Existential suffering at the end of life in long-term care: A critical essay on recognition, dignity, and witness.

Existential suffering is widely acknowledged in palliative care, but it remains inconsistently recognized and unevenly addressed in long-term care. This critical essay argues that the gap is not caused by clinician indifference, but by a persistent mismatch between whole-person ideals and care systems organized around measurable symptoms, risk management, task completion, and professional uncertainty. Drawing on literature on suffering, existential loneliness, dignity, spiritual care, and recent critiques of impersonal care systems, the essay identifies three interrelated shortcomings: the red

ST-Elevation Myocardial Infarction: A Simulation Case for Evaluation of Interprofessional Performance in a Hospital.

INTRODUCTION: Interprofessional collaboration between units in a hospital is essential in order to reach desired time for primary percutaneous intervention (PCI) in acute ST-Segment Elevation Myocardial Infarction (STEMI) cases. We developed a simulation to engage various medical and nonmedical staff in interprofessional and interunit team collaboration. METHOD: We used a scenario in this simulation. Beginning in the emergency department, it detailed a 50-year-old male presenting with progressive chest pain since 7 hours before admission. The emergency team directly examined the patient, and

Type 1 Diabetes Management in the Hospital Setting.

The incidence of diabetes in children and adolescents has increased during the past decades, with a 1.9% increase per year in type 1 diabetes mellitus (T1DM). Patients with diabetes have a greater risk of hospitalizations compared with those without diabetes. Clear evidence has emerged in the past decade that supports appropriate glycemic control in the hospital setting to improve clinical outcomes and reduce the risk of hospital complications and mortality. Determining the appropriate insulin regimen in patients with T1DM in the hospital depends on the clinical status, type of outpatient insu

Development of a Multidisciplinary Pediatric Airway Program: An Institutional Experience.

Rapid response teams have become necessary components of patient care within the hospital community, including for airway management. Pediatric patients with an increased risk of having a difficult airway emergency can often be predicted on the basis of clinical scenarios and medical history. This predictability has led to the creation of airway consultation services designed to develop airway management plans for patients experiencing respiratory distress and who are at risk for having a difficult airway requiring advanced airway management. In addition, evolving technology has facilitated ai

Comparison of a direct vs consultative advanced heart failure role in the outcomes of extracorporeal membrane oxygenation patients.

BACKGROUND: Advanced heart failure and transplant (AHFTC) teams are crucial in the management of patients in cardiogenic shock. We sought to explore the impact of AHFTC physicians on outcomes in patients receiving extracorporeal membrane oxygenation (ECMO) support. AIM: To determine whether outcomes differ in the care of ECMO patients when AHFTC physicians serve in a primary vs consultative role. METHODS: We conducted a retrospective cohort study of 51 patients placed on veno-venous (VV) and veno-arterial (VA) ECMO between January 2015 and February 2023 at our institution. We compared ECMO o

Fighting diabetic foot ulcers-The diabetologist: A king maker of the fight.

Diabetic foot ulcer is a costly and serious complication of diabetes mellitus and is the major cause of non-traumatic limb amputations worldwide. Its development is primarily the result of diabetic neuropathy and/or peripheral arterial disease with accompanied bone abnormalities and is complicated by invasive infection. The management of this clinical condition focuses on identification of the "at-risk" foot, treatment of the ulcerated foot, and prevention of further complications. As diabetic foot ulcer represents the sum of multiple etiologies, its treatment requires a multidisciplinary team

Towards an understanding of best practice: The good, the bad and the future of cardiogenic shock teams.

Cardiogenic shock (CS) remains a high-mortality condition that demands rapid diagnosis, coordinated multidisciplinary management, and timely initiation of mechanical circulatory support. As more institutions implement dedicated CS teams, substantial heterogeneity has emerged in how these teams are structured, activated, and sustained. To better characterize this variability and begin defining the components of an optimal CS team, the Society of Critical Care Cardiology (SoCCC), in partnership with the Society for Cardiovascular Angiography and Interventions (SCAI), convened the Inaugural Cardi

Scabies in healthcare settings.

PURPOSE OF REVIEW: In industrialized countries, epidemics of scabies are mainly described within families or in institutions such as healthcare settings. Control of institutional scabies is associated with a considerable working and economic burden, but guidelines for the management are scarce. RECENT FINDINGS: The prevalence of institutional scabies is probably underestimated. Identified risk factors for institutional scabies outbreaks include the institution type, extensive physical contact with patients and movement of patients, the existence of crusted scabies, a long diagnostic delay and

Changes in Care for Acute Pulmonary Embolism Through A Multidisciplinary Pulmonary Embolism Response Team.

BACKGROUND: Optimal management of acute pulmonary embolism requires expertise offered by multiple subspecialties. As such, pulmonary embolism response teams (PERTs) have increased in prevalence, but the institutional consequences of a PERT are unclear. METHODS: We compared all patients that presented to our institution with an acute pulmonary embolism in the 3 years prior to and 3 years after the formation of our PERT. The primary outcome was in-hospital pulmonary embolism-related mortality before and after the formation of the PERT. Sub-analyses were performed among patients with elevated-ri

Applying theory to practice in a large research center: creating and implementing tools for building convergence capacity in individuals and teams.

The continued expansion of team science underscores the urgent need for shared, actionable models that can be validated to support effective interdisciplinary collaboration in scientific teams. This paper presents a practice-grounded framework for building convergence capacity in large, multi-institutional research environments, drawing on insights from the Science of Team Science (SciTS) and Integration and Implementation Sciences (I2S). Using the U.S. NSF-funded Science and Technologies for Phosphorus Sustainability (STEPS) Center as a case study, we explore how integration expertise can be

Air evacuation under high-level biosafety containment: the aeromedical isolation team.

Military contingency operations in tropical environments and potential use of biological weapons by adversaries may place troops at risk for potentially lethal contagious infections (e.g., viral hemorrhagic fevers, plague, and zoonotic poxvirus infections). Diagnosis and treatment of such infections would be expedited by evacuating a limited number of patients to a facility with containment laboratories. To safely evacuate such patients by military aircraft and minimize the risk for transmission to air crews, caregivers, and civilians, the U.S. Army Medical Research Institute of Infectious Dis

Cardiogenic Shock: Protocols, Teams, Centers, and Networks.

The mortality of cardiogenic shock (CS) remains unacceptably high. Delays in the recognition of CS and access to disease-modifying or hemodynamically stabilizing interventions likely contribute to poor outcomes. In parallel to successful initiatives in other disease states, such as acute ST-elevation MI and major trauma, institutions are increasingly advocating the use of a multidisciplinary 'shock team' approach to CS management. A volume-outcome relationship exists in CS, as with many other acute cardiovascular conditions, and the emergence of 'shock hubs' as experienced facilities with an i

A systematic review of multidisciplinary teams to reduce major amputations for patients with diabetic foot ulcers.

OBJECTIVE: Multiple single-center studies have reported significant reductions in major amputations among patients with diabetic foot ulcers after initiation of multidisciplinary teams. The purpose of this study was to assess the association between multidisciplinary teams (ie, two or more types of clinicians working together) and the risk of major amputation and to compile descriptions of these diverse teams. METHODS: We searched PubMed, Scopus, Cumulative Index to Nursing and Allied Health, and Cochrane Central Register of Controlled Trials from inception through May 24, 2019 for studies re
