Surgical results and complications of trabeculectomy with intraoperative application of mitomycin C.

Trabeculectomy is reportedly less effective in aphakic eyes, secondary glaucoma, juvenile patients, and eyes with a history of unsuccessful glaucoma surgery. We evaluated the surgical effects and complications of trabeculectomy with the adjunctive use of mitomycin C, an antiproliferative drug, in a number of refractory cases. Forty patients (50 eyes) with refractory glaucoma were treated with trabeculectomy and the adjunctive use of mitomycin C and were followed postoperatively for at least 3 months. The mean follow-up period was 13.6 +/- 7.5 months (range: 3 to 31 months). In 43 eyes (86%), i

Risk factors for intraoperative and early postoperative complications in extracapsular cataract surgery.

The risk factors for intraoperative and early postoperative complications were evaluated in 351 consecutive cataract operations at the Oulu University Hospital in 1990. Bleeding into the anterior chamber during the operation was recorded in 8.6%, zonular rupture in 6.6%, posterior capsular rupture in 5.4% and vitreous loss in 3.2%, On account of the loss of capsular support an anterior chamber intraocular lens was implanted in 3.1%. Of the early postoperative complications, signs of fibrinous reaction were observed in 24.5%, corneal edema in 53.6% and rise of intraocular pressure to 30 mmHg or

Intraoperative neuromonitoring use in posterior endoscopic cervical decompression: can it reduce complications?

PURPOSE: This study aimed to address the challenges and explore potential solutions for improving IONM utilization. Posterior endoscopic cervical decompression (PECD) relieves pressure on the exiting nerve root and spinal cord, potentially leading to improvements in patient symptoms. The routine use of intraoperative neuromonitoring (IONM) currently remains debatable, despite its ability to provide additional real-time neurological observation beyond standard practice. METHODS: In this retrospective cohort study, we investigated the clinical data of 140 patients with single-segment cervical s

Preoperative prediction of intraoperative complications in cataract surgery: a machine learning approach.

PURPOSE: Intraoperative complications during cataract surgery, while uncommon, can significantly impact visual outcomes and identifying high-risk patients preoperatively improves surgical planning and supports personalized care. This study presents an early application of advanced machine learning (ML) models for predicting multiple intraoperative complications using routine preoperative data. METHODS: We analyzed 3,957 cataract surgeries performed at a tertiary hospital and developed four ML models, XGBoost, Random Forest, CatBoost and TabPFN, to classify cases into three categories: no comp

Analytical study on intraoperative temperature management for reducing complications in cardiac surgery.

Intraoperative temperature control affects postoperative problems following heart surgery. Hence, a group of 128 adult patients who were having elective heart surgery were observed. According to normothermic and hypothermic temperature control techniques, patients were divided into groups. The normothermic group experienced noticeably fewer problems, such as bleeding and arrhythmias, according to the results. Thus, we show critical accurate heat management is to better surgical outcomes.

The association between intraoperative cardiopulmonary bypass power and complications after cardiac surgery.

BACKGROUND: Low cardiac power (product of flow and pressure) has been shown to be associated with mortality in patients with cardiogenic shock after acute myocardial infarction, but has not been studied in cardiac surgical patients. This study's hypothesis was that cardiac power during cardiopulmonary bypass for cardiac surgery would have a greater association with adverse events than either flow or MAP (mean arterial pressure) alone. METHODS: We undertook a retrospective observational study using patient data from February 2015 to March 2022 undergoing cardiac surgery at Fiona Stanley Hospit

Association of Intraoperative Ventilator Management With Postoperative Oxygenation, Pulmonary Complications, and Mortality.

BACKGROUND: "Lung-protective ventilation" describes a ventilation strategy involving low tidal volumes (VTs) and/or low driving pressure/plateau pressure and has been associated with improved outcomes after mechanical ventilation. We evaluated the association between intraoperative ventilation parameters (including positive end-expiratory pressure [PEEP], driving pressure, and VT) and 3 postoperative outcomes: (1) PaO2/fractional inspired oxygen tension (FIO2), (2) postoperative pulmonary complications, and (3) 30-day mortality. METHODS: We retrospectively analyzed adult patients who underwen

Review of unusual intraoperative and postoperative complications associated with endosseous implant placement.

OBJECTIVES: Common complications associated with dental implant surgery are well recognized and are usually explained to patients during the process of informed consent. For the general dental practitioner, the periodontist, and the oral and maxillofacial surgeon, it is relevant to also be familiar with less frequent complications. This review gathers unusual complications of this surgical procedure and presents unique complications from single case reports. METHOD AND MATERIALS: Studies were located using systematic searches in Medline and the Cochrane Library electronic databases. Key words

Intraoperative use of dextran is associated with cardiac complications after carotid endarterectomy.

OBJECTIVE: Although dextran has been theorized to diminish the risk of stroke associated with carotid endarterectomy (CEA), variation exists in its use. We evaluated outcomes of dextran use in patients undergoing CEA to clarify its utility. METHODS: We studied all primary CEAs performed by 89 surgeons within the Vascular Study Group of New England database (2003-2010). Patients were stratified by intraoperative dextran use. Outcomes included perioperative death, stroke, myocardial infarction (MI), and congestive heart failure (CHF). Group and propensity score matching was performed for risk-a

Structured intraoperative assessment of pancreatic gland characteristics in predicting complications after pancreaticoduodenectomy.

BACKGROUND: The morbidity rate after pancreaticoduodenectomy remains high (20-50 per cent) and postoperative pancreatic fistula (POPF) is a major underlying factor. POPF has been reported to be associated with pancreatic consistency (PC) and pancreatic duct diameter (PDD). The aim was to quantify the risk of pancreaticojejunostomy-associated morbidity (PJAM) by means of a structured intraoperative assessment of both characteristics. METHODS: This single-centre prospective observational study included pancreaticoduodenectomies performed between 2008 and 2010 with a standardized duct-to-mucosa

A short tapered stem reduces intraoperative complications in primary total hip arthroplasty.

BACKGROUND: While short-stem design is not a new concept, interest has surged with increasing utilization of less invasive techniques. Short stems are easier to insert through small incisions. Reliable long-term results including functional improvement, pain relief, and implant survival have been reported with standard tapered stems, but will a short taper perform as well? QUESTIONS/PURPOSES: We compared short, flat-wedge, tapered, broach-only femoral stems to standard-length, double-tapered, ream and broach femoral stems in terms of intraoperative complications, short-term survivorship, and

[Intraoperative complications during cementation of hip arthroplasty. Avoiding air embolization during cementation].

Circulatory collapse is known to occur during cementation of hip arthroplasty. The pathogenesis is multifarious, but the most important cause is believed to be formation of air embolism during insertion and cementation of the femoral component followed by air embolism in the heart. Inserting the cement retrogradely with a cement gun following distal plugging of the femoral cavity with spongiosa and using a plastic catheter for suction drainage reduces the air embolisation. Cement producing as little heat as possible during the setting should be used. Maintaining anaesthesia with N2O air emboli
