Neurally adjusted ventilatory assist (NAVA) improves patient-ventilator interaction during non-invasive ventilation delivered by face mask.

PURPOSE: To determine if, compared to pressure support (PS), neurally adjusted ventilatory assist (NAVA) reduces patient-ventilator asynchrony in intensive care patients undergoing noninvasive ventilation with an oronasal face mask. METHODS: In this prospective interventional study we compared patient-ventilator synchrony between PS (with ventilator settings determined by the clinician) and NAVA (with the level set so as to obtain the same maximal airway pressure as in PS). Two 20-min recordings of airway pressure, flow and electrical activity of the diaphragm during PS and NAVA were acquired

Influence of body position, PEEP and intra-abdominal pressure on the catheter positioning for neurally adjusted ventilatory assist.

PURPOSE: Neurally adjusted ventilatory assist (NAVA) relies on the patient's electrical activity of the diaphragm (EAdi) for actuating the ventilator. Thus a reliable positioning of the oesophageal EAdi catheter is mandatory. We aimed to evaluate the effects of body position (BP), positive end-expiratory pressure (PEEP) and intra-abdominal pressure (IAP) on catheter positioning. METHODS: Twenty-one patients were enrolled in this study. In six different situations [supine or 45° head of bed elevation (HBE) at PEEP 5 and 15 cmH(2)O; left lateral anti-decubitus at PEEP 5 cmH(2)O; supine at PEEP

Chemosensitivity and regulation of ventilation during sleep at high altitudes.

The paper presents new observations on young high altitude natives (Andes and Himalayas), testing the hypothesis that periodic breathing with apnea during sleep is determined by their ventilatory sensitivity to hypoxia and its interaction with the sleep state. The hypothesis is in general supported by the evidence. But, contrary to expectation, the ventilatory sensitivity to hypoxia in the Sherpa children was significantly lower than those in the Andes. Despite that departure, the magnitude of ventilatory periodicity among the subjects was internally consistent with their ventilatory sensitivi

Efficacy of Noninvasive Neurally Adjusted Ventilatory Assist (NIV-NAVA) for Neonates With Transient Tachypnea of the Newborn: A Retrospective Analysis.

BACKGROUND: Noninvasive neurally adjusted ventilatory assist (NIV-NAVA) provides synchronized respiratory support based on diaphragmatic electrical activity and has shown efficacy in preterm neonates. However, its efficacy in late preterm and term neonates with transient tachypnea of the newborn (TTN) remains unclear. METHODS: We retrospectively analyzed 183 neonates (≥ 34 weeks' gestation) with TTN between 2016 and 2024 who received initial respiratory support with either NIV-NAVA (n = 68) or sigh positive airway pressure (SiPAP, n = 115). After propensity score matching (PSM), we compared p

Non-invasive neurally adjusted ventilatory assist (NIV-NAVA) reduces extubation failures in preterm neonates-A systematic review and meta-analysis.

AIM: To analyse the evidence of non-invasive neurally adjusted ventilatory assist (NIV-NAVA) in preterm neonates compared to nasal continuous positive airway pressure (nCPAP) or nasal intermittent positive pressure ventilation (NIPPV). METHODS: We performed a systematic review and meta-analysis of randomised controlled trials and included studies where NIV-NAVA was analysed in preterm (<37 gestational weeks) born neonates. Our main outcomes were the need for endotracheal intubation, the need for surfactant therapy, and reintubation rates. Risk ratios (RRs) with 95% confidence intervals (CIs)

Lung recruitment maneuver during proportional assist ventilation of preterm infants with acute respiratory distress syndrome.

OBJECTIVE: To investigate the effect of lung recruitment maneuver (LRM) with positive end-expiratory pressure (PEEP) on oxygenation and outcomes in preterm infants ventilated by proportional assist ventilation (PAV) for respiratory distress syndrome (RDS). STUDY DESIGN: Preterm infants on PAV for RDS after surfactant randomly received an LRM (group A, n=12) or did not (group B, n=12). LRM entailed increments of 0.2 cm H2O PEEP every 5 min, until fraction of inspired oxygen (FiO2)=0.25. Then PEEP was reduced and the lung volume was set on the deflation limb of the pressure/volume curve. When s

Non-invasive neurally adjusted ventilatory assist (NIV-NAVA) in the neonatal intensive care unit (NICU): an Australian NICU experience.

BACKGROUND: Preterm infants often require non-invasive breathing support while their lungs and control of respiration are still developing. Non-invasive neurally adjusted ventilatory assist (NIV-NAVA) is an emerging technology that allows infants to breathe spontaneously while receiving support breaths proportional to their effort. This study describes the first Australian Neonatal Intensive Care Unit (NICU) experience of NIV-NAVA. METHODS: Retrospective cohort study of infants admitted to a major tertiary NICU between October 2017 and April 2021 supported with NIV-NAVA. Infants were divided

Comparing changing neurally adjusted ventilatory assist (NAVA) levels in intubated and recently extubated neonates.

OBJECTIVE: Neurally adjusted ventilatory assist (NAVA) is a mode of mechanical ventilation that delivers ventilatory support in synchrony to the patient's respiratory needs using NAVA level, a proportionality constant that converts the electrical activity of the diaphragm (Edi) into a peak pressure (PIP). Recent published studies suggest that neonates can control the delivered ventilatory support through neural feedback. Systematically increasing the NAVA level initially increases the PIP while maintaining a constant Edi until an inflection point or breakpoint (BrP) is reached, at which time t

Mechanical ventilation in ARDS: navigating the fine line with real-time monitoring.

Acute respiratory distress syndrome (ARDS) is a heterogeneous and rapidly evolving condition in which mechanical ventilation must balance adequate gas exchange with lung protection and hemodynamic stability. Over the past decades, the traditional one-size-fits-all approach to ARDS management has been progressively replaced by precision strategies guided by patient-specific physiological characteristics. However, conventional ventilatory and hemodynamic management continues to rely on intermittent measurements that fail to capture the physiological fluctuations typical of ARDS. Recent advances,

[Artificial ventilation, a special interaction of pulmonary mechanics and gas exchange].

During artificial ventilation the lungs are intermittently insufflated by respiratory gases in a certain pattern (ventilatory conditions). The consequences in terms of stretch of lung tissue, airway pressure, alveolar pressure and gas exchange depend on the mechanical properties of airways, lung tissue and thoracic wall. These properties are age dependent and may be changed considerably by disease. The basic principles of ventilatory support can be explained as an interaction of ventilatory gases passing through the airways in and out of the lungs, the subsequent changes in stretch of lung tis

A physiologic Comparison of Continuous Neurally Adjusted Ventilation (NeuroPAP) Versus Neurally-Adjusted Ventilatory Assist (NAVA) in Infants With Respiratory Failure.

OBJECTIVES AND HYPOTHESIS: Tonic diaphragmatic activity is common during non-invasive ventilation (NIV), suggesting efforts to increase end-expiratory lung volume. We assessed the feasibility and physiological impact of NeuroPAP, a novel NIV mode continuously adjusting the delivered positive pressure proportionally to diaphragm electric activity (Edi) during both inspiration and expiration, in infants with respiratory failure. We hypothesized NeuroPAP would enable dynamic control of end-expiratory pressures (PEEP). METHODOLOGY: This prospective crossover study enrolled premature neonates (25-

Neural control of ventilation prevents both over-distension and de-recruitment of experimentally injured lungs.

BACKGROUND: Endogenous pulmonary reflexes may protect the lungs during mechanical ventilation. We aimed to assess integration of continuous neurally adjusted ventilatory assist (cNAVA), delivering assist in proportion to diaphragm's electrical activity during inspiration and expiration, and Hering-Breuer inflation and deflation reflexes on lung recruitment, distension, and aeration before and after acute lung injury (ALI). METHODS: In 7 anesthetised rabbits with bilateral pneumothoraces, we identified adequate cNAVA level (cNAVAAL) at the plateau in peak ventilator pressure during titration p

Clinical factors associated with success of proportional assist ventilation in the acute phase of critical illness: pilot study.

REASON: Proportional assist ventilation plus (PAV+) applies pressure depending on the patient's inspiratory effort, automatically adjusting flow and volume assist to changes in respiratory mechanics. We aimed to assess the clinical factors associated with the success of PAV+ as first-line support in the acute phase of critical illness. METHODS: A prospective cohort study was carried out. Mechanically ventilated patients>24h were switched from assist-control ventilation to PAV+ as soon as they regained spontaneous breathing activity. PAV+ was set to deliver the highest assistance. Feasibility of Non-invasive Neurally Adjusted Ventilator Assist After Congenital Diaphragmatic Hernia Repair.

BACKGROUND: The use of neurally adjusted ventilator assist (NAVA) in congenital diaphragmatic hernia (CDH) patients has been historically deemed unwise, since the trigger for breaths is the electromyographic activity of the diaphragmatic muscle. We report on our NAVA experience in CDH patients. METHODS: We performed an IRB-approved retrospective review of newborns from 1/1/2012-1/1/2017 at a Level I Children's Surgery Center undergoing CDH repair. Physiologic response to various levels of pressure support and NAVA in prolonged weaning.

Neurally adjusted ventilatory assist (NAVA) is a mode of ventilation wherein the delivered assistance is proportional to diaphragm electrical activity (EAdi) throughout inspiration. We assessed the physiologic response to varying levels of NAVA and pressure support ventilation (PSV) in 13 tracheostomised patients with prolonged weaning. Each patient randomly underwent 8 trials, at four levels of assistance either in PSV and NAVA. i - high (no dyspnoea and/or distress); iv - low (associated with dyspnoea and/or distress; ii and iii - at ∼75% and ∼25% of the difference between high and low suppo
