Medicare, Medicaid, and dual enrollment for adults with intellectual and developmental disabilities.

OBJECTIVE: Given high rates of un- and underemployment among disabled people, adults with intellectual and developmental disabilities rely on Medicaid, Medicare, or both to pay for healthcare. Many disabled adults are Medicare eligible before the age of 65 but little is known as to why some receive Medicare services while others do not. We described the duration of Medicare enrollment for adults with intellectual and developmental disabilities in 2019 and then compared demographics by enrollment type (Medicare-only, Medicaid-only, dual-enrolled). Additionally, we examined the percent in each e

To what extent have relaxed eligibility requirements and increased generosity of disability benefits acted as disincentives for employment? A systematic review of evidence from countries with well-developed welfare systems.

BACKGROUND: Reductions in the eligibility requirements and generosity of disability benefits have been introduced in several Organisation for Economic Cooperation and Development (OECD) countries in recent years, on the assumption that this will increase work incentives for people with chronic illness and disabilities. This paper systematically reviews the evidence for this assumption in the context of well-developed welfare systems. METHOD: Systematic review of all empirical studies from five OECD countries from 1970 to December 2009 investigating the effect of changes in eligibility require

Disparities in periodontitis risk and healthcare use among individuals with disabilities in Korea: a retrospective cohort study.

BACKGROUND: We analyzed the relationship between disability status and periodontal disease, focusing on disparities in healthcare utilisation, including outpatient visits and hospitalisation rates, among disability types and severities. METHODS: This study used data from the National Health Insurance Service(NHIS) of Korea, which includes comprehensive records of the insured population. We examined 966,200 individuals with disabilities, grouped into five categories, and applied propensity score matching to compare with a matched control population. Periodontal disease was defined by the Korea

Low-income fathers' access to health insurance.

We examine the prevalence and correlates of health insurance status among low-income fathers, a group not previously studied in this context. In a sample of 1,653 low-income fathers from a national urban birth cohort study, 29% had private, 14% had public, and 58% had no insurance. Privately insured fathers had greater levels of human capital than did publicly insured fathers; the latter more closely resembled uninsured fathers than they did privately insured fathers. Multinomial logistic regression analysis indicates that being older, being employed, being married, and having a job offering h

Disability pension and sick leave in people with HIV compared with the background population - a nationwide population-based matched cohort study.

OBJECTIVES: People with HIV (PWH) are unable to get private disability insurance on a regular basis in contrast with individuals with other chronic diseases. We aimed to estimate the risk of public disability pension and work absence due to sickness for PWH compared with the background population in Denmark. DESIGN: Nationwide, population-based, matched cohort study of employed PWH with favorable disease characteristics. A comparison cohort of employed individuals was matched 10 : 1 to PWH by date of birth and sex from the general population. METHODS: We computed time to first date of 4 week

Barriers to breast and cervical cancer screening for women with physical disability: A review.

This review critically examined the barriers to breast and cervical cancer screening services for women with physical disability and discussed ways forward to change practice. When compared to the rest of the community, women with disability were less likely to use preventive health screening services for multiple reasons. Moreover, women with disability live longer than in previous years, and as age is linked to an increased risk of developing cancer, it is imperative that the barriers to screening for these women become a focus of discussion. We designed an integrative literature review to i

The independent medical examination: cardiology assessment.

Insurance companies frequently seek medical opinions from various specialists concerning the severity of a medical problem, the appropriateness of a treatment plan and a assessment of the degree of medical impairment in persons who claim they are entitled to disability benefits. The insurer is requesting a medical opinion from a physician not involved in the care of the claimant and with no regular business ties to the insurance company; the insurance industry refers to this as an independent medical examination (IME). The purpose of the cardiology IME is to have an objective assessment concer

Labor market entry prospects of people with visual impairments: evidence from administrative register data in the Netherlands.

The UN's Convention on the Rights of Persons with Disabilities includes a right to equal work opportunities. People with visual impairments (VI) are targeted by this convention and experience low employment rates. The initial transition from education to labor is a predictor for later labor market participation. Exploiting administrative register data, we identified a graduated subpopulation with VI and matched them to a control group. This paper evaluates (1) group differences in various socio-economic outcomes following graduation, and (2) following initial employment. Following graduation t

Injury severity assessment for car occupants in frontal impacts, using disability scaling.

Injury classification and assessment is one of the most important fields of injury prevention. At present, injury assessment focuses primarily on the risk of fatalities, in spite of the fact that most people who are injured survive the trauma. The net result of a fatality-based approach is that safety and vehicle engineers must make decisions with an incomplete, and sometimes misleading, picture of the traffic safety problem. By applying disability scaling reflecting long-term consequences to injury data, the most significant disabling injuries can be identified. The priorities change with the

[Follow-up Care in the Field of Medical Rehabilitation Concerning Psychosomatic Indications Based on the New Conceptual Framework of the German Pension Insurance].

Mental disorders rank first amongst all causes for disability pensions and second in the field of medical rehabilitation. Especially alarming is the significantly lower age of entry of the mentally ill disability pensioners, compared to those with other indications. Mentally ill people often look back at a long history of diseases before getting in contact with the German pension insurance. In this regard the German pension insurance, which is obligated to effectively support people in order to keep them in working life until reaching the regular retirement age, is facing a big challenge, whic

'Yes, I've got the job, but my challenge is keeping the job': an evaluation of a new pathway to open employment to meet the needs of people with acquired brain injury in Australia.

BACKGROUND AND OBJECTIVES: In Australia, people with disability continue to experience low employment rates (48%), compared to the national average (79%), and employment is even lower (30%) for people with acquired brain injury (ABI). This paper evaluates a pilot study of a new mainstream employment pathway following ABI, called Employment CoLab. METHOD: Employment CoLab was piloted across multiple industries using a mix of reasonable employer adjustments, insurance-funded supports and/or access to capacity-building supports. Semi-structured interviews were undertaken with four stakeholders;

[Persons requiring long-term care: Recommendation and utilization of rehabilitative therapies].

In Germany all members of the statutory health insurance are also compulsory members of the nursing care insurance which financially supports nursing care at home and in nursing homes. Benefits are provided dependent on a standardized medical examination performed at the home of the applicants by trained physicians or nurses of the health insurances' medical service (MDK). The benefits are granted to those persons who are limited in the performance of activities of daily living due to physical, cognitive or mental disorders. [Assessment of eye damage for private accident insurance].

Together with a team from the HUK association (an association of liability, accident, transport and legal protection insurance-companies) the authors discussed a new assessment table for diminution of vision and have recommended its use by private accident insurance companies. The table was worked out on the basis of guidelines issued by the Deutsche Ophthalmologische Gesellschaft in 1981. In cases of bilateral eye damage the total extent of the disability - based on the specific diminution of usability of each eye - should be calculated according to the conditions of the insurance companies a

Disability Prevalence and the Awareness-Utilization Gap for Disability Services in an Urban Community of Delhi, India: A Cross-Sectional Study.

INTRODUCTION: Disability-related welfare schemes form a critical component of India's social protection architecture, yet community-level evidence on the gap between awareness and utilization in urban settings remains limited. This study aimed to estimate the prevalence of disability and its socio-demographic correlates and to assess awareness and utilization of disability-related services among adults in an urban community of Delhi, generating baseline data on the awareness-utilization gap for this setting. METHOD: A community-based cross-sectional study was conducted in Palam, an urban rese

Leaving no one behind? Social inclusion of health insurance in low- and middle-income countries: a systematic review.

BACKGROUND: One way to achieve universal health coverage (UHC) in low- and middle-income countries (LMIC) is the implementation of health insurance schemes. A robust and up to date overview of empirical evidence assessing and substantiating health equity impact of health insurance schemes among specific vulnerable populations in LMICs beyond the more common parameters, such as income level, is lacking. We fill this gap by conducting a systematic review of how social inclusion affects access to equitable health financing arrangements in LMIC. METHODS: We searched 11 databases to identify peer-

Logging injuries in Louisiana: nature, trends, and rehabilitation considerations.

Logging is a dangerous occupation, as documented through reported injuries and fatalities. Accidents in the logging industry account for pain, suffering, employee turnover, and high insurance costs. Worker injuries and high insurance costs, in addition to other factors, may be minimized and reduced through safety education and improved logging practices. Furthermore, the injured logger, or logger with a disability may return to logging or other employment through development of viable medical and vocational rehabilitation planing. This article addresses the economic impact of logging, reviews
