffuser only at wake-up time in the first experiment, and at wake-up time and bedtime in the second and third experiments. OBJECTIVE: Intestinal infections are common in the elderly, presented with atypical symptoms and may be the cause of mortality with a more severe clinical manifestation. The weakening of cellular and humoral immunity by aging affects the intestinal flora and increases the risk of infection in the presence of chronic diseases. The aim of this study was to investigate the presence of possible parasitic agents in the intestinal system of ≥65-year-old nursing home residents through fecal examination, and to determine the demographic features (age and gender) of this elderly group.

BACKGROUND: For the treatment of chronic heart failure (HF), both pharmacological and non-pharmacological treatment should be employed in HF patients. Although HF is highly prevalent in nursing home residents, it is not clear whether the recommendations in the guidelines for pharmacological therapy also are followed in nursing home residents. The aim of this study is to investigate how HF is treated in nursing home residents and to determine to what extent the current treatment corresponds to the guidelines.

Pneumonia in older adults residing in nursing homes can be challenging to diagnose and treat. Pneumococcal and influenza immunizations decrease the risk of pneumonia and are recommended for all nursing home patients. Older adults with pneumonia may not display classic signs and symptoms of infection, although most have at least one respiratory symptom. Suspicion of pneumonia is heightened if pulse oximetry measurements are low. The diagnosis of pneumonia is confirmed by chest radiography. To determine whether treatment is necessary and where treatment should occur, the patient's overall progno

OBJECTIVES: Twenty-five to thirty percent of hip fractures occur in men, and nursing home residents have a 5-10-fold greater fracture risk than community-dwellers. Osteoporosis prevalence in men in long-term care, however, is poorly defined. Our objectives were to determine the prevalence of osteoporosis, as assessed by peripheral bone mineral density (BMD), in a group of institutionalized veterans, and to determine how many men with low BMD had received a prior diagnosis of osteoporosis.
METHODS: Subjects were residents in a 740-bed skilled nursing facility (78% men). Male residents (n = 103

BACKGROUND: Scabies epidemics are not uncommon in nursing homes. Effective treatment is enhanced by prompt clinical diagnosis and early intervention. The clinical presentation of scabies may vary in older, immunocompromised or cognitively impaired persons.
METHODS: We performed a retrospective study of all residents diagnosed with scabies in a multilevel long-term care geriatric facility. The duration of the outbreak was from May to September 2000.
RESULTS: Fifteen residents contracted scabies during the outbreak. All affected residents had predominantly truncal lesions. Twelve residents had

The mean incidence of falls in nursing homes is 1.5 falls per bed per year (range 0.2-3.6). The most common precipitating causes include gait and balance disorders, weakness, dizziness, environmental hazards, confusion, visual impairment, and postural hypotension. The most important underlying risk factors for falls and injuries include some of these same items as well as others: leg weakness, gait and balance instability, poor vision, cognitive and functional impairment, and sedating and psychoactive medications. A focused history and physical examination after a fall can usually determine th

A prospective study of vision-related outcomes of cataract surgery as well as cognitive functioning, activities of daily living (ADL) functioning, and depressed affect was conducted with 19 nursing home residents scheduled for cataract surgery. Also studied were 22 candidates for cataract surgery who decided against it. Comparison of both groups at baseline revealed that residents who chose cataract surgery were less cognitively and ADL impaired. Analysis after cataract surgery showed that visual acuity improved significantly at 4 months and was maintained at 1 year after surgery.

We have compared levels of albumin and serum amino acids in a group of 87 recent admissions to a nursing home, average age 83 years, with a group of healthy moderately old subjects, average age 69 years. We found that the nursing home group was characterized by decreased levels of albumin, by increased total levels of the measured amino acids, and by increased levels of the nonessential amino acids. In contrast, there were no significant group differences in the essential amino acids. Among the nursing home patients, there was a negative correlation between essential amino acids and disability

All 225 tenants (181 female and 44 male patients) of a nursing home in New York City underwent eye examinations at least once within a 1-year period to determine the prevalence of senile cataract, age-related macular degeneration (AMD), open-angle glaucoma, and diabetic retinopathy. The average age was 85.4 years (range, 60-108 years). With aphakia and pseudophakia included in the diagnosis of "cataract," the respective prevalences were found to be 81, 37, 11, and 2.1%. There was a statistically significant increase in the prevalence of cataracts and AMD in those patients 85 years of age or ol
