[Nerve injuries in bicycle riders].

2 cases of damage to peripheral nerves following bicycle riding are described in males aged 13 and 35 years, respectively. In 1 there was injury to the ulnar nerve at the level of the wrist, with hypesthesia of the 2 ulnar digits and paresis of intrinsic muscles. In the other case there was involvement of the pudendal nerves with numbness of the buttocks and genitalia, and with difficulty in achieving erection. After cycling was stopped for a few weeks the symptoms disappeared spontaneously, with full recovery. Once symptoms subside completely cycling can be resumed, although with certain prec

[Angiomatous malformation of the inner auditory canal with the leading symptoms of vertigo and tinnitus].

A case of a rare arteriovenous malformation in the internal auditory canal is reported. It caused unilateral tinnitus, facial weakness, trigeminal hypesthesia, and vertigo with lateropulsion. The audiological and otoneurological findings together with air-cisternography a CT scan had indicated an intrameatal tumor. An extended trans-temporal exposure of the internal auditory canal demonstrated an angiomatous lesion compressing the adjacent seventh and eighth cranial nerves. It could be removed safely by a second-stage lateral suboccipital approach to the cerebello-pontine angle.

Optic neuropathy and paratrigeminal syndrome due to Aspergillus fumigatus.

Chronic Aspergillus meningitis and cerebral vasculitis occurred in a 67-year-old man. He experienced periorbital pain that increased in severity during a ten-month period. Although no focal neurologic deficits were initially present, oculosympathetic paresis, corneal hypesthesia, and optic neuropathy developed. This is the first report, to our knowledge, of paratrigeminal syndrome with optic neuropathy due to aspergillosis. The case was also unusual because it was chronic and there was no extracerebral infection or predisposing factors, such as underlying malignancy or collagen vascular diseas

Outcomes of CyberKnife Radiosurgery for Trigeminal Neuralgia in Patients with Multiple Sclerosis.

BACKGROUND: The outcomes of radiosurgery for trigeminal neuralgia (TN) in patients with multiple sclerosis (MS) are not as extensively assessed as those for idiopathic or classical TN cases. OBJECTIVE: Evaluate the safety and efficacy of radiosurgery for TN in MS patients and identify potential predictors of successful outcomes. METHODS: A retrospective single-institution cohort study with patients treated between 2009 and 2022 was performed. Fifty patients were included, and a total of 68 radiosurgical interventions were delivered. Outcomes included the maintenance of pain relief assessed u

Numb Chin Syndrome as the Initial Presentation of Mandibular Metastasis of Colorectal Cancer: A Case Report.

Numb chin syndrome (NCS) is hypesthesia of the mandible and lower lip caused by damage to the inferior alveolar or mandibular nerves, commonly due to dental treatment or osteomyelitis, but occasionally caused by malignant tumors. We report the case of a male in his 60s. He came to our hospital with a chief complaint of mandibular pain and paresthesia in the right side of the mental region. He had noticed swelling of the left mandible one month before the initial visit and strong hypesthesia of the right side of the mental region one week before the initial visit. Panoramic radiographs showed s

Active Heating following Cryolipolysis Reduces Efficacy and Side Effects: A Randomized Split-Body Trial.

BACKGROUND: Cryolipolysis-a popular noninvasive technique for body contouring-has fewer side effects compared with liposuction; however, its effectiveness in terms of reducing local adipose tissue is also lower. This study is, to the authors' knowledge, the first prospective, controlled, investigator-blinded split-body trial to evaluate whether postcryolipolytic heating can increase the efficacy. METHODS: Twenty-five subjects were treated with one session of cryolipolysis on the lower abdomen and a subsequent heating with a mud pack of a randomized side of the treated region (left or right).

Cyberknife radiosurgery for trigeminal neuralgia treatment: a preliminary multicenter experience.

OBJECTIVE: Radiosurgery has gained acceptance as a treatment option for trigeminal neuralgia. We report our preliminary multicenter experience treating trigeminal neuralgia with the CyberKnife (Accuray, Inc., Sunnyvale, CA). METHODS: A total of 95 patients were treated for idiopathic trigeminal neuralgia between May 2002 and October 2005. Radiosurgical dose and volume parameters were retrospectively analyzed in relation to pain response, complications, and recurrence of symptoms. Optimal treatment parameters were identified for patients who had excellent and sustained pain relief with no comp

Does egress of cerebrospinal fluid during percutaneous retrogasserian glycerol rhizotomy influence long term pain relief?

BACKGROUND AND OBJECTIVES: To examine the effect of cerebrospinal fluid (CSF) flow during percutaneous retrogasserian glycerol rhizotomy (PRGR) on long term pain relief in patients with trigeminal neuralgia. METHODS: Eighty-nine patients with trigeminal neuralgia underwent 102 PRGR procedures. PRGR was conducted under fluoroscopy. After the egress of CSF, anhydrous glycerol (0.3-0.4 cc) was injected in the sitting position. In the absence of CSF flow, 0.25 mL 2% lidocaine was injected to elicit hypesthesia in the affected side. Once hypesthesia was elicited glycerol was injected. Patients wer

Pulsed Radiofrequency: A Management Option for Recurrent Trigeminal Neuralgia Following Radiofrequency Thermocoagulation.

BACKGROUND: Pain relief comparable with radiofrequency thermocoagulation (RFT) alone and fewer side effects than RFT have been achieved by combination treatment with pulsed radiofrequency (PRF) and short-duration RFT in trigeminal neuralgia (TN). CASE DESCRIPTION: We report the successful management of recurrent TN after RFT with single PRF in 2 patients. The RFT treatment was performed in 2-3 cycles for each division, with the lesion setting at 75°C-80°C for 90 seconds. The PRF treatment was applied for 120 seconds, with a generator output of 45 V, not exceeding a temperature of 42°C at the

Orbital floor reconstruction with resorbable polydioxanone implants.

Many different materials are proposed for reconstruction of traumatic orbital floor defects. Donor-site morbidity of autologous transplants and infections or extrusions of nonresorbable implants lead to a widespread use of resorbable, alloplastic materials such as polydioxanone (PDS). The goal of this study was to evaluate the prevalence of orbital floor fracture-related problems after surgical treatment using PDS. Ophthalmologic and clinical examinations were performed at 194 patients before orbital floor reconstruction, 14 days and 6 months after surgery (approximate defect sizes: <1 cm², n=

Arnold-Chiari Malformation Type I Presenting With Bilateral Scapular Winging.

Bilateral scapular winging is a rare and atypical manifestation of Arnold-Chiari malformation type 1 (ACM1). We report a case of ACM with extensive syrinx formation, presenting with progressive bilateral proximal upper extremity weakness, bilateral scapular winging, and segmental hypesthesia and reduced thermesthesia over the bilateral C2-C8 dermatomal levels. The patient was successfully treated with surgical decompression and syringosubarachnoid shunting. This unusual presentation of ACM1 emphasizes the need to consider central nervous system pathologies in the differential diagnosis for bil

L1 radiculopathy mimicking meralgia paresthetica: a case report.

L1 radiculopathy is very rare and difficult to diagnose with needle electromyography. A patient presented with pain and hypesthesia on the anterolateral aspect of the left thigh. Nerve conduction studies and needle electromyography were normal, except for the quadratus lumborum and iliopsoas muscles, which showed abnormal spontaneous activity and polyphasic motor unit potentials with reduced recruitment patterns. Magnetic resonance imaging of the lumbar spine showed disc extrusion of the L1-2 intervertebral space with upward migration. This case demonstrates the usefulness of examination of th

[Microvascular decompression of trigeminal neuralgia caused by vascular compression which penetrated the trigeminal nerve: a case report].

A rare case of trigeminal neuralgia, caused by a branch of the superior cerebral artery running through the trigeminal nerve, was successfully treated by microvascular decompression. The nerve gap at the arterial penetration point was widened by the dissection of the epineurium to obtain sufficient mobilization of the artery and decompression of the nerve. This technique would be useful for securing neurovascular decompression, as well as for avoiding postoperative hypesthesia owing to the neurotomy described in previous reports. Preparing for such a rare case is important as preoperative magn

Transorbital-transpetrosal penetrating cerebellar injury--case report.

A 4-year-old boy presented with a transorbital-transpetrosal penetrating head injury after a butter knife had penetrated the left orbit. The knife tip reached the posterior fossa after penetrating the petrous bone. Wide craniotomy and the pterional, subtemporal, and lateral suboccipital approaches were performed for safe removal of the object. The patient was discharged with left-sided blindness, complete left ophthalmoplegia, and hypesthesia of the left face. Early angiography is recommended to identify vascular injury which could result in fatal intracranial hemorrhage.

Cervical spondylolisthesis associated with the multiple nevoid basal cell carcinoma syndrome.

Cervical spondylolisthesis is a rate vertebral anomaly that has not been associated with other major congenital abnormalities. In a 16-year-old girl, lower cervical spondylolisthesis was associated with a multiple nevoid basal cell carcinoma syndrome. The salient features of cervical spondylolisthesis include occipital headache, nuchal rigidity, torticollis, painful range of limited motion, hypesthesia, and depressed deep tendon reflexes. The treatment in the patient was anterior vertebral body fusion. Awareness of this associated problem and prompt treatment are essential for prevention of ne

Local anesthetic effect of intrathecal normal saline.

The effects of intrathecally administered normal saline have been studied in patients undergoing graduated spinal anesthesia. The injection of 5 or 10 ml volumes resulted in segmental hypesthesia to pin-scratch and cold stimuli extending from the lower thoracic to the sacral dermatomes. There was also partial sympathetic block evidenced by a diminished psychogalvanic skin reflex. The sites and possible mechanisms of action of saline are discussed; its use as placebo in graduated spinal anesthesia is questioned. It is concluded that normal saline acts as a weak local anesthetic when injected in
