BACKGROUND: Pathological yawning is a compulsive, frequent, repetitive yawning triggered by a specific reason besides fatigue or boredom. It may be related to iatrogenic, neurologic, psychiatric, gastrointestinal, or metabolic disorders. Moreover, it could also be seen in the course of an ischemic stroke.
AIMS: To determine whether pathological yawning is a prognostic marker of middle cerebral artery stroke and evaluate its relationship with the infarct location.
STUDY DESIGN: Cross-sectional study.
METHODS: We examined 161 patients with acute middle cerebral artery stroke, consecutively ad

BACKGROUND: The accessory middle cerebral artery (AMCA) is a middle cerebral artery (MCA) anomaly originating from the anterior cerebral artery. We report our experience of a case in which thrombectomy was undertaken for a patient with hemodynamics that were specific to the AMCA.
CASE PRESENTATION: An 84-year-old man with a history of atrial fibrillation developed paralysis of the left upper and lower extremities. Imaging examinations suggested tandem occlusion of the right internal carotid artery and the origin (M2 segment) of the right MCA. An extremely narrow MCA was visualized. Because th

BACKGROUND: Early neurological deterioration (END) is not a rare phenomenon in single subcortical infarction (SSI; traditionally known as lacunar infarction) patients. Predictors of END in SSI patients are uncertain.
AIMS: We aimed to investigate the association between infarct lesion characteristics, penetrating artery morphology, carrier artery plaque features and END using whole-brain vessel-wall imaging.
METHODS: We prospectively collected data from SSI patients without stenosis of the corresponding carrier artery. The infarct lesion size and location, lenticulostriate artery (LSA) morph

BACKGROUND AND PURPOSE: Malignant middle cerebral artery infarction (MMI) is a severe condition with a high mortality rate. While decompressive hemicraniectomy has been demonstrated to reduce mortality, there is limited knowledge regarding blood pressure (BP) management following the surgery. This study aimed to investigate whether early blood pressure variability after surgery is associated with functional outcomes.
METHODS: This study was a retrospective cohort analysis of patients with MMI who underwent decompressive hemicraniectomy. We calculated BP variability (BPV) by measuring BP hourl

BACKGROUND: Malignant middle cerebral artery (MCA) infarctions result in cerebral edema that can lead to brain herniation and death. Standard management includes decompressive hemicraniectomy (DHC) and comprehensive neurocritical care. Some patients may continue to decline despite these measures. Reopening of the scalp incision may allow for additional decompression and provide mortality benefit.
OBSERVATIONS: A 47-year-old man developed malignant right MCA territory infarction following intravenous thrombolysis and unsuccessful mechanical thrombectomy. Despite aggressive hyperosmolar therapy

OBJECTIVE: This study evaluated the utility of dual-energy computed tomography angiography (DECT) in predicting malignant middle cerebral artery infarction (MMI) in acute ischemic stroke (AIS).
METHODS: A total of 153 AIS patients undergoing DECT within 12 hours of symptom onset and follow-up imaging within 48 hours were included. DECT-derived parameters-virtual non-contrast (VNC) images, virtual monoenergetic (VM) images (40/60 keV), iodine concentration (IC), effective atomic number (Zeff), and spectral Hounsfield unit curve slope (λHU)-were analyzed. Clinical and imaging parameters were co

BACKGROUND: Malignant middle cerebral artery infarction (MMI) is associated with a high incidence of severe disability and mortality. Decompressive hemicraniectomy has become a recognized treatment that can improve the prognosis for patients if performed within a certain time window. Nevertheless, despite this intervention, a mortality rate of approximately 20-40% persists following the surgery. The trends and clinical implications of intracranial pressure (ICP) in these situations remain unclear. We aimed to investigate whether intracranial pressure (ICP) trends are associated with clinical o

CASE: A 70-year-old man underwent anterior cervical diskectomy and fusion (ACDF) of the C3/4 and C4/5 levels by a left-sided approach to address canal stenosis causing right arm weakness. Intraoperative neuromonitoring demonstrated an intermittent decrease in left-sided motor signals. Postoperatively, the patient experienced a right middle cerebral artery stroke. At the 1-year follow-up, right arm strength had improved, but there was persistent left-sided weakness due to stroke.
CONCLUSION: Although rare, possible intraoperative stroke should be considered when there are changes in motor or s

Pituitary apoplexy typically presents with acute headache, vomiting, visual disturbance, and confusion. Herein, we report a rare presentation of ischemic stroke due to pituitary apoplexy. A 16.5-year-old male presented with reduced Glasgow Coma Scale (GCS) score, slurred speech, right-sided hemiparesis, and bitemporal hemianopia. Magnetic resonance imaging of the brain showed a large hemorrhagic sellar/suprasellar mass and an area of cortical T2/FLAIR hyperintensity with corresponding diffusion restriction in the middle cerebral artery territory. Computed tomography (CT) intracranial angiogram

OBJECTIVES: Predictors of malignant middle cerebral artery infarction (mMCAi) in patients after intravenous thrombolysis were well documented, but the risk factors of mMCAi after endovascular thrombectomy (EVT) were not fully explored. Therefore, the present study aimed to investigate the predictors of mMCAi after EVT in stroke patients.
METHODS: This was a secondary analysis of the DIRECT-MT trial. Patients who underwent EVT for the occlusions of MCA and/or intracranial internal carotid artery were analyzed. Primary outcome was the occurrence of mMCAi after EVT. Demographic, clinical, imagin

BACKGROUND: Subacute subdural hematomas (ASDH) are only treated surgically when they cause mass effect significant enough to give symptoms. Rarely, sub-ASDH may cause enough pressure to result in a malignant middle cerebral artery (MCA) territory infarction. Decompressive craniectomy (DC) is the last resort to reduce intracranial pressure following malignant MCA infarction. Herein, we review the literature and describe a case of MCA/posterior cerebral artery (PCA) territories infarction following drainage of a sub-ASDH that was treated with DC with good outcome.

BACKGROUND: Swollen middle cerebral artery infarction is a life-threatening disease and decompressive craniectomy is improving survival significantly. Despite decompressive surgery, however, many patients are not discharged from the hospital alive. We therefore wanted to search for predictors of early in-hospital death after craniectomy in swollen middle cerebral artery infarction.
METHODS: All patients operated with decompressive craniectomy due to swollen middle cerebral artery infarction at the Department of Neurosurgery, Oslo University Hospital Rikshospitalet, Oslo, Norway, between May 1

We aimed to investigate the overall prevalence and possible factors influencing the occurrence of crossed cerebellar diaschisis after acute middle cerebral artery infarction using whole-brain CT perfusion. A total of 156 patients with unilateral hypoperfusion of the middle cerebral artery territory formed the study cohort; 352 patients without hypoperfusion served as controls. We performed blinded reading of different perfusion maps for the presence of crossed cerebellar diaschisis and determined the relative supratentorial and cerebellar perfusion reduction. Moreover, imaging patterns (locati

INTRODUCTION: Functional outcomes following stroke thrombectomy are commonly measured by the modified Rankin Scale (mRS). Due to hemisphere-specific representation of neuropsychological function, the mRS might underestimate persisting functional deficits, particularly in large right-hemispheric strokes. This study aimed to assess infarct volumes and neuropsychological symptoms in patients with left- and right-sided middle cerebral artery (MCA) infarctions treated with thrombectomy, having a favorable outcome at 3 months. We hypothesized that patients with right-sided strokes are affected by la

INTRODUCTION: Endovascular treatment (EVT) performed in the early time window has been shown to decrease the incidence of malignant middle cerebral artery infarction (MMI). However, the incidence of MMI in patients undergoing EVT during the late time window is unclear. This study aimed to investigate the prevalence of MMI in patients undergoing late EVT and compare it with that in patients undergoing early EVT.
METHODS: We retrospectively analyzed consecutive patients with anterior large vessel occlusion stroke who underwent EVT at Xuanwu Hospital between January 2013 and June 2021. Eligible

OBJECTIVE: Intracranial atherosclerotic stroke occurs through various mechanisms, mainly by artery-to-artery embolism (AA) or branch occlusive disease (BOD). This study evaluated the spatial relationship between middle cerebral artery (MCA) plaques and perforating arteries among different MCA territory infarction types using vessel wall magnetic resonance imaging (VW-MRI).
MATERIALS AND METHODS: We retrospectively enrolled patients with acute MCA infarction who underwent VW-MRI. Thirty-four patients were divided into three groups according to infarction pattern: 1) BOD, 2) both BOD and AA (BO

BACKGROUND Development of syndrome of inappropriate antidiuretic hormone secretion or cerebral salt wasting has been commonly noted in post-traumatic brain injury, and this condition may lead to hyponatremia resulting in cerebral edema and possible cerebral herniation. However, the predominant topographic pattern of edema from hyponatremia has not been well documented. Unlike numerous reports on hyponatremia and vasospasm following aneurysmal subarachnoid hemorrhage, the data for traumatic brain injury patient are still limited. We report on a rare patient with malignant middle cerebral artery
