Management of blood pressure after acute ischemic stroke: An evidence-based guide for the hospitalist.

Hospitalists are frequently called upon to manage blood pressure after acute ischemic stroke. A review of both post infarction cerebral perfusion physiology and the data from randomized trials of antihypertensive therapy is necessary to explain why consensus guidelines for blood pressure management after stroke differ from those of other hypertensive emergencies. The peri-infarct penumbra is the central concept in understanding post ischemic cerebral perfusion. This area of impaired cerebral blood flow is dependent on mean arterial blood pressure and acute reduction of blood pressure may expan

Why the 2025 Nobel Prize in Economics Matters at the Bedside: Innovation, Institutions, and Hospitalist Leadership.

The 2025 Nobel Prize in Economics, awarded to Joel Mokyr, Philippe Aghion, and Peter Howitt for their work on innovation-driven growth, offers an unexpectedly practical framework for hospital medicine. Their scholarship emphasizes that progress depends not only on technological advances, but also on institutions capable of implementing, governing, and adapting innovation in real operational settings. This perspective resonates with hospital medicine, a field born from institutional redesign and now facing a new inflection point driven by digital technology and artificial intelligence. Hospital

Implementation of a comprehensive hospitalist-led initiative to improve care for patients with opioid use disorder.

BACKGROUND: As opioid-related hospitalizations rise, hospitals must be prepared to evaluate and treat patients with opioid use disorder (OUD). We implemented a hospitalist-led program, Project Caring for patients with Opioid Misuse through Evidence-based Treatment (COMET) to address gaps in care for hospitalized patients with OUD. OBJECTIVE: Implement evidence-based treatment for inpatients with OUD and refer to postdischarge care. DESIGN, SETTING, AND PARTICIPANTS: Project COMET launched in July 2019 at Duke University Hospital (DUH), an academic medical center in Durham, NC. INTERVENTION,

Introducing the Hospitalist Morale Index: A new tool that may be relevant for improving provider retention.

OBJECTIVE: To develop a valid instrument to assess morale and explore the relationship between morale and intent to leave employment due to unhappiness. PATIENTS AND METHODS: An expert panel identified 46 drivers of hospitalist morale. In May 2009, responders of our single-site pilot survey rated each driver in terms of current contentment and importance to their morale. With exploratory factor analysis, a 28-item/7-factor instrument emerged. In May 2011, the refined scale was distributed to 108 hospitalists from 2 academic and 3 community hospitals. Confirmatory factor analysis (CFA) was use

Assessing the clinical relevance of point-of-care ultrasound for hospitalists: Influence on clinical reasoning and decision-making.

INTRODUCTION: Point-of-care ultrasonography (POCUS) is increasingly recognized in internal medicine, yet its influence on hospitalists' clinical reasoning remains underexplored. OBJECTIVES: This study aimed to evaluate the influence of POCUS on diagnostic hypotheses, management strategies, and confidence levels among hospitalists in a transitional care unit with limited access to advanced imaging. METHODS: Prospective descriptive study in a transitional care unit enrolling 19 hospitalists. During routine care, clinicians used POCUS at their discretion and, immediately afterward, completed a

Comparative salary-related costs of a brief app-directed delirium identification protocol by hospitalists, nurses, and nursing assistants.

BACKGROUND: Systematic screening can improve delirium identification among hospitalized older adults. Prior studies have shown clinicians and health system leaders may believe they do not have the time and resources for assessment. We conducted a comparative salary-related cost analysis of an adaptive delirium identification protocol directed by an iPad app. METHODS: We recruited 527 older adult medicine patients from an urban academic medical center (n = 269) and a rural community hospital (n = 258). Physicians and nurses completed the two-step Ultra-brief Confusion Assessment Method (UB-CAM

Society of Dermatology Hospitalists supportive care guidelines for the management of Stevens-Johnson syndrome/toxic epidermal necrolysis in adults.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening conditions with high morbidity and mortality. Supportive care management of SJS/TEN is highly variable. A systematic review of the literature was performed by dermatologists, ophthalmologists, intensivists, and gynecologists with expertise in SJS/TEN to generate statements for supportive care guideline development. Members of the Society of Dermatology Hospitalists with expertise in SJS/TEN were invited to participate in a modified, online Delphi-consensus. Participants were administered 9-point Likert sca

Timing of Renal and Bladder Ultrasound After First Febrile Urinary Tract Infection: A Hospitalist Dilemma.

BACKGROUND: Renal/bladder ultrasonography (RBUS) is often obtained before discharge in hospitalized children with first febrile urinary tract infection (fUTI). However, dilatation or uroepithelial thickening (UET) during infection may be transient and thus confound interpretation. We aimed to examine practice patterns of timing of RBUS, assess the significance of abnormal findings relative to fever, and identify the incidence of patient lost to follow-up. METHODS: We performed a multicenter, retrospective study of previously healthy children aged 2 to 24 months hospitalized for first fUTI in

Unplanned 30-Day Readmissions in a General Internal Medicine Hospitalist Service at a Comprehensive Cancer Center.

PurposeHospital readmissions are considered by the Centers for Medicare and Medicaid as a metric for quality of health care delivery. Robust data on the readmission profile of patients with cancer are currently insufficient to determine whether this measure is applicable to cancer hospitals as well. To address this knowledge gap, we estimated the unplanned readmission rate and identified factors influencing unplanned readmissions in a hospitalist service at a comprehensive cancer center.MethodsWe retrospectively analyzed unplanned 30-day readmission of patients discharged from the General Inte
