Seven cases of fistula developing between aortic or iliac aneurysms and adjacent major venous trunks are presented. The clinical diagnosis and principles of management of this exceptionally rare complication are discussed in the light of the literature on the subject.

This is a report on a patient of 64 years of age who was admitted to hospital as an inpatient thirteen years after obliteration of the iliac artery with an extension plasty. He complained of irregular stools and occasional abdominal pain. Imaging methods were employed on account of a drop in the haemoglobin count, progressive renal insufficiency and a palpable tumor in the lower abdomen. These methods supplied evidence of a false aneurysm originating from the iliac endarterectomy, combined with compression of the ureter and small intestine. Based on the present case report, the general complex

Anastomotic false aneurysm (AFA) of the aorta or iliac artery is a potentially lethal complication of prosthetic grafts. To study this complication, the records of 18 patients with 22 noninfected AFAs (15 aortic and seven iliac) were reviewed. Patients with an intact AFA had a pulsatile abdominal mass, abdominal pain, an occluded graft, peripheral emboli, or a femoral anastomotic false aneurysm. All patients with a ruptured AFA were in shock, but 67% (four of six) had symptoms before hemorrhage. For diagnosis, single-plane angiography was 69% accurate (11 of 16), computed tomography was 100% a

We report a case of an 86-year-old asymptomatic patient, who underwent a repair of the infrarenal abdominal aortic aneurysm 13 years ago. He … sm. Although coiling additive to other procedures is applied frequently, only few cases of internal iliac aneurysm were treated with coil embolisation alone. During a first outpatient visit 2 months following the procedure the aneurysm was still completely thrombosed.

This is a report of a patient presenting with a contained rupture of an internal iliac aneurysm following proximal ligation after abdominal aortic aneurysm repair three years earlier. The patient presented with a large pelvic mass with symptoms of urgency, frequency, dysuria, tenesmus and fevers associated with anemia. Following evacuation of the aneurysm and direct suture ligation of the distal branches of the internal iliac artery, the patient's aortic graft was covered with omentum which also filled the pelvic cavity. The importance of proximal and distal control of aneurysms and/or the imp…

Isolated internal iliac artery aneurysm (IIIAA) is a rare condition, with limited information available about its symptoms and surgical treatment outcomes, particularly in Ghana. This case report presents the 4-year surgical treatment outcomes of three patients (aged 42, 58, and 22 years) with IIIAA, each exhibiting different clinical presentations. All patients experienced pelvic pain and difficulty walking but had uneventful post-operative recoveries. They made significant progress with physiotherapy and were able to mobilise with a Zimmer frame before discharge. Currently, all three patient

BACKGROUND: Internal iliac artery aneurysms (IIAA) are uncommon. Open repair is technically challenging and has been associated with increas … an age of 74 */- 9 years. Six patients had isolated IIAAs (26%); the remaining 17 patients had aortoiliac aneurysms. Five patients (22%) required emergent repair for ruptured IIAAs. OBJECTIVES: There is no standardised technique for internal iliac artery aneurysm (IIAA) embolisation and results of long-term prevention of rupture are unknown.
DESIGN: We retrospectively evaluated technical aspects and results of IIAA embolisation in a multicentre study.
METHODS: Aneurysm morphology and embolisation techniques were reviewed. Aneurysm-related death, rupture, diameter increase, endoleak, secondary procedure and complication related to the IIA occlusion were recorded.
RESULTS: Between 2001 and 2011, 53 patients with 57 IIAA were treated. Mean diameter of IIAA was 41 mm (rang

Ruptured false iliac aneurysms carry high mortality of up to 57%. It should be suspected whenever a patient who had previous aortic aneurysm repaired presents with bizarre lower abdominal symptoms. In the literature, different presentations have been reported, e.g progressive hydronephrosis, small intestinal obstruction, chronic venous insufficiency, arterio-venous fistula, and gross haematuria from arterio-vesical fistula. We report a patient suffering from a ruptured iliac aneurysm associated with a new sign that has not been described hitherto: perineal bruising.

We report a very rare case of spontaneous ilioappendicial fistula with right common iliac aneurysm. After the aneurysm was opened, afferent and efferent vessels were closed following extraanatomical femorofemoral bypass, and the appendectomy was performed. The wall of the aneurysm showed the atherosclerotic change and histologic study of the appendix confirmed the diagnosis of acute appendicitis. Enhanced computed tomography was useful for the diagnosis and the extraanatomical bypass was deemed the most effective operative strategy. The pathogenesis of the fistula was surmised to be related to…

A case of bilateral iliac artery aneurysms in a 24 year old patient with a Behçet-like disease is presented. Behçet's disease is characterized by multiple organ involvement. Primary manifestations are genital and oral mucosal ulcerations and eye and skin affections. Among secondary manifestations are non-specific arteritis causing thrombosis as well as aneurysmal disease of the medium-sized and large arteries. Behçet's disease should be considered in young patients with arterial aneurysms.

The authors report a case of a perianeurysmal retroperitoneal fibrosis (RPF) with oligoanuric renal failure. A complete assessment was performed with computed tomography. Bilateral ureteral catheterization restored normal renal function and allowed surgical treatment to be performed including an aorto-iliac replacement and a bilateral ureterolysis with intra-peritoneal diversion of the ureter. A review of the literature revealed 15 similar cases. Pathophysiology and treatment of the retroperitoneal peri-aneurysmal fibrosis are discussed.

A 73-year-old female with severe sciatica suffered from an aneurysm of the left internal iliac artery. At first minimal caudographic abnormalities suggested intervertebral disc herniation and lumbar root compression but at exploratory surgery the diagnosis had to be rejected. Once the correct diagnosis was established by echography, CT-scanning and angiography surgical treatment of the aneurysm resulted in complete recovery. Because sciatic nerve lesions due to aneurysms in the pelvic region very seldom occur and may be the cause of diagnostic confusion, symptoms and treatment of these aneurys

During a 4-year-period, 5 cases of ureteral obstruction by aorto-iliac aneurysm were detected. Over the same period 50 aorto-iliac aneurysms were resected. One of the 5 patients was not operated on. In the remaining 4, the aneurysm was resected and replaced by a Dacron bifurcation graft. Nephrectomy was performed in the first 2 patients. Autotransplantation of the remaining kidney was carried out in one case following enucleation of a hypernephroma ex vivo. In the other 2 patients, ureterolysis and intraperitoneal transposition of the ureter was performed in conjunction with resection of the a…

A 78-year-old male had undergone endovascular aortic aneurysm repair (EVAR) 7 years prior to presentation. Although the sac was stable 6 months ago, the patient presented with shock at arrival, and CT showed aortic rupture with rapid expansion due to type Ib endoleak caused by iliac neck dilatation (IND). The aneurysm sac was excluded using an endovascular strategy. Bell-bottom iliac limbs can cause IND associated with type Ib endoleak. Additionally, the risk of rupture is high when re-expansion of an aneurysm occurs after sac regression after EVAR. Therefore, close follow-up is mandatory for
