Nosocomial bacteraemia in a teaching hospital in Saudi Arabia.

During a period of one year, 117 episodes of nosocomial bacteraemia were documented at King Khalid University Hospital, an incidence of 5 per 1000 admissions. Sixty-two percent were gram-negative organisms with Escherichia coli, Klebsiella spp., Serratia spp. and Pseudomonas spp. being the most frequent. Staphylococcus aureus was the most common gram-positive organism isolated. The source of infection was identified in 75% of patients. Intravenous lines accounted for a high proportion of cases (22%). Most deaths occurred in infants and patients with serious underlying disease.

The accuracy of diagnosing pulmonary tuberculosis at a teaching hospital.

Accuracy of the clinical diagnosis of tuberculosis and of the mycobacteriology laboratory test results was assessed in a teaching hospital by reviewing clinical and microbiologic data on patients from whom Mycobacterium tuberculosis had been recovered. Mycobacteria were isolated in 230 of 6,550 specimens (3.5%). Clinical data were available for 42 patients with tuberculosis, 20 of whom had significant underlying nonmycobacterial disease. Positive tuberculin skin tests wee recorded for 90% of the patients with no underlying disease and for 29% of the patients with underlying disease. Tuberculos

Bacteraemia in a London teaching hospital 1966-75.

Analysis of blood-cultures done at St. Bartholomew's Hospital over the decade 1966-75 has revealed a five-fold increase in the incidence of bacteraemia. This increase was particularly prominent in patients with non-malignant disease. The predominance of bacteraemia caused by gram-negative organisms declined in recent years, whereas bacteraemia due to gram-positive organisms, anaerobes, and Candida spp. increased. A recent increase in the number of blood-cultures performed was associated with the introduction of commercially prepared bottles. This led to a higher rate of investigation of patien

INTRODUCTION/BACKGROUND: Ankylosing spondylitis (AS), a type of spondyloarthropathy, is an autoimmune disease that mainly involves spine joints, sacroiliac joints and their adjacent soft tissues, such as tendons and ligaments. Progression of disease can lead to fibrosis and calcification, resulting in the loss of flexibility and mobility of the spine. The common clinical presentation is inflammatory back pain which is often neglected. The aim of our study was to assess the demographic and clinical proflie of patients of ankylosing spondylitis diagnosed on the basis of radiographic sacroillitis

BACKGROUND: Hypoglycaemia is an important metabolic condition in the neonate although there is no common consensus with regards to diagnosis, management and follow-up. It is common in preterm, small and large for gestational age and in birth asphyxia. Although the incidence of these predisposing factors is common among neonates in Ghana, the prevalence of neonatal hypoglycaemia (NH) has not been studied in this environment. AIM AND OBJECTIVES: To determine the prevalence of neonatal hypoglycaemia at the Cape Coast Teaching Hospital, Ghana. DESIGN: Cross sectional. SETTING: Cape Coast Teachi

Understanding and improving the use of allopurinol in a teaching hospital.

BACKGROUND: An emphasis on renal function in deciding maintenance doses of allopurinol to prevent allopurinol hypersensitivity has resulted in ineffective prevention of attacks of gout. New therapeutic guidelines for gout have shifted the focus back to titrating maintenance doses to reach a serum uric acid (SUA) concentration target of ≤ 0.36 mmol/L. AIMS: To examine trends in the prescribing of allopurinol in a teaching hospital and their concordance with the new guidelines for gout management, and to explore prescribers' approaches and attitudes to the use of allopurinol. METHODS: An audit

Horizontal strabismus surgical outcomes in a teaching hospital.

Strabismus may result in impaired stereopsis, diplopia, undesirable appearance, amblyopia and negative psychological impact. This study provides epidemiological and surgical outcome information about patients attending University College Hospital Galway requiring strabismus surgery. We report a retrospective analysis of 75 consecutive patients, who underwent horizontal strabismus surgery. Sixty-one (81.3%) patients had clinically significant refractive errors, hyperopia being the most common. Thirty-four (45.3%) patients had amblyopia and nine (12%) required further treatment. Postplacental intrauterine device insertion at a teaching hospital.

OBJECTIVE: To determine whether postplacental intrauterine device (IUD) insertion can be safely and effectively performed within a teaching program. STUDY DESIGN: This was a prospective cohort of 177 subjects planning vaginal delivery enrolled antenatally who desired postplacental IUD insertion of either the copper T380A IUD or levonorgestrel IUS. Insertions were performed primarily by resident physicians following a training session. Follow-up included a 4- to 8-week visit and telephone calls at 3 and 6 months. RESULTS: Ninety-nine subjects underwent successful postplacental IUD insertion o

Analysis of antimicrobial consumption and cost in a teaching hospital.

BACKGROUND: The aim of this study is to compare the periods before and after the intervention applied using the ATC/DDD method in order to ascertain the rational use of antibiotics in a newly established hospital. METHOD: The appropriateness of the hospital's antibiotic use, consumption rates and the costs were calculated and compared with other hospitals. Based on these data, an intervention has been planned in order to raise the quality of antibiotic use. The periods before and after the intervention were compared. Between 16 May 2011 and 23 May 2012, data were collected from all hospital u

Machete injuries as seen in a Nigerian teaching hospital.

BACKGROUND: The morbidity and mortality of machete-related injuries is rather appreciable in Nigeria although it is under-reported. Machetes are multipurpose tools in the farm, home and construction sites. Machetes may also be used as a close-range weapon. In Nigeria, there is no legislation guiding their Acquisition, so they are readily available in many homes. Injuries from machetes could be as detrimental as fatal or non-fatal with long-term morbidity. METHODS: We performed a retrospective study to assess the pattern and outcome of patients presenting with machete injuries in Ebonyi State

Cases of melioidosis in a university teaching hospital in Malaysia.

BACKGROUND AND PURPOSE: Melioidosis is an infectious disease caused by Burkholderia pseudomallei that is endemic in Southeast Asia and northern Australia and has also been reported from non-endemic areas of the world.
