The distribution of income is worse than you think: Including pollution impacts into measures of income inequality.

This paper calculates the distribution of an adjusted measure of income that deducts damages due to exposure to air pollution from reported market income in the United States from 2011 to 2014. The Gini coefficient for this measure of adjusted income is 0.682 in 2011, as compared to 0.482 for market income. By 2014, we estimate that the Gini for adjusted income fell to 0.646, while the market income Gini did not appreciably change. The inclusion of air pollution damage acts like a regressive tax: with air pollution, the bottom 20% of households lose roughly 10% of the share of income, while th

Prevalence of chronic pain in low-income and middle-income countries: a systematic review and meta-analysis.

BACKGROUND: The global burden of chronic pain and disability could be related to unmet surgical needs. This systematic review and meta-analysis aims to characterise existing data regarding the prevalence and associations of chronic pain in low-income and middle-income countries; this is essential to allow better assessment of its relationship to pre-operative and post-operative pain as emergency and essential surgical services are expanded. METHODS: According to PRISMA guidelines, we searched PubMed, PsycInfo, and Cochrane registries for articles published before Dec 31, 2013, using the searc

Advancing oncology education in low-income and middle-income African settings: global insights for local action.

Cancer incidence and mortality are rising globally, with their burden falling disproportionately on low-income and middle-income countries. Africa faces major gaps in oncology education and workforce development, limiting access to high-quality cancer care. Focusing on efforts relevant to oncology in Africa, we provide an overview of existing programmes and models for educating health-care personnel in resource-constrained regions, highlighting challenges and effective strategies. Current efforts described include formal curricula, fellowship programmes, mentorship networks, and technology-dri

Lessons for long-acting lenacapavir: catalysing equitable PrEP access in low-income and middle-income countries.

Despite substantial advances in biomedical HIV prevention, including long-acting injectable pre-exposure prophylaxis (PrEP) options such as cabotegravir, barriers to widespread adoption and scale-up persist in low-income and middle-income countries. Long-acting injectable lenacapavir is a potentially transformative HIV prevention tool, providing an unprecedented opportunity to accelerate progress. However, the global HIV response is under threat like never before, with drastic funding cuts undermining the gains of the past 25 years. The challenges of introducing and scaling up long-acting lena

Mind the gap: rethinking global alcohol metrics in high-abstention low-income and middle-income countries.

Alcohol per capita consumption (APC; total pure alcohol consumed per person 15 years or older per year) is the primary indicator used to track global progress in reducing harms associated with alcohol use. However, in many low-income and middle-income countries (LMICs), where most of the population abstain from alcohol and risk of alcohol-associated harm is concentrated in a heavy-drinking minority, APC can misrepresent both exposure and risk. This Viewpoint argues for the routine inclusion of drinker-adjusted metrics, specifically litres of alcohol consumed per drinker (alcohol per drinker),

Extreme weather events and maternal health in low-income and middle-income countries: a scoping review.

BACKGROUND: Despite global efforts to improve maternal health and healthcare, women throughout the world endure poor health during pregnancy. Extreme weather events (EWE) disrupt infrastructure and access to medical services, however little is known about their impact on the health of women during pregnancy in resource-poor settings. OBJECTIVES: This review aims to examine the current literature on the impact of EWE on maternal health to identify the pathways between EWE and maternal health in low-income and middle-income countries to identify gaps. Objective Relative Income Impacts Life Satisfaction and Positive Affect Only When Subjective Absolute Income is High.

Given the crucial role of money in individuals' daily life and the potential societal impact of income inequality, it is not surprising that the income-subjective well-being (SWB) link has been a topic of great research interest. The present study extends past research by examining the independent and interactive effects of three personal income factors. An experiment was conducted where subjective income (both absolute and relative) were manipulated and objective relative income was measured. Subjective relative income impacted perceived deprivation, negative affect, and state hostility, wher

Scoping review of research on self-managed medication abortion in low-income and middle-income countries.

BACKGROUND: We undertook a scoping review of recent studies on self-managed medical abortion (MA) or abortion where some or all of the process is led independently by the person having the abortion, in low-income and middle-income countries (LMICs) to uncover evidence gaps and help stakeholders leverage existing evidence. METHODS: We searched five bibliographic databases for all articles published on MA between 2007 and July 2020 in LMICs. The search yielded 1294 articles. We identified 107 articles in which one or more of the three WHO-defined subtasks for MA was self-led outside of a clinic

Safety of daily low-dose aspirin use during pregnancy in low-income and middle-income countries.

BACKGROUND: The daily use of low-dose aspirin may be a safe, widely available, and inexpensive intervention for reducing the risk of preterm birth. Data on the potential side effects of low-dose aspirin use during pregnancy in low- and middle-income countries are needed. OBJECTIVE: This study aimed to assess differences in unexpected emergency medical visits and potential maternal side effects from a randomized, double-blind, multicountry, placebo-controlled trial of low-dose aspirin use (81 mg daily, from 6 to 36 weeks' gestation). STUDY DESIGN: This study was a secondary analysis of data f

Conditional economic incentives to improve HIV prevention and treatment in low-income and middle-income countries.

New and innovative approaches are needed to improve the prevention, diagnosis, and treatment of HIV in low-income and middle-income countries. Several trials use conditional economic incentives (CEIs) to improve HIV outcomes. Most CEI interventions use a traditional economic theory approach, although some interventions incorporate behavioural economics, which combines traditional economics with insights from psychology. Incentive interventions that are appropriately implemented can increase HIV testing rates and voluntary male circumcision, and they can improve other HIV prevention and treatme

Timeliness of children's vaccinations in 91 low-income and middle-income countries: an analysis of survey data.

BACKGROUND: For many children in low-income and middle-income countries (LMICs) the actual age of vaccination is later than the recommended target age. We provide updated evidence on the coverage and timeliness of vaccination across a broad and geographically diverse set of LMICs. METHODS: We included 91 LMICs, using data from 45 Demographic and Health Surveys and 46 Multiple Indicator Cluster Surveys conducted between 2010 and 2019. We calculated coverage by day of age for children younger than 3 years for BCG, hepatitis B birth dose vaccine (HBV0), polio vaccine (Polio0-3), diphtheria-tetan

Interventions to support recovery of people with psychosis in low-income and middle-income countries: a systematic review.

This systematic review synthesises the evidence on effectiveness and cost-effectiveness of interventions to support the recovery of people with psychosis in low-income and middle-income countries (LMICs). Prioritised recovery-oriented outcomes were defined as social inclusion, personal recovery, reduction in stigma and discrimination, and protection from human rights violations. Outcomes were mapped to the socioecological model of disability, demonstrating how interventions can have direct and indirect effects at the individual, family, service, and community level simultaneously. Study partic

Avoidable pitfalls on the path to health financing self-reliance in low-income and middle-income countries.

Low-income and middle-income countries (LMICs) are facing an urgent and complex challenge: how to transition to greater self-sustainability in health financing amid declining donor support. While this shift is inevitable, the policy responses it elicits carry significant implications for health system equity and access. This commentary highlights four policy choices increasingly observed in LMICs that we argue are unacceptable in the pursuit of sustainability. These include: (1) shifting the financial burden to out-of-pocket payments; (2) over-reliance on contributory health insurance schemes;

Two sides of the same coin? Absolute income and perceived income inadequacy as social determinants of health.

BACKGROUND: Absolute income is commonly used in studies of health inequalities, however it does not reflect spending patterns, debts, or expectations. These aspects are reflected in measures concerning perceived income inadequacy. While health inequities by absolute income or perceived income inadequacy are well established, few studies have explored the interplay of absolute income and perceived income inadequacy in relation to health. METHODS: Multiple data sources were linked into a nationally representative dataset (n = 445,748) of Dutch adults (18 +). The association between absolute inc

Barriers and facilitators of mental health programmes in primary care in low-income and middle-income countries.

Integration of services into primary health care for people with common mental disorders is considered a key strategy to improve access to mental health care in low-income and middle-income countries, yet services at the primary care level are largely unavailable. We did a systematic review to understand the barriers and facilitators in the implementation of mental health programmes. We searched five databases and included studies published between Jan 1, 1990, and Sept 1, 2017, that used qualitative methods to assess the implementation of programmes for adults with common mental disorders at
