Uveitis-Glaucoma-Hyphema Syndrome From Subtle Malposition of a Hydrus Microstent.

Uveitis-glaucoma-hyphema (UGH) syndrome is a rare condition characterized by intraocular inflammation, elevated intraocular pressure, and recurrent hyphema. Although UGH syndrome is typically caused by chafing between an intraocular lens and the iris or ciliary body, microinvasive glaucoma devices are becoming increasingly recognized as potential causes of recurrent intraocular inflammation. Herein, we discuss a case of UGH syndrome due to the subtle malposition of a Hydrus microstent that ultimately required surgical removal to manage. This represents the first case of Hydrus-related UGH synd

A Case of Uveitis-Glaucoma-Hyphema Syndrome Related to a Hydrus Microstent.

Uveitis-Glaucoma-Hyphema (UGH) syndrome is characterized by episodes of anterior chamber inflammation, elevated intraocular pressure and hyphema. It is caused by a mechanical chafing of the iris or ciliary body typically by a malpositioned intraocular lens. We present a rare case of UGH syndrome related to the insertion of a Hydrus Microstent. Because of the increased number of microinvasive glaucoma surgeries being performed and a reduction of UGH syndrome patients related to the insertion of intraocular lenses, it is an important diagnosis to consider in patients with recurrent postoperative

Clinical evaluation of aminocaproic acid for managing traumatic hyphema in children.

PURPOSE: The purpose of this study is to determine the incidence of secondary hemorrhage after traumatic hyphema in children and to evaluate the efficacy of epsilon aminocaproic acid in reducing this incidence. METHODS: In a prospective, randomized, double-blind study performed between November 1987 and February 1994, 94 children admitted for traumatic hyphema were assigned to receive either aminocaproic acid (n = 48) (100 mg/kg every 4 hours; maximum, 30 g daily) or placebo (n = 46) for 5 days. Patients who had ingested aspirin in the week preceding admission were excluded from the study. R

Oral administration of tranexamic acid in the management of traumatic hyphema in children.

In a prospective study 163 patients aged 17 years or less admitted to a children's hospital between April 1985 and December 1990 with traumatic hyphema were treated with tranexamic acid, 25 mg/kg given orally every 8 hours to a maximum of 1500 mg every 8 hours for 5 days. Secondary hemorrhage occurred in 5 patients (3%), none of whom had more than one rebleeding episode. In contrast, 24 (8%) of 316 patients aged 17 years or less admitted to the same hospital between January 1977 and March 1985 with traumatic hyphema who were not treated with antifibrinolytics had a secondary hemorrhage, severa

Sickle cell hyphema with secondary glaucoma in a non-black patient.

A 5-year-old Mexican-American boy developed marked elevation of intraocular pressure in association with a small traumatic hyphema. Laboratory studies prompted by this clinical presentation revealed that the child had sickle trait. Pressure returned to normal after surgical evacuation of blood from the anterior chamber. Sickle cell hemoglobinopathy should be considered in the differential diagnosis of secondary glaucoma following blunt ocular trauma even in non-black patients.

Bimanual bipolar diathermy for recurrent hyphema after anterior segment intraocular surgery.

Phacoemulsification with mechanical pupil dilation was performed on a functionally monocular glaucoma patient with pseudoexfoliation syndrome. The postoperative course was complicated by persistent intraocular hemorrhaging from the pupil margin in multiple locations that ceased temporarily with a marked elevation in intraocular pressure (IOP). Normalization of IOP with medication or paracentesis resulted in recurrent bleeding and a subsequent increase in IOP elevation. Surgical intervention using bipolar diathermy was required to control the bleeding and the elevated IOP. A bimanual approach a

Factors associated with the poor final visual outcome after traumatic hyphema.

In order to determine the factors related to the worse final visual outcome following nonperforating traumatic hyphema, the clinical characteristics of 18 patients with visual outcome of 0.1 or worse were compared with those of 166 patients with visual outcome of 0.15 or better. The presence of posterior segment injuries such as macula edema, retinal hemorrhage, epiretinal membrane, and choroidal rupture were significant factors of a poor final visual outcome (P < 0.01). The presence of anterior segment injuries such as corneal blood staining, traumatic mydriasis, iridodialysis, cataract, and

Two cases of spontaneous hyphema after posterior chamber phakic intraocular lens implantation.

The implantable collamer lens (ICL, STAAR Surgical Co.) surgery is popular for its effectiveness and safety, but it carries potential risks. This study presents two unreported cases of hyphema after ICL surgery, highlighting the need for prompt investigation and management of both hyphema and IOP to prevent ocular damage.

A case of delayed recurrent hyphema following Implantable Collamer Lens (ICL) surgery.

PURPOSE: To present a case of delayed recurrent hyphema following toric ICL implantation. OBSERVATIONS: This case reports a 24-year-old Asian female who presented with sudden decrease in vision in the right eye, accompanied by recurrent massive anterior chamber hemorrhage, six months after bilateral implantation of toric ICL with central holes for myopia correction. Despite initial conservative treatment with immobilization and intraocular pressure (IOP)-lowering medication at another hospital, the hyphema persisted. At our hospital, her corrected visual acuity (CDVA) in the right eye was cou

Navigating postoperative complications: Uveitis-glaucoma-hyphema syndrome after Ahmed glaucoma valve implantation.

Altwijri and Alsirhy reported a case of uveitis-glaucoma-hyphema syndrome after an Ahmed glaucoma valve implantation surgery in an advanced primary open-angle glaucoma patient, being the first ever recorded of its kind. The author describes the position of the tube as the origin of the anterior chamber inflammation and hyphema, which resolved shortly after shortening and relocating it. This publication emphasizes the importance of precise implant positioning and close-up patient follow-up after glaucoma filtration surgery as an important standard for healthcare providers.

Silent sicklers and traumatic hyphema: the importance of maintaining a high index of suspicion.

A previously healthy 6-year-old white boy presented emergently after trauma to his right eye. On examination, intraocular pressure (IOP) was 49 mm Hg, and visual acuity was 20/50. He was diagnosed with a 3 mm layering hyphema. Despite medical therapy and lifestyle modifications, the patient's IOP continued to rise, peaking at 77 mm Hg on day 3 after presentation. Given his elevated IOP despite the addition of new topical medications on daily follow-up, the patient was screened for sickle-cell disease to assess for an underlying pathology that could be exacerbating his elevated IOP. After testi

Transcorneal oxygen therapy for persistent hyphema in a patient with sickle cell disease.

UNLABELLED: Clinical case The case concerns a 10-year-old boy of African origin, who suffered a mild ocular trauma to the left eye. Upon examination, the best visual acuity was 0.2 using the Snellen scale, with a 1mm height hyphema, intraocular pressure (IOP) of 12mmHg on left eye, with an increase up to 20mmHg within 72h. With a positive test for sickle cell disease, it was decided to treat medically with transcorneal oxygen therapy. Clearing of the anterior chamber was achieved, with and improvement in the best visual acuity to 0.8, and lowering of IOP to 8mmHg. DISCUSSION: In the context o
