Challenging anatomical paradigms: unexpected supratentorial localization of posterior fossa B ependymomas identified by DNA methylation.

Molecular subgroups of ependymomas are strongly associated with their primary anatomical compartments, namely the supratentorial region, posterior fossa and spine locations. Although it is generally accepted that posterior fossa ependymomas may arise from radial glia-like progenitor cells, the precise cellular origin of the distinct posterior fossa molecular subgroups remains to be determined. Given that DNA methylation profile strongly recapitulates developmental lineage and the cell of origin, the characterization of unexpectedly located molecular subgroups of ependymomas may provide insight

How I do it: pediatric lateral-type posterior fossa ependymoma resection.

BACKGROUND: Lateral-type ependymomas, or cerebellopontine ependymomas, of the posterior fossa are rare tumors extending into the cerebellopontine angle, encasing important neurovascular structures, presenting as a particular surgical challenge. METHOD: We performed a radical resection of lateral-type ependymoma in a 2-year-old patient with anamnesis of torticollis and vomiting. Resection of the tumor was preceded by insertion of external ventricular drainage. CONCLUSION: Optimal treatment improving prognosis requires achieving gross total resection of the tumor.

Placement of EVD in pediatric posterior fossa tumors: safe and efficient or old-fashioned? The Vienna experience.

PURPOSE: The perioperative treatment of hydrocephalus in pediatric posterior fossa tumors with an external ventricular drain (EVD) is the treatment of choice in our center. We analyzed our experience in using EVD concerning safety and effectivity. METHODS: This is a single-center retrospective cohort study of 100 consecutive pediatric patients who underwent resection for a newly diagnosed tumor in the posterior fossa between 2011 and 2022. RESULTS: Of the 100 patients with posterior fossa tumors, 80 patients (80%) had radiological signs of hydrocephalus at presentation, 49 patients (49%) of

Surgical site infection in paediatric posterior fossa surgery: does pathology matter?

OBJECTIVES: The aim of this study was to explore the rates and potential risks of surgical site infection (SSI) after posterior fossa surgery for tumour resection in children. METHODS: We retrospectively reviewed our local paediatric (age < 16 years) database for all cases of posterior fossa (PF) brain tumour surgery between November 2008 and November 2019. We collected patient demographics, tumour histology/location, and the event of postoperative surgical site infection. RESULTS: Overall, 22.1% (n=15) developed SSI out of sixty-eight children undergoing PF surgery for resection of brain tu

Cerebellar mutism: the predictive role of preoperative language evaluation.

BACKGROUND: The association between preoperative language impairment and development of cerebellar mutism after surgical treatment of posterior fossa tumors has gained increasing interest in recent years based on the concept that both local compression/infiltration of the infratentorial periventricular anatomical structures involved in speech and language, as well as an increased distraction of supratentorial periventricular anatomical structures, due to an associated hydrocephalus, involved in the coordination of speech circuits, might lead to the subclinical presence of language disturbances

Impact of a pediatric posterior fossa tumor and its treatments on motor procedural learning.

INTRODUCTION: Posterior fossa tumor (PFT) survivors have difficulty learning new skills. Procedural memory is a skill learning system that allows, through training, the automatization of procedures and progressive improvement of performance. It underlies most of the motor procedures in everyday life that we perform automatically, such as riding a bike or writing. Motor procedural memory is divided into two components: motor sequence learning involving mainly cortico-striatal networks, and motor adaptation involving mainly cortico-cerebellar networks. The aim of this work was to explore the imp

Imaging pitfalls in paediatric posterior fossa neoplastic and non-neoplastic lesions.

Paediatric posterior fossa lesions can have much overlap in their clinical and radiological presentation. There are, however, a number of key imaging features that can help the reading radiologist to distinguish tumours from important tumour mimics which are often inflammatory or metabolic entities. This pictorial review provides a number of important cases that proved challenging on imaging and illustrates some common pitfalls when interpreting lesions in the posterior fossa in children. Not everything that is abnormal will be a tumour, but often other causes are overlooked and misinterpreted

Melanotic intracranial epidermoid: Case report and description of a new subtype.

Intracranial epidermoids are generally seen as hypodense nonenhancing lesions on computed tomography scans; and, as T1 hypointense and T2 hyperintense lesions on magnetic resonance imaging (MRI). Unusual radiological findings have been reported earlier. The authors present the case of a 54-year old male patient who had prior intracranial surgery. On MRI, there was a thick peripheral mantle of diffusion restriction with a central core of brilliant T1 hyperintensity and very black T2 hypointensity. The peripheral mantle showing diffusion restriction was heterogeneously T1 hypointense and T2 hype

Minimally invasive endoscopic-assisted approaches to the posterior fossa.

Modern surgical approaches are becoming more minimalistic, associated with the term "minimally invasive." The endoscope provides a more panoramic anatomical view in addition to the ability to access narrow deep corners with decent illumination and clear visualization. Endoscopic assisted microsurgery through a tailored small craniotomy is the foundation of keyhole surgery. The endoscope can be advanced deeper into the field, thus enhancing an exposure and allowing bimanual dissection, ultimately providing smaller craniotomies and tailored key exposures. The term "minimally invasive" became ass

Development of Chromosome 1q+ Specific Treatment for Highest Risk Pediatric Posterior Fossa Ependymoma.

PURPOSE: There are no effective treatment strategies for children with highest-risk posterior fossa group A ependymoma (PFA). Chromosome 1q gains (1q+) are present in approximately 25% of newly diagnosed PFA tumors, and this number doubles at recurrence. Seventy percent of children with chromosome 1q+ PFA will die because of the tumor, highlighting the urgent need to develop new therapeutic strategies for this population. EXPERIMENTAL DESIGN: In this study, we utilize 1q+ PFA in vitro and in vivo models to test the efficacy of combination radiation and chemotherapy in a preclinical setting.

Role of computed tomography in the evaluation of pediatric brain tumor.

A total of forty two clinically diagnosed pediatric brain tumors were studied over a period of two years. The study was aimed to evaluate the efficacy of computed tomography (CT) in pre-operative diagnosis of pediatric brain tumors by correlating the imaging findings with postoperative histopathological findings. Site, density, mass effect and contrast enhancement of the lesion were studied as primary efficacy variables of CT scan. In the present study, the common pediatric brain tumors were astrocytoma, medulloblastoma, craniopharyngioma and ependymoma. In 18(42.9%) cases the tumors were supr

Treatment-resistant Inflammatory Demyelinating Pseudotumor with Marburg-Like Features: A Rare Case Report.

Inflammatory demyelinating pseudotumors (IDPs), also known as tumor-like demyelinating lesions, are rare conditions that mimic intracranial neoplasms, presenting significant diagnostic challenges. The Marburg variant of multiple sclerosis (MS), an aggressive form of demyelinating disease, can present with IDP, further complicating diagnosis. We report the case of a 51-year-old male who developed progressive neurological deficits over one month. Brain MRI revealed bilateral supra- and infratentorial lesions, initially raising suspicion of both demyelinating disease and neoplastic processes. Ext

MRI features of atypical teratoid/rhabdoid tumors in children.

BACKGROUND: Atypical teratoid/rhabdoid tumors (AT/RTs) are rare, highly malignant tumors of the central nervous system, usually occurring in young children. OBJECTIVE: To investigate the MRI features of AT/RT, with special emphasis on diffusion-weighted imaging (DWI) and MR spectroscopy (MRS). MATERIALS AND METHODS: MRI findings of 11 children with AT/RT were reviewed retrospectively, including DWI in 9 AT/RT children and MRS in 6 children. RESULTS: The neoplasms were infratentorial in 4 children and supratentorial in 5 children, both infra- and supratentorial in 1 child and multifocal in 1

Clinico-pathological correlations in meningiomas: a DNA and immunohistochemical study.

We have studied 41 meningiomas classified histologically as benign, atypical or anaplastic. There were 26 females and 15 males and the mean age was 53 years. 36 tumours were supratentorial, 4 infratentorial and one spinal. Flow cytometry was performed on paraffin-embedded tissue using a selective staining technique for DNA. The ploidy index of DNA and percentage of cells in the S and G2/M phases were calculated. Results were correlated with clinical, histological and immunohistological data. 16/41 tumours were found to be diploid, 17/41 aneuploid and 8/41 could not be analysed. Significant cor

Refractory Nausea and Vomiting Due to Central Nervous System Injury: A Focused Review.

The area postrema (AP) is a circumventricular organ (CVO) at the base of the fourth ventricle. It has a crucial role in regulating nausea and vomiting due to its unique blood-brain barrier (BBB)-permeability and extensive neural connectivity. Here, we present two cases of area postrema syndrome (APS), a rare condition of intractable nausea and vomiting resulting from direct AP injury. Our cases each occurred in the context of infratentorial neoplasms or their treatment. Using these cases as a framework, we review the literature on central emetic pathways and propose a treatment algorithm for m

Case report: Infratentorial Embryonal Tumor with Abundant Neuropil and True Rosettes (ETANTR) in an 8-month-old Maine Coon.

An 8-month-old female Main Coon with a history of recurrent behavioral changes and anorexia was presented with sternal recumbency and depression. Within 5 days, the cat progressively worsened with symptoms of stupor and coma and was euthanized. At post-mortem examination, a solid, grayish infratentorial mass located in the midline rostrally to the cerebellum, was observed. Histologically, highly cellular clusters of small-to-medium undifferentiated cells were intermingled with paucicellular areas with fibrillary eosinophilic (neuropil-like) appearance.
