Long-term efficacy of olanzapine in the control of psychotic and behavioral symptoms in nursing home patients with Alzheimer's dementia.

BACKGROUND: Psychotic symptoms and behavioral disturbances are a leading cause of institutionalization in elderly patients with Alzheimer's disease (AD). OBJECTIVES: Elderly nursing home patients (n=105) with possible or probable AD were entered into a multicenter study to determine the long-term efficacy and safety of olanzapine in treatment of psychotic symptoms and behavioral disturbances due to AD. METHODS: Following a double-blind, 6-week exposure to fixed-dose olanzapine (5, 10, or 15 mg/d), patients entered an additional 18-week, open-label, flexible-dose treatment. Baseline was defin

Development and Validation of Machine Learning Models for Predicting Early Cognitive Decline Using Home Sensor-Derived Behavioral Data: Sensors in-Home for Elder Wellbeing (SINEW) Cohort Study.

BACKGROUND: As the global population continues to age, the prevalence of geriatric conditions, including dementia and frailty, is also increasing. Early identification of individuals at an elevated risk of these conditions, such as those presenting with mild cognitive impairment (MCI) or prefrailty, can provide a critical window for prompt intervention aimed at preventing or reversing disease progression. To promote such early identification, there is a burgeoning interest in the use of digital sensor technology and predictive modeling. OBJECTIVE: This study aimed to use a continuous, home-ba

Evaluation of the "Los Filabres" Protocol on Behavioral and Psychological Symptoms of Dementia and Psychotropic Drug Use in Nursing Home Residents.

Background/Objectives: Behavioral and psychological symptoms of dementia (BPSD) are highly prevalent among nursing home residents and represent a major clinical and care-related challenge. These symptoms are frequently associated with increased psychotropic drug use despite limited efficacy and important safety concerns. This study aimed to evaluate longitudinal changes in BPSD and psychotropic drug use following implementation of the "Los Filabres" protocol, a structured person-centered non-pharmacological intervention, in nursing home residents with dementia. Methods: A single-arm longitudin

Home-cage monitoring as a sensitive tool for detecting subtle behavioral alterations following mild traumatic brain injury.

Home-cage monitoring (HCM) has emerged as a powerful approach for detecting subtle and context-dependent behavioral alterations in rodent models. In this mini-review, we integrate clinical observations and preclinical data from the closed-head weight-drop model, emphasizing studies from our laboratory, to argue that mTBI should be conceptualized as a disorder of behavioral regulation rather than of basic behavioral capacity. We first outline the limitations of short, novelty-driven gold-standard assays (e.g., Open Field, Elevated Plus Maze, Rotarod), which often indicate intact motor function

Behaviorally Informed Interventions to Promote Activity in the Home and Community for Adults With Neurological Disorders: A Systematic Review and Meta-Analysis.

IMPORTANCE: Physical activity can improve clinical outcomes among people with neurological conditions; however, people with these conditions rarely engage in recommended levels of activity. Remote monitoring (RM) with the incorporation of behavior change strategies is purported to be an effective approach to promote increased physical activity in the home setting, however, its effectiveness in promoting activity for people with neurological conditions is unclear. OBJECTIVES: The objectives of this review were to examine the effectiveness of behaviorally informed RM interventions on physical a

Behavioral symptoms, depression symptoms, and medication use in Michigan nursing home residents with dementia during COVID-19.

BACKGROUND: The COVID-19 pandemic significantly disrupted nursing home (NH) care, including visitation restrictions, reduced staffing levels, and changes in routine care. These challenges may have led to increased behavioral symptoms, depression symptoms, and central nervous system (CNS)-active medication use among long-stay NH residents with dementia. METHODS: We conducted a retrospective, cross-sectional study including Michigan long-stay (≥100 days) NH residents aged ≥65 with dementia based on Minimum Data Set (MDS) assessments from January 1, 2018 to June 30, 2021. Residents with schizoph

Virtual Reality Experience Intervention May Reduce Responsive Behaviors in Nursing Home Residents with Dementia: A Case Series.

BACKGROUND: People with advanced dementia often exhibit responsive behaviors such as apathy, depression, agitation, aggression, and psychosis. Non-pharmacological approaches (e.g., listening to music, watching television, doing arts and crafts) are now considered as a first-line strategy to manage responsive behaviors in clinical practice due to the potential risks associated with the antipsychotic medications. To date, no evaluations of immersive non-head mounted virtual reality (VR) experience as a non-pharmacologic approach for people with advanced dementia living in nursing homes have been

[The home for the aged: a source of growing concern (2). Behavior assessment over a 2 1/2-year period].

In 1983 we published the results of 12 months of behaviour assessment in an old people's home. We noticed an increase in physical and psychosocial problems. In the following 18 months we observed a further increase, both in the cross-sectional group as well as in the longitudinal group. Newly admitted people have rather serious problems from the onset. Furthermore, there is a marked increase from then on. The most vulnerable category consists of those elderly who die or are relocated to a nursing home within a short period of time. In the past the temporary increase in care for this group was

Withdrawn at home, withdrawn from school: Internalizing behaviors and school absenteeism in low-income children post-Covid.

Following the COVID-19 pandemic, school absenteeism has remained severely elevated, with significant repercussions for educational and social-emotional outcomes. In order to increase understanding of factors contributing to absenteeism, this study examined the relationship between internalizing behaviors and school attendance and the extent to which that relationship is moderated by age and gender among low-income children living in a community of concentrated poverty (N=145; age range=5-17). Poisson regression models found that internalizing behaviors were significantly associated with school

Behavior determines times to care home admission and death in semantic dementia: Insights from a cohort of 90 cases.

INTRODUCTION: In semantic dementia (SD), no consistent prognostic factors for early care home (CH) admission or mortality have been identified, despite their potential utility in care management. METHODS: Over a 31-year period, we collected sociodemographic, neuroimaging, neuropsychological, and behavioral data within a large cohort of 90 SD patients. Survival and risk factor analyses were conducted to identify variables mostly influencing CH admission and/or death. RESULTS: Half of the patients were admitted to CH 7.5 years after diagnosis, and half died after 8.4 years. Behavior emerged as

Operant ethanol self-administration behaviors do not predict sex differences in continuous access home cage drinking.

Understanding sex differences in disease prevalence is critical to public health, particularly in the context of alcohol use disorder (AUD). The goal of this study was to understand sex differences in ethanol drinking behavior and define the precise conditions under which sex differences emerge. Consistent with prior work, C57BL/6J females drank more than males under continuous access two-bottle choice conditions. However, using ethanol self-administration - where an operant response results in access to an ethanol sipper for a fixed time period - we found no sex differences in operant respons

Temporal Associations between Caregiving Approach, Behavioral Symptoms and Observable Indicators of Aspiration in Nursing Home Residents with Dementia.

OBJECTIVES: Dysphagia, or impaired swallowing, is common in nursing home (NH) residents with dementia and contributes to malnutrition and diminished quality of life. Dysphagia also commonly leads to aspiration or passage of food or fluids into the airway, which can result in aspiration pneumonia-a leading cause of death for people with dementia. Currently available interventions for dysphagia aim to modify the risk of aspiration events primarily by modifying diet and positioning to improve the safety of an individual's swallow. However other potentially modifiable contextual factors relevant t

No evidence of compensatory drug use risk behavior among heroin users after receiving take-home naloxone.

INTRODUCTION: Some fear that distribution of naloxone to persons at risk of experiencing an opioid overdose may reduce the perceived negative consequences of drug use, leading to riskier patterns of use. This study assessed whether participation in naloxone/overdose training altered drug use frequency, quantity or severity among heroin users in and out of treatment. METHODS: Clinical interviews were performed assessing patterns of heroin and other drug use prior to, and at multiple timepoints after overdose education and naloxone training. This study compared baseline drug use to that at 1 an

Use of electric field sensors for recording respiration, heart rate, and stereotyped motor behaviors in the rodent home cage.

BACKGROUND: Numerous environmental and genetic factors can contribute significantly to behavioral and cardiorespiratory variability observed experimentally. Affordable technologies that allow for noninvasive home cage capture of physio-behavioral variables should enhance understanding of inter-animal variability including after experimental interventions. NEW METHOD: We assessed whether EPIC electric field sensors (Plessey Semiconductors) embedded within or attached externally to a rodent's home cage could accurately record respiration, heart rate, and motor behaviors.
