Comparable effectiveness of caudal vs. trans-foraminal epidural steroid injections.

STUDY DESIGN: Retrospective case-control study. OBJECTIVE: To compare the effectiveness between caudal and trans-foraminal epidural steroid injections for the treatment of primary lumbar radiculopathy. SUMMARY OF BACKGROUND DATA: Spinal injections with steroids play an important role in non-operative care of lumbar radiculopathy. The trans-foraminal epidural steroid injection (TESI) theoretically has a higher success rate based on targeted delivery to the symptomatic nerve root. To our knowledge, these results have not been compared with other techniques of epidural steroid injection. METHO

Epidural corticosteroid injections for sciatica due to herniated nucleus pulposus.

BACKGROUND: Although epidural corticosteroid injections are commonly used for sciatica, their efficacy has not been established. METHODS: In a randomized, double-blind trial, we administered up to three epidural injections of methylprednisolone acetate (80 mg in 8 ml of isotonic saline) or isotonic saline (1 ml) to 158 patients with sciatica due to a herniated nucleus pulposus. All patients had Oswestry disability scores higher than 20 (on a scale of 1 to 100, with scores of 20 or less indicating minimal disability, and higher scores greater disability). RESULTS: At three weeks, the Oswestry

Bacterial meningitis and cauda equina syndrome after epidural steroid injections.

PURPOSE: To describe a rare adverse outcome resulting from lumbar epidural steroid injections for the treatment of chronic lower back pain. CLINICAL FEATURES: We report a case of staphylococcus aureus meningitis and cauda equina syndrome following a series of epidural steroid injections for chronic back pain. Although rare, bacterial meningitis following epidural analgesia has been reported, but epidural steroid injections have not been associated with either bacterial meningitis or cauda equina syndrome. The causal relationship between epidural steroid injections, bacterial meningitis, and c

A retrospective analysis of the efficacy of epidural steroid injections.

Forty patients were studied retrospectively to evaluate the effect of epidural steroid injections on low back pain and sciatica characteristic of spinal stenosis or a herniated lumbar disc. All but one of these patients had radicular symptoms. The average age was 55 years, and the average follow-up time was eight months. All patients were injected by the same anesthesiologist with 2 cc of Depomedrol-40. Thirty-six patients received either one, two, or three injections. Four patients received either four or five injections. The overall results were poor, with about 60% of patients reporting var

The Utility of Caudal Epidural Steroid Injections: A Comprehensive Review.

PURPOSE OF REVIEW: Caudal epidural steroid injections (ESIs) remain an important treatment option for lumbar radicular pain and other spinal pain disorders. Although transforaminal and interlaminar approaches are more commonly used in practice, the caudal approach offers unique technical and safety advantages in select patient populations. This review summarizes the current literature regarding the safety profile, technical considerations, efficacy, and clinical utility of caudal ESIs. RECENT FINDINGS: Recent literature supports the role of caudal ESIs as a safe and effective alternative epid

Evaluating Bilateral Symptom Relief Following Parasagittal Interlaminar Epidural Steroid Injections.

INTRODUCTION: Interlaminar epidural steroid injections (ILESIs) using a parasagittal approach are frequently employed to manage lumbar radicular pain. The degree of contralateral symptom relief achieved with parasagittal ILESIs remains underexplored. This study aims to evaluate the short-term effectiveness of parasagittal ILESIs in providing ipsilateral and contralateral symptom relief in patients with bilateral lumbar radiculopathy and unilateral contrast spread. METHODS: A retrospective review was conducted on 12 patients with bilateral lumbar radicular symptoms who underwent parasagittal I

Herpes zoster prevalence following epidural steroid injections: a retrospective review.

INTRODUCTION: Herpes Zoster, or shingles, is an infection caused by the reactivation of the latent Varicella zoster virus within a sensory ganglion, leading to painful skin lesions localized along dermatomes. Patients undergoing pain medicine procedures involving steroids may face an elevated risk of shingles, which can significantly impact their quality of life. Though rare, HZ has been reported following minimally invasive procedures, such as epidural steroid injections. OBJECTIVES: We evaluated the prevalence of shingles within 31 days after epidural steroid injections within Mayo Enterpri

FACTFINDERS FOR PATIENT SAFETY: Minimizing risks with cervical epidural injections.

This series of FactFinders presents a brief summary of the evidence and outlines recommendations to minimize risks associated with cervical epidural injections. Evidence in support of the following facts is presented. Minimizing Risks with Cervical Interlaminar Epidural Steroid Injections - 1) CILESIs should be performed at C6-C7 or below, with C7-T1 as the preferred access point due to the more generous dorsal epidural space at this level compared to the more cephalad interlaminar segments. This reduces the risk of the minor complication of dural puncture and the major complication of spinal

Practice Patterns of Physicians who Perform Caudal Epidural Steroid Injections.

INTRODUCTION: Caudal epidural steroid injections (ESIs) are commonly employed in the management of low back pain and radiculopathy. Despite their widespread use, practice patterns among physicians performing caudal ESIs can vary significantly. This study aims to identify variability in injection techniques utilized by physicians during caudal ESIs, focusing on steroid use, needle selection, and catheter use. This study also looks at major permanent neurological injuries related to caudal ESIs. METHODS: A survey was distributed to a cohort of physicians who regularly perform ESIs. The survey c

Establishing a common language for lumbar transforaminal epidural steroid injections.

BACKGROUND: Patients presenting to spine surgeons for lumbar radiculopathy often undergo initial conservative treatment including medications, therapy, and lumbar transforaminal epidural steroid injections. Despite a growing number of spinal injections performed, there is a lack of available data regarding the occurrence of wrong-site injections. However, when examined, the discrepancies between ordering level and level of epidural steroid injection performed are immense. To aid with this issue, we propose that instead of ordering a lumbar transforaminal epidural steroid injections at a given

The Clinical Efficacy for Two-Level Transforaminal Epidural Steroid Injections.

BACKGROUND: Epidural steroid injections have been used in combination with other modalities for the treatment of lumbar radicular pain. The literature has shown that 1-level lumbar fluoroscopically guided transforaminal epidural steroid injections (TFESIs) are effective in decreasing pain and morbidity; however, the efficacy of 2-level TFESIs has not been investigated, although they are performed by many practitioners in the treatment of lumbar radicular pain. OBJECTIVES: To assess the clinical effectiveness of 2-level TFESIs in patients with unilateral, single-level lumbar radicular pain. In

Pneumocephalus during cervical transforaminal epidural steroid injections: a case report.

A cervical transforaminal epidural injection of anesthetic and corticosteroids (CTFESI) is a frequently used procedure for cervical radiculopathy. Most cases of pneumocephalus after an epidural block occur when using an interlaminar approach with the loss-of-resistance technique. The authors present the first case of pneumocephalus after cervical transforaminal epidural injection of anesthetic and corticosteroids. A 64-yr-old woman with left C7 radiculopathy was undergoing C6-7 transforaminal epidural injection of anesthetic and corticosteroids. The epidural spread of contrast was checked by f

Single-Shot Epidural Injections in the Management of Radicular Pain.

Epidural injections are commonly used in the treatment of chronic low back pain due to symptomatic lumbar spinal disorders. The aim of the present investigation was to study their therapeutic value for different age subgroups. A consecutive series of 356 patients were treated with at least one injection, and assessed before and after injection. Significant pain reduction was observed in all age groups following a series of injections with the greatest reduction after the first one. Especially in patients younger than 50 years, pain medication could be reduced substantially. Surgery was perform

Intravascular flow detection during transforaminal epidural injections: a prospective assessment.

BACKGROUND: Transforaminal epidural steroid injections (TFESI) are a mainstay in the treatment of spine pain. Though this commonly performed procedure is generally felt to be safe, devastating complications following inadvertent intra-arterial injections of particulate steroid have been reported. The use of digital subtraction angiography (DSA) has been suggested as a means of detecting intra-arterial needle placements prior to medication injection. OBJECTIVE: To examine the efficacy of DSA in detecting intra-arterial needle placements during TFESI. STUDY DESIGN: Prospective cohort study eva

Epidural steroid injections for radicular lumbosacral pain: a systematic review.

Most clinical guidelines do not recommend routine use of epidural steroid injections for the management of chronic low back pain. However, many clinicians do not adhere to these guidelines. This comprehensive evidence overview concluded that off-label epidural steroid injections provide small short-term but not long- term leg-pain relief and improvement in function; injection of steroids is no more effective than injection of local anesthetics alone; post-procedural complications are uncommon, but the risk of contamination and serious infections is very high. The evidence does not support rout
