Wells' Syndrome Mimicking Facial Cellulitis: A Report of Two Cases.

Wells' syndrome (WS), or eosinophilic cellulitis, is an uncommon inflammatory dermatosis of unknown etiology that typically presents with pruritic cellulitis-like plaques on the extremities. Therefore, WS is often misdiagnosed as bacterial cellulitis due to its similarity in presentation. Here, we report two cases of WS that masqueraded as bacterial facial cellulitis. Under treatment with oral prednisone and/or a combination therapy with levocetirizine and hydroxyzine, both patients showed a dramatic improvement of the skin lesions. These cases highlight the need for clinicians to consider WS

Clinical pharmacokinetics of H1-receptor antagonists (the antihistamines).

This article reviews clinical pharmacokinetic data on the H1-receptor antagonists, commonly referred to as the antihistamines. Despite their widespread use over an extended period, relatively little pharmacokinetic data are available for many of these drugs. A number of H1-receptor antagonists have been assayed mainly using radioimmunoassay methods. These have also generally measured metabolites to greater or lesser extents. Thus, the interpretation of such data is complex. After oral administration of H1-receptor antagonists as syrup or tablet formulations, peak plasma concentrations are usua

[Wells syndrome mimicking facial cellulitis: Three cases].

BACKGROUND: Wells syndrome, or eosinophilic cellulitis, is an uncommon inflammatory dermatosis of unknown etiology that is characterized by clinical features of pruritic cellulitis-like plaques on the extremities and a histological picture of eosinophilic infiltrate of the dermis with "flame figures". PATIENTS AND METHODS: Herein, we report three cases of idiopathic Wells syndrome masquerading as bacterial facial cellulitis. Under treatment with oral prednisone and/or combined therapy with levocetirizine and hydroxyzine, all patients showed a dramatic improvement of their skin lesions. DISCU

Cetirizine. A reappraisal of its pharmacological properties and therapeutic use in selected allergic disorders.

Cetirizine, the carboxylated metabolite of hydroxyzine, is a specific and long-acting histamine H1-receptor antagonist. It has marked antiallergic properties and inhibits eosinophil chemotaxis during the allergic response. Clinical trial results indicate that cetirizine is an effective and well tolerated treatment for seasonal/perennial allergic rhinitis and chronic idiopathic urticaria in adults, and for seasonal/perennial allergic rhinitis in children. Cetirizine 10 mg/day appears to be as effective as conventional dosages of other established antihistamines such as astemizole, hydroxyzine,

Interaction of ephedrine and theophylline.

The effectiveness and safety of individualized theophylline doses administered as such and in combination with ephedrine and hydroxyzine was investigated in 23 children with chronic asthma who received these medications and a placebo for 1 week each, double-blind and in randomized sequence. Theophylline, determined by prior clinical titration in each patient, averaged 7.3 mg/kg per dose administered every 6 hr, with peak serum theophylline concentrations averaging about 16 mug/ml. Asthmatic symptoms were controlled with theophylline. Ephedrine, alone or in combination, was relatively ineffectu

A Comparative, Three-Arm, Randomized Clinical Trial to Evaluate the Effectiveness and Tolerability of Bilastine vs Fexofenadine vs Levocetirizine at the Standard Dose and Bilastine vs Fexofenadine at Higher Than the Standard Dose (Up-Dosing) vs Levocetirizine and Hydroxyzine (in Combination) in Patients with Chronic Spontaneous Urticaria.

INTRODUCTION: Though second-generation antihistamines (SGAH) are first-line drugs in chronic spontaneous urticaria (CSU), 50% of patients do not respond to them. In such patients, guidelines recommend either up-dosing of SGAH or combination of different antihistamines. However, the studies comparing these treatment regimens are limited. METHODS: In this comparative, three-arm study, CSU patients were randomized to receive standard dose of either bilastine, fexofenadine, or levocetirizine for 2 weeks. After 2 weeks of treatment, non-responders received double dose of either bilastine or fexofe

The effects of some centrally acting drugs on ganglionic transmission in the cat.

Transmission through the cat superior cervical ganglion was studied by recording the response of the nictitating membrane to both pre- and postganglionic cervical sympathetic nerve stimulation. The intra-arterial injection of central depressant drugs to the ganglion through the lingual artery depressed transmission through the ganglion. The central depressant drugs tested were (in decreasing order of activity): amylobarbitone, pentobarbitone, carbromal, benactyzine, mephobarbitone, hydroxyzine, phenobarbitone, azacyclonol, methylpentynol carbamate, paraldehyde, phenytoin, mephenesin, chlorbuto

Domestic Abuse of Codeine: A Case Study of Non-Medical Use Leading to Fatal Outcome.

Codeine, an opioid analgesic present in many over-the-counter (OTC) formulations, is frequently misused through non-medical extraction techniques such as cold water extraction (CWE). These practices carry substantial risks, including incomplete removal of hepatotoxic co-formulants, contamination, and highly unpredictable dosing. We report a fatal case of a 29-year-old man who ingested codeine extracted from Antidol® tablets in combination with energy drinks and psychotropic medications. Post-mortem LC-MS/MS analysis revealed the presence of codeine (0.66 µg/mL), morphine (0.02 µg/mL), hydroxyz

Chronic Dermatographic Urticaria Secondary to Systemic Lupus Erythematosus.

Systemic lupus erythematosus (SLE) poses significant challenges in diagnosis and management due to its diverse clinical manifestations, including various skin abnormalities. Chronic dermatographic urticaria, although less recognized, has been suggested to have an association with SLE; however, evidence supporting this connection remains limited. We present a case of chronic dermatographic urticaria secondary to SLE in a 26-year-old female. Despite ineffective conventional treatments, the initiation of hydroxyzine resulted in notable symptom improvement without adverse effects. This case underl
