Proximal humerus fractures (PHFs) are frequently encountered by upper-extremity surgeons. Despite recent advances in both surgical techniques and implant designs, numerous management controversies exist that are primarily centered on indications for operative treatment as well as the decision to use fixation techniques or arthroplasty in the management of these injuries in older patients. The introduction and widespread adoption of reverse total shoulder arthroplasty in the management of three- and four-part PHFs represents a paradigm shift with respect to operative treatment options. BACKGROUND: The anatomical variations and deviations from the expected intertubercular sulcus morphometry are associated with the physiology of the long head of the biceps brachii tendon. These variations may affect tendon biomechanics, potentially impacting shoulder joint stability.
AIM: to evaluate the osteometric features of the intertubercular sulcus, including its dimensions, angles, and bony protrusions, and to compare these variables according to side and sex.
METHODS: The descriptive, cross-sectional study included a sample of 85 humeri. Morphometric analysis involved measurements of

BACKGROUND: Osteosarcoma is the most common primary bone tumor in children. While the humerus is the third most common anatomic location, it is the most common site for fractures. Our objective is to characterize magnetic resonance imaging (MRI) features of humerus osteosarcoma in children and with respect to risk of sustaining a pathologic fracture.
METHODS: Children with histologically confirmed humerus osteosarcoma, who underwent preoperative MRI (2008-2023) were included. Two radiologists retrospectively reviewed all MRIs to determine medullary (skip lesions and trans-epiphyseal tumor), j

The patient was a 41-year-old man who injured his right shoulder falling onto his outstretched hand while skiing. Radiographs taken in the emergency department demonstrated a 3-part fracture dislocation of the proximal humerus for which he underwent same-day surgery. Physical therapy was initiated on postoperative day 27, and postoperative radiographs and physical examination results assisted in devising an appropriate plan of care and treatment. J Orthop Sports Phys Ther 2018;48(9):728. doi:10.2519/jospt.2018.6208.

Humeral fractures typically occur as the result of direct, external trauma. Here however, we describe the case of a young, amateur athlete presenting with acute right, upper arm pain after throwing a ball. Examination showed right upper arm deformity and tenderness to palpation, without any distal neurovascular deficits. X-ray demonstrated a spiral fracture of the humerus. The patient had operative repair of the injury several days later, with no complications noted on outpatient visits up to 3 months later.

Supracondylar fractures of the humerus are the most frequent fractures of the paediatric elbow, with a peak incidence at the ages of five to eight years.Extension-type fractures represent 97% to 99% of cases. Posteromedial displacement of the distal fragment is the most frequent; however, the radial and median nerves are equally affected. Flexion-type fractures are more commonly associated with ulnar nerve injuries.Concomitant upper-limb fractures should always be excluded. To manage the vascular status, distal pulse and hand perfusion should be monitored. Compartment syndrome should always be

Proximal humerus, humeral shaft, and distal humerus fractures are all common adult fractures, and often occur in older patients. While the treatment of proximal humerus fractures remains controversial, certain fractures benefit from plate fixation such as fracture-dislocations and head-split fractures. When plate fixation is chosen, anatomic reduction and restoration of the medial calcar are important for successful results. Further research is required to minimize complications and determine the optimal surgical candidates for plate fixation. Humeral shaft fractures are generally treated non-

Obstetric traumatic separation of the distal humeral epiphysis is a very uncommon injury, which presents a diagnostic challenge. These case serials reviewed the functional outcomes of 5 patients who had sustained a fracture-separation of the distal humeral epiphysis at birth. The diagnosis was made at a mean time of 40.8 h after delivery. All the patients were treated with gentle close manipulation, reduction under fluoroscopy and above-elbow cast application. After discharge, the patients were followed up for a mean of 30 months. Clinico-radiological results were excellent in four patients. O

Distal humerus fractures present complex challenges in the elderly patient. These fractures often occur in patients who are living independently but have poor bone quality and low physiologic reserve, thus complicating management decisions and treatment. The goal is a painless, functional, stable elbow that allows completion of the activities of daily living. Nonsurgical management is reserved for those who cannot tolerate surgery. Open reduction and internal fixation is the preferred choice in fractures amenable to rigid fixation and early motion. Although total elbow arthroplasty provides im

Charcot's arthropathy or neuropathic osteoarthropathy is a form of destructive arthropathy associated with decreased sensory innervation. We present an unusual case of a patient referred to oncology clinic for further evaluation of a spindle cell neoplasm of the humerus concerning for malignancy. The mass presented as a lytic lesion associated with a right humerus fracture. This was later diagnosed as Charcot's shoulder secondary to cervical syringomyelia. A detailed case report and discussion are presented here.

Proximal humerus fractures are rare in paediatric traumatology. Metaphyseal fractures account for about 70% of cases and epiphyseal separation for the remaining 30%. The development and anatomy of the proximal humerus explain the various fracture types, displacements, and potential complications; and also help in interpreting the radiographic findings, most notably in young children. Physicians should be alert to the possibility of an underlying lesion or pathological fracture requiring appropriate diagnostic investigations, and they should consider child abuse in very young paediatric patient

The majority of proximal humerus fractures are low-energy osteoporotic injuries in the elderly and their incidence is increasing in the light of an ageing population. The diversity of fracture patterns encountered renders objective classification of prognostic value challenging. Non-operative management has been associated with good functional outcomes in stable, minimally displaced and certain types of displaced fractures. Absolute indications for surgery are infrequent and comprise compound, pathological, multi-fragmentary head-splitting fractures and fracture dislocations, as well as those

Knowledge of regional and topographic anatomy is paramount for success when using minimally invasive plate osteosynthesis (MIPO) for fracture management. Preoperative planning is essential for an optimal outcome and reducing stress among the surgical team; factors to consider include biologic assessment, mechanical assessment, clinical assessment, portal placement, and implant selection. MIPO is a useful technique for the direct or indirect reduction of humeral diaphyseal fractures. Implants should span the length of the bone for ease of implant application and to optimize the mechanical advan

Management of coronal shear fractures of the distal humerus has evolved considerably over the past 3 decades, with an increased appreciation of the complexity of these fractures, improvements in internal fixation techniques, and the use of more extensile exposures. Nearly all of these fractures are displaced, given the paucity of soft tissue attachments and correspondingly, nonsurgical management is fraught with complications including chronic pain, mechanical symptoms, and instability and is not recommended. Good to excellent outcomes can be achieved in the majority of patients with open redu

Proximal humeral fractures account for 4% to 5% of all fractures. Most of these fractures are nondisplaced or minimally displaced and amenable to nonsurgical treatment or open reduction and internal fixation. Complex proximal humeral fractures with displaced three- and four-part fragments, fracture-dislocations, and humeral head splits are more difficult to treat. In older patients, hemiarthroplasty or reverse shoulder arthroplasty is often the indicated treatment. Arthroplasty in this patient cohort is very technique-dependent and relies on preserving deltoid function, proper component placem

Supracondylar fractures of the humerus are a common pediatric elbow injury that are historically associated with morbidity due to malunion, neurovascular complications, and compartment syndrome. True anteroposterior and lateral radiographs are essential not only for an accurate diagnosis, but also for creating a treatment plan for these injuries. A staging system (based on the lateral radiograph) for classifying the severity of the fracture helps guide definitive management. Nondisplaced fractures are treated initially with a posterior splint, followed by a long-arm casting. Closed reduction a

A 14-year-old intact male Maltese dog was presented with a history of pain and swelling of the left upper forelimb and lameness for 3 weeks. Hematological and radiographical examinations showed regenerative anemia and osteolysis of the humerus. Fine needle aspiration biopsy detected epithelioid- and fibroblast-like anaplastic cells with blood components. A hemorrhagic and osteolytic malignant tumor was suspected, and the affected forelimb was amputated. Histopathologically, the dog was diagnosed with primary hemangiosarcoma of the humerus. Thereafter, metastatic lesions appeared in the skin, a

BACKGROUND: The purpose of this retrospective study was to identify the patterns of distal humeral articular fractures and to analyze the results of open reduction and internal fixation of these injuries.
METHODS: The cases of twenty-one patients with an articular fracture of the distal part of the humerus were reviewed at an average of forty months after the injury. Five components of the injury were identified: (1) the capitellum and the lateral aspect of the trochlea, (2) the lateral epicondyle, (3) the posterior aspect of the lateral column, (4) the posterior aspect of the trochlea, and (
