Unique variation in the course of maxillary artery in infratemporal fossa: a case report.

Maxillary artery is one of the key contents of the infratemporal fossa. Mandibular nerve and its branches form a clinically important relation of maxillary artery in this region. A comprehensive knowledge of variations of maxillary artery in the fossa is of special relevance in oral maxillofacial surgeries, management of epistaxis, intractable neuralgias or headaches. We found a unique variation of maxillary artery, presenting bilaterally, in relation to branches of mandibular nerve. During routine dissection in a 55-year-old male cadaver, maxillary artery was seen passing deep to lateral pter

Surgical treatment of eosinophilic granuloma of the infratemporal fossa: a successful treatment option.

The patient was a 16-year-old girl complaining about progressive swelling on the right side of her face, right headache, and right orbital proptosis. A CT scan revealed a lytic mass centered in the greater wing of the right sphenoid bone with extension into the orbit, middle cranial fossa, and infratemporal fossa. The patient underwent a subtemporal-preauricular infratemporal fossa approach, which allowed complete resection of the tumor, with a low morbidity. The pathological diagnosis was eosinophilic granuloma. At 36 months after the initial treatment the patient is free of disease. We concl

Large trigeminal schwannoma of the infratemporal fossa: evaluation of neoangiogenesis in this rare neoplasm.

BACKGROUND: Trigeminal schwannomas are uncommon intracranial tumors. Extracranial trigeminal schwannomas in the infratemporal fossa are rare. METHODS: We present a case with a clinical history of facial pain. MRI and CT scans showed a mass arising from the infratemporal fossa extending into the intracranial space. RESULTS: We performed a combined neurosurgical and maxillofacial approach with preoperative endovascular embolization. Complete removal of the parasellar component was achieved with a minimal extracranial neoplastic residual. High microvessel density, reflecting intense neoangiogen

Zygomatic-transmandibular approach for giant tumors of the infratemporal fossa and parapharyngeal space.

OBJECTIVE: The surgical anatomy of the infratemporal fossa and parapharyngeal space is often not properly understood by neurosurgeons, because these areas are more related to other medical disciplines. This article provides a detailed description of the infratemporal fossa and parapharyngeal space anatomy in cadaveric specimens and offers a neurosurgical perspective on a surgical approach that allows wide exposure and complete resection of giant tumors in this location. METHODS: Ten cadaveric specimens were prepared for anatomic study. Dissections were performed to emphasize the relationship

Visualization of the infratemporal fossa: enhancing anatomical learning through three-dimensional modeling.

The infratemporal fossa and pterygopalatine fossa are critical pathways for blood vessels and nerves leading to the orbit, nasal cavity, and oral cavity. Anatomical observation of these areas is challenging for learners due to their complex connections with surrounding structures and their deep location within the body. Since it is not easy to understand this area in three dimensions with only textbook images, there is a need to produce three-dimensional (3D) content. Most existing 3D data have reconstructed the digital imaging and communication in medicine files from computed tomography image

Transnasal Endoscopy and Combined Approach to Infratemporal Fossa Abscess and Parapharyngeal Abscess.

The infratemporal fossa is a very important anatomical space that is relatively closed with an irregular shape and is adjacent to the parapharyngeal space. Infratemporal fossa abscess is rare clinically. It can occur as a complication of maxillary sinusitis, maxillary sinus fracture, or odontogenic infection. If not handled in time, it may endanger the lives of patients. This paper reports the diagnosis and treatment of infratemporal fossa abscess in 2 diabetic patients. Computed tomography and magnetic resonance imaging are the best methods to diagnose suspected cases of this disease. The key

The Infratemporal Fossa Sign: Pilot Study of a Potential Clue to Eustachian Salpingitis.

Aural discomfort may be the result of obstructive eustachian tube (ET) dysfunction, temporomandibular joint dysfunction, or other causes. The infratemporal fossa (ITF) sign, in which a patient points to a characteristic location below the auricle, is proposed as an indicator of nonobstructive eustachian salpingitis. A preliminary study included patients with a complaint of aural discomfort who were prompted to localize symptoms using a single finger. Group 1 localized by using the ITF sign; group 2 localized deep within the external ear canal (suggesting ET dysfunction); and group 3 localized

Transoral (transvestibular-paramandibular) endoscopic approach for benign tumours in the infratemporal fossa.

We describe a minimally invasive transoral approach to the infratemporal fossa, by means of endoscopy, which facilitates examination and resection of lesions in this area. Furthermore, we outline the technique employed and a case treated with this approach, which permits a rapid access with very low patient morbidity, due to the use of endoscopy and of transoral access, instead of traditional transfacial approaches.

Inflammatory pseudotumour of the infratemporal fossa visualized with 18F-FDG PET/CT.

The inflammatory pseudotumour of the head and neck is a benign lesion, extremely rare outside the cranial orbits. A case is presented of an inflammatory pseudotumour not associated with the IgG4-related disease. The pseudotumour was found as a solitary mass in the infratemporal fossa of a young woman who complained of otalgia and hearing loss. A clear image of the lesion was obtained using an 18F-fluoro-deoxy-glucose (18F-FDG) PET. After the histopathological diagnosis, and treatment with corticosteroids, a second 18F-FDG PET was performed. The metabolic image had returned to normal, and the p

Delayed removal of a maxillary third molar accidentally displaced into the infratemporal fossa.

The removal of an impacted maxillary third molar is an easy procedure for an oral and maxillofacial surgeon. The most commonly seen complications associated with this type of surgery are excessive hemorrhage, infection, pain, swelling, trismus, and root fractures. Although rarely encountered, unexpected complications may also arise during this procedure, such as the displacement of the tooth into an anatomic space. In this article, a case of a maxillary left third molar accidentally displaced into the infratemporal fossa is presented, and the delayed removal of the tooth after 3 weeks from the

Septal dislocation for endoscopic access of the anterolateral maxillary sinus and infratemporal fossa.

BACKGROUND: Transnasal approaches to the anterolateral maxillary sinus and infratemporal fossa are challenging with traditional endoscopic techniques and instrumentation. Additional access in the anterior and lateral direction can be obtained with modified endoscopic medial maxillectomy (MEMM) or total endoscopic medial maxillectomy (TEMM) or via a transseptal approach. Alternatively, we have used a septal dislocation technique to help access these areas. Access to these areas may be necessary for treatment of inverted papilloma, schwannoma, and juvenile nasopharyngeal angiofibromas. The aim o

Hemangiopericytoma of the infratemporal fossa: progression toward malignancy in a 30-year history.

Hemangiopericytoma is a rare vascular tumor first described by Stout and Murray in 1942 and characterized by a proliferation of Zimmermann's pericytes, smooth muscle cells arranged around blood vessels. This tumor presents as a slowly enlarging painless mass. Diagnosis with certainty is often a difficult one because of the close likeness with other spindle cell tumors; it requires the help of immunohistochemical techniques and sometimes ultrastructural techniques. Only 15% of hemangiopericytomas are localized in the cervicofacial region; in particular, occurrence in the infratemporal fossa is

Infratemporal fossa approach to the hypoglossal canal: practical landmarks for elusive anatomy.

OBJECTIVE: At the conclusion of the article the readers should be able to safely and reliably find the hypoglossal canal using the infratemporal fossa approach. HYPOTHESIS: Very little has been written on the regional anatomy of the hypoglossal canal as seen through a transtemporal approach. This project attempts to further define the anatomy of the hypoglossal canal and provide the surgeon with guidelines for reaching it. Our hypothesis is that the hypoglossal canal can be safely and consistently reached by way of the temporal bone with preservation of hearing and cranial nerves (CN) IX to X

Infratemporal fossa and maxillary sinus metastases from colorectal cancer: a case report.

Head and neck metastases from colorectal carcinoma are very rare. We report on a 47-year-old woman who had a vegetating tumor located at the distal sigmoid colon with initial liver metastases. She underwent palliative transverse colostomy to prevent intestinal obstruction. She was then treated with 5-fluorouracil, leucovorin and CPT-11. At the third cycle she presented with toothache and numbness of her upper lip. Computed tomography (CT) of the head and neck showed a gross mass involving the sphenoid sinus, left maxillary sinus, left pterygopalatine plate, pterygopalatine and infratemporal fo

Surgical strategy for tumors located in or extending from the intracranial space to the infratemporal fossa-Advantages of the transcranial approach (zygomatic infratemporal fossa approach) and the indications for a combined transcranial and transcervical approach-.

The surgical strategy for tumors located in or extending from the intracranial space to the infratemporal fossa was analyzed in 12 cases with various pathologies. A case of mandibular nerve schwannoma, which extended 1 cm below the external orifice of the foramen ovale, was completely removed via the epidural subtemporal approach without zygomatic osteotomy with partial removal of the middle cranial base. The inferior margin of infratemporal tumor could be accessed via the transcranial route with zygomatic or orbitozygomatic osteotomy without complications including facial nerve injury in nine

Reconstructive options and issues after extensive infratemporal fossa clearance for bucco-alveolar cancer.

PURPOSE OF REVIEW: This review addresses the timely and relevant challenge of reconstructing extensive infratemporal fossa defects following composite bucco-alveolar cancer resection. The complexity of these three-dimensional defects, involving mucosa, soft tissue, and skin, alongside the frequent need for adjuvant radiation and the proximity to dissected cervical vessels, necessitates a deliberate and robust reconstructive strategy. RECENT FINDINGS: Recent literature underscores a shift towards a structured, component-based reconstructive approach. This review details the utility and specifi

Efficacy Evaluation of Ultrasound Fusion-Guided Core Needle Biopsy for Infratemporal Fossa Lesions.

BACKGROUND: Lesions of infratemporal fossa (ITF) represent a group of pathologically heterogeneous diseases. Endoscopic biopsy has been the primary diagnostic method. To facilitate precise treatment, we introduced ultrasound fusion-guided core needle (UFCN) biopsy, which offers accuracy, minimal invasiveness, and real-time capability. This study aimed to evaluate the efficacy of UFCN biopsy for ITF lesions. METHODS: This retrospective cohort study compared patients with ITF lesions who underwent UFCN biopsy or not. Using logistic Poisson regression analysis, we evaluated the efficacy of UFCN
