[Treatment of intestinal failure in adults. I. Dietary measures].

Patients with intestinal failure, predominantly caused by short-bowel syndrome, have impaired quality of life due to the frequent development of complications. Dietary modifications have an established role in the treatment of short-bowel syndrome. Treatment of short-bowel syndrome includes optimising the balance of fluids and nutrients in the presence of reduced absorption. The population is heterogeneous due to differences in anatomical structure and the functional status of the remaining intestine. Diet must therefore be tailored to the individual patient. Determining the appropriate amount

Irreversible intestinal failure, nutrition support, and small bowel transplantation.

OBJECTIVES: Prolonged total parenteral nutrition (TPN) is a palliative but life-saving treatment for patients with irreversible intestinal failure (IIF). During the past few years, intestinal transplantation (ITx) has become the most realistic alternative to TPN. METHODS: We identified potential candidates for ITx, reported clinical data of the international Intestinal Transplant Registry, and analyzed the intestinal function after transplantation and the common nutrition strategies usually adopted in the recipients. RESULTS: At present, candidates for ITx are patients with IIF who develop a

Short bowel syndrome-associated intestinal failure: diagnosis and prognosis.

INTRODUCTION: Short bowel syndrome is characterized by severe intestinal malabsorption of macronutrients, micronutrients, fluid, and electrolytes. As such, short bowel syndrome often results in permanent need of parenteral support, that is, intestinal failure. To optimize patient outcomes, it is important to establish precise definitions of short bowel syndrome and intestinal failure. Classifying those with SBS can provide useful prognostic, risk and therapeutic implications. AREAS COVERED: This narrative review will focus on the diagnosis and prognosis of this rare condition. Pathogenesis and Management of Intestinal Failure-Associated Liver Disease.

Long-term parenteral nutrition (PN) has considerably improved the management of intestinal failure (IF) in children and adults, particularly those with short bowel syndrome; however, it carries a significant risk of hepatotoxicity, specifically, intestinal failure-associated liver disease (IFALD), also known as PN-associated liver disease. This review provides an update on the latest understanding of IFALD pathogenesis, emerging therapies, and ongoing challenges in the management of this complication. A number of factors are associated with the development of IFALD. PN lipid emulsions, phytost

Incidence, predictors and outcome of neonatal-onset intestinal failure.

BACKGROUND: Neonatal-onset Intestinal Failure (IF) and Short Bowel Syndrome (SBS) are associated with significant morbidity. We aimed to estimate incidence, identify predictors and determine outcomes of neonatal IF and SBS. MATERIALS AND METHODS: Retrospective review of children admitted to Queensland Children's Hospital between April, 2018 to March, 2022 who received parenteral nutrition (PN) in the neonatal period. IF was defined as those who required PN> 60 days from a gastrointestinal cause. Population-based estimate of incidence was calculated using census data from the national bureau o

Does Intestinal Peristalsis Cause Suture Failure After Instrument Suture?

BACKGROUND/AIM: Gastrectomy with lymphadenectomy is a standard treatment for gastric cancer. Anastomotic leakage remains a potentially fatal complication of gastrectomy. Forceful stapler extraction may cause anastomotic complications. We focused on the duodenal peristalsis, as we hypothesized that it might cause forceful stapler extraction. We then retrospectively investigated duodenal peristalsis and reviewed videos of Da Vinci system cases to clarify the relationship between peristalsis and anastomotic complications. PATIENTS AND METHODS: Forty-nine cases with stored videos of laparoscopic

Factors Affecting the Gut Microbiome in Pediatric Intestinal Failure.

BACKGROUND: There is little data on gut microbiome and various factors that lead to dysbiosis in pediatric intestinal failure (PIF). This study aimed to characterize gut microbiome in PIF and determine factors that may affect microbial composition in these patients. METHODS: This is a single-center, prospective cohort study of children with PIF followed at our intestinal rehabilitation program. Stool samples were collected longitudinally at regular intervals over a 1-year period. Medical records were reviewed, and demographic and clinical data were collected. Medication history including the

The gut microbiome and intestinal failure-associated liver disease.

Intestinal failure-associated liver disease (IFALD) is a common hepatobiliary complication resulting from long-term parenteral nutrition (PN) in patients with intestinal failure. The spectrum of IFALD ranges from cholestasis, steatosis, portal fibrosis, to cirrhosis. Development of IFALD is a multifactorial process, in which gut dysbiosis plays a critical role in its initiation and progression in conjunction with increased intestinal permeability, activation of hepatic immune responses, and administration of lipid emulsion. Gut microbiota manipulation including pre/probiotics, fecal microbiota

Management of Short Bowel Syndrome (SBS) and Intestinal Failure.

Short bowel syndrome (SBS) is a chronic disease whose natural history requires a changing array of management strategies over time. Chief amongst these is the chronic use of parenteral nutrition (PN) to ensure adequate nutritional intake. With time and appropriate management, approximately half of all SBS patients will successfully regain a functional, baseline level of intrinsic bowel function that will allow for them to achieve PN independence. However, the other half of SBS patients will progress into chronic intestinal failure which warrants a change in therapy to include more aggressive m

Complementary food introduction practices in infants with intestinal failure.

BACKGROUND: Oral intake in infants with intestinal failure (IF) may be limited due to intolerance or feeding difficulties. Guidelines for the introduction of semisolid or solid complementary foods (CFs) to infants with IF do not exist. CF intake and caloric contribution from CF is difficult to assess due to malabsorption and incomplete recording. The aim of this study was to identify institutional approaches to introducing CF to infants with IF. METHODS: The American Society for Parenteral and Enteral Nutriton (ASPEN) Pediatric Intestinal Failure Section Registered Dietitian/Nutritionist (RDN

Current strategies for managing intestinal failure-associated liver disease.

Introduction: Intestinal failure-associated liver disease (IFALD) refers to hepatic dysfunction that results from prolonged parenteral nutrition (PN) use. IFALD is multifactorial in origin and remains a major cause of morbidity and mortality. Prior to 2004, IFALD was associated with mortality as high as 90% in infants who remained on PN greater than 1 year. The advent of new strategies for intravenous lipid emulsion (ILE) administration and improved catheter care now allow many patients to remain on PN and recover from this once fatal condition. Several additional treatment modalities are ofte

Intravenous lipid emulsions in pediatric patients with intestinal failure.

UNLABELLED: The incidence of cholestatic liver disease (CLD) in pediatric patients suffering intestinal failure (IF) is not well established. Due to persistent portal inflammation, about 20% of these patients will progress to end-stage intestinal failure associated liver disease (IFALD) leading to liver transplant or death. PURPOSE OF REVIEW: Premature babies as well as infants with short bowel syndrome (SBS) and repeated sepsis (catheter or small intestinal bacterial overgrowth related) are at risk of developing CLD. Clinical data in SBS infants focused on intravenous lipid emulsion (ILE) as

Short Bowel Syndrome and Intestinal Failure in Crohn's Disease.

Crohn's disease is a chronic and progressive inflammatory disorder of the gastrointestinal tract. Despite the availability of powerful immunosuppressants, many patients with Crohn's disease still require one or more intestinal resections throughout the course of their disease. Multiple resections and a progressive reduction in bowel length can lead to the development of short bowel syndrome, a form of intestinal failure that compromises fluid, electrolyte, and nutrient absorption. The pathophysiology of short bowel syndrome involves a reduction in intestinal surface area, alteration in the ent

Some new insights in intestinal failure-associated liver disease.

PURPOSE OF REVIEW: To point new insights in the cholestasis that is a complication of both intestinal failure and parenteral nutrition. View on liver disease has recently evolved with the onset of fish oil-based intravenous lipid emulsions (ILE). RECENT FINDINGS: Focused on the role of ILE in causing liver disease. Reversal of cholestasis was recently achieved in infants with short bowel syndrome, by replacing the 'reference' soybean oil-based ILE by fish oil-based ILE. SUMMARY: It is likely that this reversal involves several factors such as the change in n-6: n-3 ratio, the reduction in ph

Metformin retention independent of renal failure in intestinal occlusion.

Metformin is eliminated by the kidneys, and metformin accumulation has always been noticed in oligo-anuric patients. We have reported an exception to the rule with the case of a metformin-treated patient having metformin accumulation contrasting with a mild increase in serum creatinine in the context of a volvulus of the sigmoid colon. This case prompted us to examine the association between intestinal occlusion and plasma metformin concentrations. For this purpose, we developed an experimental animal model of mechanical obstruction of the intestine. Rats were pre-treated during 3 weeks via dr
