Restoration of natural somatic sensations to the amputees: finding the right combination of neurostimulation methods.

UNLABELLED: Limb amputation results in such devastating consequences as loss of motor and sensory functions and phantom limb pain (PLP). Neurostimulation-based approaches have been developed to treat this condition, which provide artificial somatosensory feedback such as peripheral nerve stimulation (PNS), spinal cord stimulation (SCS), and transcutaneous electrical nerve stimulation (TENS). Yet, the effectiveness of different neurostimulation methods has been rarely tested in the same participants. Meanwhile, such tests would help to select the most effective method or a combination of method

From subthalamic local field potentials to the selection of chronic deep brain stimulation contacts in Parkinson's disease - A systematic review.

BACKGROUND: Programming deep brain stimulation (DBS) of the subthalamic nucleus for optimal symptom control in Parkinson's Disease (PD) requires time and trained personnel. Novel implantable neurostimulators allow local field potentials (LFP) recording, which could be used to identify the optimal (chronic) stimulation contact. However, literature is inconclusive on which LFP features and prediction techniques are most effective. OBJECTIVE: To evaluate the performance of different LFP-based physiomarkers for predicting the optimal (chronic) stimulation contacts. METHODS: A literature search w

A most unusual pacemaker.

We present a case of pacemaker artifact on 12-lead electrocardiogram that was initially interpreted as malfunction of the patient's dual-chamber cardiac pacemaker. It was ultimately determined to arise from an extracardiac gastric pacemaker, which had been inserted for refractory, severe diabetic gastroparesis. Proper functioning of both devices was confirmed, and settings were adjusted to test for interdevice interference. This is a report of a patient with both cardiac pacemaker and a gastric pacemaker. We review the literature on patients with multiple pacemaker devices, and conclude with s

Spatial and Amplitude Dynamics of Neurostimulation: Insights from the Acute Intrahippocampal Kainate Seizure Mouse Model.

OBJECTIVE: Neurostimulation is an emerging treatment for patients with medically refractory epilepsy, which is used to suppress, prevent, and terminate seizure activity. Unfortunately, after implantation and despite best clinical practice, most patients continue to have persistent seizures even after years of empirical optimization. The objective of this study is to determine optimal spatial and amplitude properties of neurostimulation in inhibiting epileptiform activity in an acute hippocampal seizure model. METHODS: We performed high-throughput testing of high-frequency focal brain stimulat

Cognitive benefits of using non-invasive compared to implantable neural feedback.

A non-optimal prosthesis integration into an amputee's body schema suggests some important functional and health consequences after lower limb amputation. These include low perception of a prosthesis as a part of the body, experiencing it as heavier than the natural limb, and cognitively exhausting use for users. Invasive approaches, exploiting the surgical implantation of electrodes in residual nerves, improved prosthesis integration by restoring natural and somatotopic sensory feedback in transfemoral amputees. A non-invasive alternative that avoids surgery would reduce costs and shorten cer

Theoretical Basis of Vagus Nerve Stimulation.

Despite a myriad of medical and surgical treatments for epilepsy developed over the past few decades, a large subset of patients remains refractory to treatment. Over this time period, vagus nerve stimulation (VNS) has become an accepted and viable treatment modality for this population. Since the earliest report of VNS implantation in 1988, tens of thousands of patients worldwide have received VNS therapy, and >100,000 patient-years of experience have been accrued. The mechanisms underlying the response to VNS therapy continue to be elucidated. As understanding of the VNS mechanisms of action

Peripheral nerve/field stimulation for chronic pain.

Peripheral nerve stimulation and peripheral nerve field stimulation involve the delivery of electrical stimulation using implanted electrodes either over a target nerve or over the painful area with the goal of modulating neuropathic pain. The selection of appropriate candidates for this therapy hinges on skillful application of inclusion and exclusion criteria, psychological screening, and an invasive screening trial. Patients with significant improvement in pain severity and pain-related disability during the trial are considered candidates for implantation of a permanent system. As with oth

Sphenopalatine ganglion neuromodulation in migraine: what is the rationale?

OBJECTIVE: The objective of this article is to review the prospect of treating migraine with sphenopalatine ganglion (SPG) neurostimulation. BACKGROUND: Fuelled by preliminary studies showing a beneficial effect in cluster headache patients, the potential of treating migraine with neurostimulation has gained increasing interest within recent years, as current treatment strategies often fail to provide adequate relief from this debilitating headache. Common migraine symptoms include lacrimation, nasal congestion, and conjunctival injection, all parasympathetic manifestations. In addition, stud

Diaphragm pacing in amyotrophic lateral sclerosis: a literature review.

Amyotrophic lateral sclerosis (ALS) remains a rapidly progressive fatal degenerative disease of motor neurons for which there are few interventions to slow disease progression or improve quality of life. A diaphragm pacing system was approved by the U.S. Food and Drug Administration in September 2011 for ALS under a Humanitarian Device Exemption. News of this approval has been met with a combination of excitement and uncertainty by members of the ALS community. We review the currently available data on the diaphragm pacing system and its use in ALS. Diaphragm pacing appears to be reasonably sa

Neurostimulation therapy for epilepsy.

Neurostimulation therapy for epilepsy is growing in popularity. By appropriate targeting of applied electrical activation at selected nervous system sites, antiseizure effects may be achieved without the common sedative side effects of antiepileptic medications. Risks of neurostimulation therapy are those associated with the device implantation surgical procedures. Vagus nerve stimulation (VNS) reduces seizures by 45% and has been employed in over 13,000 patients worldwide. New reports suggest VNS is particularly beneficial for patients with Lennox-Gastuat syndome. VNS also reduces sudden unex

Epicranial focal cortex stimulation for minimally invasive neuromodulation of the epileptogenic region: A review.

Epicranial focal cortex stimulation (FCS) is a new type of neurostimulation for pharmacoresistant focal epilepsy, which has recently been CE-certified for treatment of European patients. Stimulation is performed via an epicranially placed five-contact electrode array, which applies high frequency stimulation and DC-like cathodal stimulation to the individual epileptogenic brain region. Stimulation at appropriate intensities is not perceived by patients, and first evidence from two prospective unblinded clinical trials suggests excellent tolerability of both, subgaleal implantation and transcra

Restorative Neurostimulation: A Clinical Guide for Therapy Adoption.

In this review, we present a comprehensive clinical approach to restorative neurostimulation, a novel form of stimulation for refractory chronic mechanical low back pain, targeting impaired neuromuscular control and degeneration of the multifidus muscle. We focus on patient identification, technique guidance, and review of the scientific background and clinical evidence. As our understanding of back pain grows, there is clear evidence that impaired neuromuscular control and consequent degeneration of the multifidus muscle contribute to mechanical low back pain development and maintenance. We p

Neuromodulation for the treatment of refractory interstitial cystitis.

Interstitial cystitis (IC) is a symptom complex of urinary urgency, frequency and pelvic pain. Multimodality behavioral and phamacologic treatment is often effective in treating IC. Unfortunately, some patients with IC are refractory to standard treatments. Neuromodulation has been shown to be effective in treating voiding dysfunction. Small studies have demonstrated improvement in pelvic pain and IC symptoms during temporary sacral nerve stimulation. This current study demonstrates that patients refractory to traditional therapies for IC can respond well to sacral nerve stimulation and mainta

An Observational Study of Intraoperative Neuromonitoring as a Safety Mechanism in Placement of Percutaneous Dorsal Root Ganglion Stimulation and Spinal Cord Stimulation Systems.

INTRODUCTION: Percutaneous neurostimulator device placement, specifically dorsal root ganglion (DRG) stimulation and spinal cord stimulation (SCS), involves the placement of thin wires within the spinal canal at specific locations, the DRG or dorsal column of the spinal cord, respectively, to provide an electrical current that modifies the pain signal as it enters the central nervous system from the periphery. Placement of neurostimulator devices is generally safe overall, but not without risk of major and minor complications. In this study, we assess the use of intraoperative neuromonitoring

Primary headache disorders: From pathophysiology to neurostimulation therapies.

Primary headache disorders including migraine, cluster headache, and tension-type headache are among the most common disabling diseases worldwide. The unclear pathogenesis of primary headache disorders has led to high rates of misdiagnosis and limited available treatment options. In this review, we have summarized the pathophysiological factors for a better understanding of primary headache disorders. Advances in functional neuroimaging, genetics, neurophysiology have indicated that cortical hyperexcitability, regional brain dysfunction, central sensitization and neuroplasticity changes play v
