Tolerability of intracameral cefuroxime during cataract surgery in case of penicillin allergy.

INTRODUCTION: Since the results of the ESCRS study in 2007 and then the AFSSAPS recommendations of 2011, postoperative endophthalmitis prophylaxis in cataract surgery has evolved toward intracameral cefuroxime. Penicillin allergy is frequent and is not considered as a contra-indication to cefuroxime injection, but cross-reactions do exist. The goal of this study was to assess the tolerability of intracameral cefuroxime in patients with a penicillin allergy. MATERIALS AND METHODS: In this monocentric open prospective study, adult patients undergoing cataract surgery and declaring themselves pe

Endophthalmitis prevention in Refractive Lens Exchange.

The aim of this study was to evaluate the safety and efficacy of the intracameral antibiotics vancomycin, cefuroxime, and moxifloxacin, as well as the need for topical antibiotics preoperatively, postoperatively, or both. A systematic review and meta-analysis were conducted using PubMed to compare their efficacy in cataract surgery performed by phacoemulsification. We also assessed the additional benefit of topical antibiotics in preventing endophthalmitis when intracameral antibiotics and povidone-iodine were used. All included studies supported the use of intracameral antibiotics at the end

The Role of Prophylactic Topical Administration of Antibiotics in Uncomplicated Cataract Surgery in the Era of Intracameral Antibiotics.

INTRODUCTION: Cataract surgery is one of the most commonly performed procedures in human medicine. Although the risk of postoperative inflammatory complications is low, their consequences can be devastating. The current global standard for preventing such complications is intracameral administration of antibiotics, while the role of prophylactic topical antibiotics in uncomplicated cases remains controversial. AIM: The aim of this paper is to evaluate the available evidence regarding the effectiveness of topical antibiotic prophylaxis in uncomplicated cataract surgery in the era of routine in

Safety of intracameral application of moxifloxacin and dexamethasone (Vigadexa®) after phacoemulsification surgery.

BACKGROUND: Intracameral antibiotics, such as moxifloxacin and cefuroxime, are safe to corneal endothelial cells and effective prophylaxis of endophthalmitis after cataract surgery. Corneal endothelial cells decrease in density after cataract surgery. Any substance used in the anterior chamber may affect corneal endothelial cells and lead to a greater decrease in density. This study wants to determine the percentage of endothelial cell loss after cataract extraction by phacoemulsification with off-label intracameral injection of moxifloxacin and dexamethasone (Vigadexa®). METHODS: An observat

Ocular delivery of lipid nanoparticles-formulated mRNA encoding lanosterol synthase ameliorates cataract in rats.

Cataract caused by crystallin aggregation is the leading cause of vision impairment and blindness globally. The only available treatment option so far is surgery. In this study, we leverage lipid nanoparticles (LNPs)-formulated mRNA encoding human lanosterol synthase (hLSS) to elevate lanosterol levels in the lens as a potential anti-cataract therapy. hLSS mRNA delivered with aromatized LNPs can be avidly taken up and translated into hLSS proteins in mammalian cells. mRNA formulations administered via intravitreal, subconjunctival, intracameral, or subretinal injection in rats display distinct

Role of eicosanoids in the ocular response to intracameral hydrogen peroxide.

The intracameral injection of hydrogen peroxide induces a sequence of responses in the tissues bounding the anterior chamber. These changes include intraocular pressure, corneal thickness, iris hyperemia, increased leakiness of the iris vasculature, and edema of the ciliary processes as judged from microscopic examination. Some of these responses appear to include inflammatory effects that may be the result of the local release of eicosanoids. Several antagonists of the arachidonic acid cascade, indomethacin, aspirin, dexamethasone, and nordihydroguaiaretic acid (NDGA) were used to examine the

Sulfur hexafluoride gas in the repair of Descemet's membrane detachment.

We report a case of successful attachment of a detached Descemet's membrane using an intracameral injection of sulfur hexafluoride (SF6). Three months after cataract surgery, a 77-year-old woman was seen for almost total detachment of Descemet's membrane. Surgical repair with a through-and-through mattress suture was unsuccessful. One month later, Descemet's membrane was apposed by injecting 0.25 ml of 100% SF6 into the anterior chamber. Six months later the central corneal thickness measured 0.48 mm, and Descemet's membrane was attached. We believe this to be the first reported case of succes

Evaluation of Systemic Exposure to Intracameral Tropicamide 0.02%, Phenylephrine 0.31%, and Lidocaine 1% in Pediatric Cataract Surgery.

Purpose: Pediatric cataract surgery requires mydriasis, typically achieved with eye drops. An intracameral mydriatic combining tropicamide 0.02%, phenylephrine 0.31%, and lidocaine 1% is established in adults, but not in children. We aimed to evaluate the safety profile of this intracameral mydriatic in children. Methods: We conducted a prospective observational cohort study on 40 patients, aged 8 weeks to 17 years, undergoing cataract surgery under general anesthesia (GA) and receiving 1 intracameral injection (0.1 mL) of the mydriatic. Thirteen patients underwent immediate sequential bilater

Ocular effects of endothelin-1 in the cat.

The effects of intravitreal and intracameral administration of endothelin-1 were studied in the anaesthetized cat. Intravitreal injection of 0.4 nmol endothelin-1 induced a 34 +/- 10% (n = 5; P less than 0.05) reduction in retinal blood flow in the experimental eye compared to the control eye 80 minutes after the injection. Blood flow in the ciliary body, iris and choroid remained unaffected by the injection of endothelin-1. Intracameral administration of endothelin-1 at pmol doses caused a reduction in pupil size, an increase in the aqueous humor protein concentration (indicating a break-down

Suprachoroidal hemorrhage postintracameral bevacizumab injection: An unusual case report and review of literature.

Suprachoroidal hemorrhage (SCH) is a dreaded complication of intraocular surgery. We report a case of a 65-year-old male with left eye primary angle-closure glaucoma and idiopathic elevated episcleral venous pressure with neovascularization of the iris. Due to uncontrolled intraocular pressure even after maximum medical therapy, trabeculectomy with mitomycin C was planned. Preoperative intracameral bevacizumab was given to avoid intraoperative bleeding. Postinjection, localized SCH was noted, which settled spontaneously on conservative management. A combination of ocular and systemic risk fact
