West Indies Glaucoma Laser Study (WIGLS) 3. Anterior Chamber Inflammation Following Selective Laser Trabeculoplasty in Afro-Caribbeans with Open-angle Glaucoma.

PRECIS: In the absence of prophylactic anti-inflammatory therapy, anterior chamber inflammation (characterized by cells and less commonly flare) is minimal and transient after selective laser trabeculoplasty in Afro-Caribbean glaucoma patients. PURPOSE: The purpose of this study was to characterize the prevalence, severity, and duration of anterior chamber inflammation (cells and flare) following selective laser trabeculoplasty (SLT) in Afro-Caribbean eyes with primary open-angle glaucoma (POAG). METHODS: In total 144 eyes of 72 POAG patients underwent first-time 360-degree SLT treatment fol

The prevalence of extraintestinal diseases in inflammatory bowel disease: a population-based study.

OBJECTIVE: The aim of this study was to determine the prevalence of the major extraintestinal manifestations of inflammatory bowel disease (IBD) and their relation to disease diagnosis and gender. METHODS: We used the population-based University of Manitoba IBD Database, which includes longitudinal files on all subjects of all health system contacts identified by International Classification of Diseases, 9th Revision, Clinical Modification codes for visit diagnosis. We extracted a cohort from our database, which included subjects with a known diagnosis of IBD for at least 10 yr. We then deter

Iris metastasis preceding diagnosis of gastric signet ring cell adenocarcinoma: a case report.

BACKGROUND: A case of iris metastasis preceding the diagnosis of gastric signet ring cell adenocarcinoma is very rare. To report the findings in a patient who presented with an iris tumor that was later identified to have metastasized from a gastric signet ring cell adenocarcinoma. CASE PRESENTATION: A-74-year-old woman presented with visual disturbance and an increased intraocular pressure (IOP) in the right eye. She had no history of systemic cancer. She was initially diagnosed with acute iritis from diabetes mellitus and secondary glaucoma. She underwent trabeculectomy because of the uncon

Ocular involvement in acute febrile neutrophilic dermatosis (Sweet syndrome): new cases and review of the literature.

Sweet syndrome (acute febrile neutrophilic dermatosis) is a dermatologic disorder with accompanying features of systemic inflammation. It is commonly associated with conjunctivitis, but a variety of types of ocular inflammation have been reported. The ocular manifestations of Sweet syndrome include periorbital and orbital inflammation, dacryoadenitis, conjunctivitis, episcleritis, scleritis, limbal nodules, peripheral ulcerative keratitis, iritis, glaucoma, and choroiditis. The ocular inflammation appears concurrently with skin lesions. An overview of Sweet syndrome is presented with a review

Efficacy of polyacrylamide vs. sodium hyaluronate in cataract surgery.

A prospective randomized study was conducted in 137 patients who underwent cataract surgery and implantation of a polymethylmethacrylate intraocular lens with either polyacrylamide (Orcolon) (68 patients) or sodium hyaluronate (Healon) (69 patients) as the viscoelastic material. Both viscoelastics were aspirated at the end of surgery. Patients were examined before surgery and 1 and 14 days after surgery. Endothelial cell counts were obtained before surgery and 3 months after surgery in 34 patients (16 in the polyacrylamide group and 18 in the sodium hyaluronate group). There were no reports of

[Ocular localization appearing in a child with acute lymphoblastosis after hematological remission for 18 months].

Case report of a 4-year-old girl in whom a unilateral ocular localization appeared after 1 1/2 year of maintained remission of acute lymphoblastic leukaemia. The evolution of this localization can be divided in three phases: 1. During the first 17 months, an apparent simple relapsing conjunctivitis of the left eye. 2. During the following 5 months, repeated episodes of the left iritis with mydriasis and discolouration of the iris. 3. Finally, left uveitis involving the anterior chamber, with hypopion, hyphaema and glaucoma; the clinical presentation at this stage was suggestive of ocular lymph

Pyoderma gangrenosum complicating Crohn's disease.

Of the many acknowledged systemic complications of inflammatory bowel disease, arthritis, iritis and erythema nodosum are observed most commonly and considered parts of the natural history. Pyoderma gangrenosum is a more ominous, less common but similarly associated complication that classically occurs in the course of ulcerative colitis. Its rarity in Crohn's disease stimulated the following report.

Managing the red eye.

The commonest causes of a red eye, excluding trauma, are conjunctivitis, allergies and lid conditions. These can usually be managed by the GP and are not serious if properly treated. Keratitis and corneal ulcers, acute iritis and acute glaucoma are uncommon conditions but are always serious and require immediate referral to an ophthalmologist for treatment.

A novel autoantibody to the putative oncoprotein DEK in pauciarticular onset juvenile rheumatoid arthritis.

OBJECTIVE: To define the frequency of a novel autoantibody reactive with a 45 kDa protein in children with juvenile rheumatoid arthritis (JRA). This protein is expressed by the putative oncogene DEK associated with a subtype of acute myeloid leukemia. METHODS: The sera of 158 children with JRA were analyzed for the presence of anti-DEK by immunoblotting using purified DEK protein and compared with sera of 109 children with other rheumatic diseases and 25 healthy controls with no connective tissue disease. RESULTS: Antibodies to DEK were found significantly more frequently among children with

An ocular model of adenovirus type 5 infection in the NZ rabbit.

Ocular adenoviral infections occur worldwide, and currently, there is no ocular animal model for evaluating new antivirals or studying pathogenesis. With a paired-eye design, an ocular model was developed in 32 New Zealand rabbits following topical and intrastromal inoculation with a clinical isolate of adenovirus type 5 (Ad5 McEwen). Clinical signs of infection--conjunctivitis, corneal edema, subepithelial infiltrates, and iritis--and seroconversion were evaluated. Replicating virus on the ocular surface was determined by serial ocular titers. Reproducible acute ocular infection was demonstra

Acute meningoencephalomyelitis in a rabbit infected with Pasteurella multocida.

Pasteurella multocida was isolated in pure culture from the optic chiasm of a rabbit that was euthanatized subsequent to acute development of neurological signs. Histopathologically, there was meningoencephalomyelitis, bilateral otitis interna, retrobulbar cellulitis, optic neuritis and iritis. The ocular involvement, severity of the spinal lesions and the lack of otitis media was unusual.

Ocular lesions associated with coccidioidomycosis in dogs: 35 cases (1980-1985).

Thirty-five dogs with ocular lesions associated with coccidioidomycosis were examined. Serologic and/or histologic evaluation confirmed the diagnoses of coccidioidomycosis. The disease most frequently associated with signs referable to the anterior segment of the eye, such as iritis and granulomatous uveitis. Histologic evaluation of the eye revealed a primary posterior segment disease, such as chorioretinitis or retinal separation, with an extension into the anterior segment of the eye.

Ocular lesions caused by caterpillar hairs (ophthalmia nodosa).

The ocular reactions to caterpillar hairs are diverse in nature and location, ranging from a toxic reaction to the external foreign bodies, keratoconjunctivitis or the formation of conjunctival nodules, to intense iritis, vitritis or papillitis. Four cases are presented in this paper to illustrate the various degrees of ocular involvement. Included are clinical photographs of intravitreal and subretinal hairs. A classification of these reactions is suggested to facilitate the choice of treatment.

Acquired syphilitic uveitis.

Secondary syphilis is an overlooked cause of iritis and posterior uveitis. Three cases of acquired syphilitic uveitis were observed, two of which occurred in male homosexuals. In the first patient, a delay in diagnosis and treatment led to irreversible loss of vision and visual field. In the latter two cases, an increased index of suspicion led to earlier diagnosis and treatment, with complete recovery of vision.

Acute ocular hypotony as a manifestation of mumps. A case report.

A 27-year-old woman with epidemic parotitis developed acute hypotony in her right eye. This complication is not mentioned in earlier reports concerning this disease. She also suffered from keratitis and iritis. The inflammatory reaction of the anterior eye subsided on topical medication, but the hypotony persisted for two weeks. Factors possibly influencing the low intraocular tension are discussed.

Traumatic eye and intracranial air-movement from a subconjunctival to an intracranial position.

In a 12-year-old boy, air accidentally introduced subconjunctivally from the pointed tip of an air compressor hose, moved to an intracranial position over the sella turcica as demonstrated by x-ray films. The globe and a portion of the optic nerve were also outlined by air contrast. A minor corneal abrasion and traumatic iritis cleared promptly and no fractures or other damage occurred.
