Development and present state of hormone replacement therapy.

The first therapeutic trials to evaluate the treatment of climacteric symptoms by the administration of ovarian tissue were reported in 1896, i.e. before the endocrine function of the ovary was known or estrogens had been discovered. These were followed by investigations using various ovarian, placental and urine extracts, implantation of ovarian tissue and by oral treatment with dried ovarian tissue. However, reasonably acceptable hormonal drugs did not become available until the third decade of this century, while drugs meeting modern requirements, including that of oral activity, have been

Shouldn't adults with growth hormone deficiency be offered growth hormone replacement therapy?

Growth hormone as therapy for adults with growth hormone deficiency has not been universally accepted by endocrinologists who treat adult patients. The following are addressed in this commentary: the evidence on safety and efficacy in the literature supporting the idea that growth hormone should be offered as replacement therapy to adults who are growth hormone deficient; common concerns of the average prescribing endocrinologist, including the purported association between insulin-like growth factor-I and malignant neoplasms and quality-of-life issues with long-term therapy; and controversial

Histological signatures of hormone replacement therapy in the endometrium.

BACKGROUND: Hormone replacement therapy (HRT) is widely prescribed for managing menopausal symptoms, yet its effects on the endometrium remain clinically important. While large-scale studies have established that estrogen-only HRT increases endometrial cancer risk and continuous combined regimens reduce this risk, real-world data on endometrial histology remain valuable for understanding contemporary practice patterns. METHODS: We conducted a retrospective analysis of women who underwent hysteroscopy with endometrial sampling at Princess Royal University Hospital. Of the 62 patients, we inclu

Update on the cardiovascular risks of hormone replacement therapy.

Systematic reviews of randomized, controlled trials have shown that use of hormone replacement therapy in women increases the risk of total cardiovascular disease (venous thromboembolism, stroke and coronary heart disease; there may also be an increased risk of peripheral arterial disease). The relative increase in risk is similar to that for use of combined oral contraceptives, but the absolute increase in risk is higher owing to the higher age of hormone replacement therapy users. The increased risk appears confined to current users, increases with age and obesity, and may differ with type o

Hormonal replacement therapy (HRT) in postmenopause: a reappraisal.

Hormone replacement therapy (HRT) is the most effective treatment currently available for vasomotor and urogenital symptoms and decreased libido. Because harmful effects were evidenced in some clinical trials, health authorities now consider that risk-benefit considerations do not favour the use of HRT for prevention of cardiovascular diseases and bone fractures in postmenopausal women. However, experimental and clinical studies indicate that adverse effects of HRT may largely depend on the estrogen and progesterone/progestin formulation, dosage, mode of administration, patient's age, associat

Evaluation of the benefits and risks of hormone replacement therapy.

OBJECTIVE: To review recent data on outcome of hormone replacement therapy in postmenopausal women. DATA SOURCES: Medline search of the literature and local data. STUDY SELECTION: Data on efficacy of hormone replacement and its unwanted side effects were examined. DATA EXTRACTION: Statistical data were extracted from published studies and meta-analyses. DATA SYNTHESIS: Statistical data came mainly from observational studies and not from randomised trials, and were therefore subject to bias. Overall, it would appear reasonable to say that risks for breast and endometrial cancers, as well as

Impact of hormone replacement therapy on serum thyroid-stimulating hormone: a retrospective analysis.

RESEARCH QUESTION: What is the impact of hormone replacement therapy (HRT) on serum thyroid-stimulating hormone (TSH) during endometrial preparation for single-embryo transfer (SET)? DESIGN: This retrospective cohort study included 516 women (≤49 years) who underwent SET with either autologous or donor oocytes, between January 2022 and January 2023 at IVI Rome. TSH was measured at baseline (T0), during endometrial thickness assessment (T1), on embryo transfer day (T2), and at the pregnancy test (T3). TSH >2.5 mU/l was treated with levothyroxine. HRT consisted of oral estradiol valerate (6 mg/

Growth hormone replacement therapy in adults with growth hormone deficiency improves vascular reactivity.

OBJECTIVE: Patients with adult growth hormone (GH) deficiency are thought to be of increased risk of cardiovascular disease. Impaired vascular reactivity to endothelium derived nitric oxid (NO) is an early event in the development of atherosclerosis. In order to detect a possible effect of GH on vascular endothelium we examined forearm vasodilator responses in 8 patients with adult GH-deficiency before and after 3 months GH replacement therapy. METHODS: Forearm blood flow studies were performed using venous occlusion plethysmography. Blood flow was measured at baseline and during intra-arteri

Thyroid hormone replacement therapy in dialysis/renal insufficiency patients.

Dialysis/renal insufficiency patients are often accompanied by hypothyroidism due to renal damage, the mechanisms of which are complex. The use of thyroid hormone replacement therapy in such patients has become an important clinical issue. This article reviews the mechanism of hypothyroidism in dialysis/renal insufficiency patients and describes the importance and precautions of thyroid hormone replacement therapy to provide a reference for clinical diagnosis and treatment.

Effect of Hormone Replacement Therapy on Chronic Rhinosinusitis Management.

OBJECTIVES: To investigate whether hormone replacement therapy (HRT) impacts health care resource utilization in the management of chronic rhinosinusitis (CRS) in older women. METHODS: Using the TriNetX US health record database, women 55 years or older with a diagnosis of CRS were included and followed for 3 years. The cohort was stratified into two groups: women who received HRT at the beginning of the study were compared to women who did not receive HRT. The groups were matched by age, race, ethnicity, history of asthma, and history of nasal polyps. Outcomes included whether the patient un

Hormone replacement therapy after surgery for epithelial ovarian cancer.

BACKGROUND: Women who have undergone surgical treatment for epithelial ovarian cancer (EOC) may develop menopausal symptoms due to immediate loss of ovarian function following surgery and chemotherapy. Women may experience vasomotor symptoms, sleep disturbance, difficulty concentrating, sexual dysfunction, vaginal symptoms and accelerated osteoporosis. Although hormone replacement therapy (HRT) is the most effective treatment to relieve these symptoms, its safety has been questioned for women with EOC. OBJECTIVES: To assess the safety and efficacy of HRT for menopausal symptoms in women surgi

[Effects of growth hormone replacement therapy on bone metabolism].

Growth hormone (GH) as well as insulin like growth factor-1 (IGF-1) are essential hormones to maintain homeostasis of bone turnover by activating osteoblastogenesis and osteoclastogenesis. Results from GH replacement therapy for primary osteoporosis and adult-onset GH deficiency (AGHD) suggest that one year or more treatment period by this agent is required to gain bone mineral density (BMD) over the basal level after compensating BMD loss caused by dominant increase in bone resorption which was observed at early phase of GH treatment. A recent meta-analysis demonstrates the efficacy of GH rep

Hormone replacement therapy increases the risk of cranial meningioma.

AIM: We investigated the influence of hormone replacement therapy (HRT) use on the risk of meningioma in a population-based setting. METHODS: We conducted a nationwide case-control study in Denmark based on population-based administrative and health registries. The study included all female patients aged 55-84 years with a first time diagnosis of meningioma during 2000-2009. The cases were matched on birth year with female population controls. Ever use of HRT since 1995 was defined as ≥2 HRT prescriptions and categorised according to HRT type (oestrogen only, combined oestrogen-progestagen, a

Periodontal aspects in menopausal women undergoing hormone replacement therapy.

AIMS: Currently, an important number of women use HRT to control their hormonal problems during menopause. A large percentage of these have problems at periodontal level. The present study aims at examining the effects that menopause, due to a decline in the synthesis of hormones, mainly of estrogens, can cause on the oral dental health of such women; in particular on the characteristics of the gingiva and periodontium, checking whether characteristics such as gingival recession, pain, tooth mobility and periodontal pocket formation might permit physicians to evaluate the degree of bone loss i

Hormone replacement therapy and endometrial, ovarian and colorectal cancer.

Sex-steroid-related tumours in women are represented by breast cancer and endometrial cancer, but a possible relationship may exist between sex steroids and both ovarian and colon cancer. Unopposed oestrogen therapy is known to increase the risk of endometrial cancer and is appropriate only for hysterectomized women. In women with an intact uterus, an appropriate combination of oestrogen and progestin does not appear to increase-and may even decrease-the risk of endometrial cancer. Current users of HRT seem to benefit from a reduced risk for colon cancer. As for epithelial ovarian cancer, the

Women's knowledge of and attitudes towards hormone replacement therapy.

BACKGROUND: Hormone replacement therapy (HRT) could benefit women who have reached the natural menopause, have had a hysterectomy or have a family history of osteoporosis. OBJECTIVE: Our aim was to monitor changes in women's knowledge of, and attitudes towards, HRT since 1991. METHODS: The study was a repeat of a postal survey conducted in 1991 in the Grampian region in the North East of Scotland. Six hundred women, aged 20-69 years, were selected randomly from the eight Local Health Care Co-operatives in Grampian, Scotland. The main outcome measures were women's knowledge of HRT, their atti
