Dynamic patchplasty--a tension-free reconstruction of incisional hernias.

BACKGROUND AND AIMS: An anatomical and physiological reconstruction of the abdominal wall after incisional hernia must include reconstruction of the linea alba to restore the physiological function of the vertebral column and respiration. Against this background, a special reconstruction method employing a mesh, which is performed in our department, is presented. MATERIALS AND METHODS: In the period between 1 January 2000 and 1 June 2004, a total of 106 patients with incisional hernias after median laparotomy of the upper and lower abdomen were operated on in our department using this tension

THE IMPACT OF ANEMIA ON THE DEVELOPMENT OF INCISIONAL HERNIA, PROSPECTIVE STUDY.

OBJECTIVE: An incisional hernia is a common postoperative complication following abdominal surgery, influenced by various contributing factors, including the patient's comorbidities and the surgical technique used. The role of perioperative anemia in the development of incisional hernia remains insufficiently studied. The aim of the study is to determine the role of anemia in the manifestation of incisional hernia. MATERIALS AND METHODS: The study included patients who underwent either elective or emergency abdominal surgery. Postoperative follow-up lasted for 18 months. The patients were cla

A Rare Case of Obstructed Recurrent Incisional Hernia With Incidentalomas.

Incisional hernias (IHs) are the most common postoperative complication of incisions during laparotomy and contribute to a significant burden. The aetiology of IHs varies depending on the surgical technique, patient's condition, and surgeon's experience. Many patients present with abdominal swelling and some degree of discomfort, and in an emergency, the presentation is usually as bowel obstruction or strangulation, necessitating immediate exploration. Hernias can be repaired by closing the defect with a nonabsorbable suture or using mesh. Amidst the use of invasive techniques and mesh, the ra

Intussusception in Incisional Hernia: A Case Report and Literature Review.

Intussusception in adults is a rare condition. Most frequently, intussusception involves the small intestine and, very rarely, the large intestine. In this report, we present the case of a 79-year-old male who was admitted with symptoms and signs of bowel obstruction due to an incarcerated incisional hernia (a tender irreducible incisional hernia associated with nausea and vomiting). His CT scan confirmed intussusception in his incisional hernia, showing the target sign. An emergency laparotomy, small bowel resection, and anastomosis were done. The histopathology report revealed the cause of i

Rare Presentation of Incarcerated Incisional Richter's Hernia of the Cecum.

Richter's hernia, also called a partial enterocele, involves a protrusion of peritoneum with subsequent strangulation or incarceration of only part of the lumen of the anti-mesenteric portion of the small bowel through a fascial defect. We report a rare presentation of incarcerated incisional Richter's hernia of the cecum in a 39-year-old female. The patient presented with acute abdominal pain that gradually improved. Physical examination revealed right lower quadrant tenderness and nodularity just above an abdominoplasty scar. Subsequent computed tomography scan demonstrated a 1 cm by 1 cm hy

Peritoneal flap hernioplasty for reconstruction of transverse incisional hernias.

BACKGROUND: Repair of transverse incisional hernias is a surgical challenge with current methods of abdominal wall reconstruction. The peritoneal flap hernioplasty addresses this problem using flaps of hernial sac to bridge the fascial gap and isolate the mesh from both the intraperitoneal contents and the subcutaneous space exploiting the retro-rectus space medially and the avascular plane between the internal and external oblique muscles laterally. The operative technique and long-term results of 80 consecutive cases with transverse incisional hernias undergoing repair with this method are r

Incarceration of the hepatic lobe in incisional hernia: A case report.

INTRODUCTION: Incisional hernias are abnormal peritoneal outward pouch-like protrusions that develop due to defects that arise as a result of the disruption of the fascia's continuity after abdominal surgery. PRESENTATION OF CASE: A 77-year-old female patient presented to the emergency department of our hospital with complaints of abdominal swelling, abdominal pain, nausea and vomiting. The patient was recommended for surgery. It was decided that the primary fascia closure and onlay patch was the most appropriate approach. DISCUSSION: When the defect in the abdominal wall grows, the function

[Preliminary evaluation of composite materials for the repair of incisional hernias].

Recent investigations have shown that direct exposure of the intestines to currently available absorbable and nonabsorbable biomaterials can result in their adhesion to the bowel. This can then lead to bowel obstruction or migration of the mesh into the intestinal lumen and formation of intestinal fistula. It has been suggested by some authors that an absorbable covering of the peritoneal side of a nonabsorbable mesh would prevent adhesions and biomaterial related intestinal fistula formation. However, the effectiveness of such a combination has not been substantiated by recent experimental an

Robotic repair of Post-Transplant lateral incisional hernias: outcomes in complex hernia management.

INTRODUCTION: Post-transplant incisional hernias (PTIH) are a common complication in transplant patients, arising due to chronic immunosuppression, associated comorbidities, and the type of incisions used, most commonly lateral. PTIH repair is complex, frequently requiring abdominal wall reconstruction techniques, including transversus abdominis release (TAR). The robotic platform offers enhanced anatomical identification and precision, particularly in cases where the dissection plane is limited and complex, improving surgeons’ dexterity. Our objective was to evaluate the management and outcom

Modified SCOLA Repair for Large Incisional Hernia With Rectus Diastasis.

BACKGROUND: Postoperative incisional ventral hernias (POIH) can sometimes be avoided and are challenging to repair. If the underlying rectus diastasis (RD) is left untreated, the attenuated linea alba predisposes to hernia formation and recurrence. Guidelines recommend concomitant repair of hernia and RD using mesh, but data on endoscopic subcutaneous onlay techniques for large or complex POIH defects remain scarce. This study reports long‑term outcomes of a modified subcutaneous onlay endoscopic approach (SCOLA) combined with bilateral rectus fascia release (RFR), triple‑action seroma prevent

Transversus abdominis release in incisional hernia repair: a comprehensive review.

Incisional hernia remains a significant challenge following abdominal surgery, necessitating robust repair strategies to mitigate recurrence and wound complications. While the introduction of synthetic mesh has improved durability, complex defects with significant loss of domain or lateral extension often exceed the limitations of traditional repair techniques. The transversus abdominis release (TAR) has emerged as a cornerstone technique in abdominal wall reconstruction, offering a physiological solution by mobilizing the posterior rectus sheath and creating a wide retromuscular plane for ext

Online information for incisional hernia repair: What are patients reading?

PURPOSE: Patients increasingly access online materials for health-related information. Using validated assessment tools, we aim to assess the quality and readability of online information for patients considering incisional hernia (IH) repair. METHODS: The top three online search engines (Google, Bing, Yahoo) were searched in July 2022 for "Incisional hernia repair" and "Surgical hernia repair". Included websites were classified as academic, hospital-affiliated, commercial, and unspecified. The quality of information was assessed using the Journal of the American Medical Association (JAMA) be

Risk factors for umbilical incisional hernia after laparoscopic colorectal surgery.

BACKGROUND: Laparoscopic colorectal surgery (LCRS) requires a small laparotomy at the umbilicus. The wound is small and inconspicuous, but if the patient develops an umbilical incisional hernia (UIH), the wound is visible and the patient suffers from symptoms of discomfort. However, the incidence of UIH after LCRS and its risk factors are not well understood. The purpose of this study was to investigate the risk factors for UIH after LCRS for colorectal cancer. METHODS: This was a single-centre retrospective study of 135 patients with colorectal cancer, conducted at our hospital from April 20

Biomechanical Influences on Mesh-Related Complications in Incisional Hernia Repair.

Aim: Hernia repair strengthens the abdominal wall with a textile mesh. Recurrence and pain indicate weak bonds between mesh and tissue. It remains a question which biomechanical factors strengthen the mesh-tissue interface, and whether surgeons can enhance the bond between mesh and tissue. Material and Methods: This study assessed the strength of the mesh-tissue interface by dynamic loads. A self-built bench test delivered dynamic impacts. The test simulated coughing. Porcine and bovine tissue were used for the bench test. Tissue quality, mesh adhesiveness, and fixation intensity influenced th

Spontaneous mesh evacuation per rectum after incisional ventral hernia repair.

We present a case of a 58-year-old man who was admitted to our hospital because of abdominal pain. He underwent incisional ventral hernia repair with intraabdominal mesh (ePTFE). On the day of admission, physical examination included the discovery of a foreign body in the rectum. There were no signs of acute abdomen. We induced stool, and the mesh came out with it. His further course was uneventful. Gastrografin series showed persisting fistula between the small intestine and colon, but without extralumination into the peritoneal cavity. The patient was discharged in good health and without si

Laparoscopic incisional hernia repair: our experience with 105 consecutive cases.

Conventional repair of incisional hernia is associated with significant complications and a high recurrence rate (30-50%). The laparoscopic approach offers an effective alternative and reduces the recurrences to less than 5%. The aim of this study was to review our experience with laparoscopic incisional hernia repair. Medical records of all patients who underwent laparoscopic incisional hernia repair from January 2002 to December 2006 were reviewed. Demographic and postoperative data were recorded. The study population consisted of 105 patients, 72 females (68.5%) and 33 males (31.5%); the me

Factors involved in abdominal wall closure and subsequent incisional hernia.

Incisional hernia is a frequent complication of abdominal wall closure with a reported incidence of between 5% and 15% following vertical midline incisions at one-year follow-up. Evidence from randomised clinical trials and meta-analysis indicate that a continuous running non-absorbable or slowly absorbed suture such as polydioxanone is the method of choice for abdominal wall closure. Continuous polydioxanone has a similar incisional hernia rate to its non-absorbable counterparts but causes less chronic pain and wound sinuses. Evidence from randomised clinical trials indicates that a lateral p
