Iron Fortification Practices and Implications for Iron Addition to Salt.

This introductory article provides an in-depth technical background for iron fortification, and thus introduces a series of articles in this supplement designed to present the current evidence on the fortification of salt with both iodine and iron, that is, double-fortified salt (DFS). This article reviews our current knowledge of the causes and consequences of iron deficiency and anemia and then, with the aim of assisting the comparison between DFS and other common iron-fortified staple foods, discusses the factors influencing the efficacy of iron-fortified foods. This includes the dietary an

The role of cellular iron deficiency in controlling iron export.

BACKGROUND: Iron export via the transport protein ferroportin (Fpn) plays a critical role in the regulation of dietary iron absorption and iron recycling in macrophages. Fpn plasma membrane expression is controlled by the hepatic iron-regulated hormone hepcidin in response to high iron availability and inflammation. Hepcidin binds to the central cavity of the Fpn transporter to block iron export either directly or by inducing Fpn internalization and lysosomal degradation. Here, we investigated whether iron deficiency affects Fpn protein turnover. METHODS: We ectopically expressed Fpn in HeLa

Iron Sucrose: A Wealth of Experience in Treating Iron Deficiency.

Iron deficiency and iron-deficiency anemia are associated with increased morbidity and mortality in a wide range of conditions. In many patient populations, this can be treated effectively with oral iron supplementation; but in patients who are unable to take or who do not respond to oral iron therapy, intravenous iron administration is recommended. Furthermore, in certain conditions, such as end-stage kidney disease, chronic heart failure, and inflammatory bowel disease, intravenous iron administration has become first-line treatment. One of the first available intravenous iron preparations i

Intravenous iron: a framework for changing the management of iron deficiency.

For decades intravenous iron was considered dangerous. Newer formulations with carbohydrate cores binding elemental iron more tightly allow complete iron replacement within 15-60 min in one visit. Meta-analyses and prospective comparisons of different formulations support equivalent safety to placebo with less toxicity than oral iron. Of the available formulations, the preponderance of published evidence supports equal safety and efficacy. In this Viewpoint, we report evidence supporting repositioning of intravenous iron to the frontline in multiple disorders with iron deficiency, which includ

The Role of Iron and Iron Overload in Chronic Liver Disease.

The liver plays a major role in iron homeostasis; thus, in patients with chronic liver disease, iron regulation may be disturbed. Higher iron levels are present not only in patients with hereditary hemochromatosis, but also in those with alcoholic liver disease, nonalcoholic fatty liver disease, and hepatitis C viral infection. Chronic liver disease decreases the synthetic functions of the liver, including the production of hepcidin, a key protein in iron metabolism. Lower levels of hepcidin result in iron overload, which leads to iron deposits in the liver and higher levels of non-transferrin

Iron metabolism in the pathogenesis of iron-induced kidney injury.

In the past 8 years, there has been renewed interest in the role of iron in both acute kidney injury (AKI) and chronic kidney disease (CKD). In patients with kidney diseases, renal tubules are exposed to a high concentration of iron owing to increased glomerular filtration of iron and iron-containing proteins, including haemoglobin, transferrin and neutrophil gelatinase-associated lipocalin (NGAL). Levels of intracellular catalytic iron may increase when glomerular and renal tubular cells are injured. Reducing the excessive luminal or intracellular levels of iron in the kidney could be a promi

Siderophore-mediated iron trafficking in humans is regulated by iron.

Siderophores are best known as small iron binding molecules that facilitate microbial iron transport. In our previous study we identified a siderophore-like molecule in mammalian cells and found that its biogenesis is evolutionarily conserved. A member of the short chain dehydrogenase family of reductases, 3-hydroxy butyrate dehydrogenase (BDH2) catalyzes a rate-limiting step in the biogenesis of the mammalian siderophore. We have shown that depletion of the mammalian siderophore by inhibiting expression of bdh2 results in abnormal accumulation of cellular iron and mitochondrial iron deficienc

[The iron letter--an update on the treatment of iron deficiency anemia].

Iron deficiency is a frequent complication in chronically ill patients and in pregnant women. Iron status can now be characterised precisely and relatively easily by determining serum ferritin, transferritin saturation and if necessary hypochromic erythrocytes and the haemoglobin content of erythrocytes (CHr). Oral iron replacement is usually restricted by limited absorption and low tolerability. Intravenous iron therapy is possible in such cases and can be combined with rHuEPO (e.g. EPREX/Epoetin alfa) in severe cases. Iron saccharate Switzerland and this permits high dose iron replacement wi

Transgenic rice is a source of iron for iron-depleted rats.

Iron deficiency is one of the most prevalent nutrient deficiencies in the world. A sustainable solution to dietary iron deficiency may be achieved in part by increasing bioavailable iron in seeds used for foods such as rice. Because ferritin is used as a natural source of iron in the early development of humans, other animals and plants, the bioavailability of iron in rice seeds, provided in amounts equal to ferrous sulfate, and in transgenic rice with ferritin was tested in iron-deficient rats. A standard hemoglobin (Hb) repletion bioassay was used with rats made anemic followed by complete d

Ironing out disease: inherited disorders of iron homeostasis.

Iron presents us with a paradox. Without it, cells simply cannot survive because iron is an essential cofactor for many enzymes in critical biochemical pathways. However, when iron is present in excess, it can be highly cytotoxic due to its propensity to catalyze the formation of reactive oxygen radicals. To cater for this dual nature, cells and organisms have developed elaborate mechanisms for regulating iron intake and efflux. When these mechanisms are disrupted, as is the case in a number of inherited disorders of iron metabolism, the pathological consequences can be severe. Many of these d

Measurements of iron status and survival in African iron overload.

INTRODUCTION: Dietary iron overload is common in southern Africa and there is a misconception that the condition is benign. Early descriptions of the condition relied on autopsy studies, and the use of indirect measurements of iron status to diagnose this form of iron overload has not been clarified. METHODS: The study involved 22 black subjects found to have iron overload on liver biopsy. Fourteen subjects presented to hospital with liver disease and were found to have iron overload on percutaneous liver biopsy. Eight subjects, drawn from a family study, underwent liver biopsy because of ele

Intravenous iron supplement for iron deficiency in cardiac transplant recipients (IronIC): A randomized clinical trial.

AIMS: Heart transplant recipients have reduced exercise capacity despite preserved graft function. The IronIC trial was designed to test the hypothesis that intravenous iron therapy would improve peak oxygen consumption in these patients. METHODS AND RESULTS: This randomized, placebo-controlled, double-blind trial was performed at our national center for heart transplantation. One hundred and 2 heart transplant recipients with a serum ferritin <100 µg/liter or 100 to 300 µg/liter, in combination with transferrin saturation of <20%, and hemoglobin level >100 g/liter were enrolled ≥1 year after

The Iron Chaperone PCBP2 Controls Systemic Iron Homeostasis by Regulating Iron Flux in Red Pulp Macrophages.

Systemic iron homeostasis critically depends on macrophages for the recycling of iron from senescent erythrocytes. The iron chaperone poly(rC)-binding protein 2 (PCBP2) is considered to direct intracellular iron trafficking. However, its specific role in macrophage-mediated iron redistribution remains unclear. We show that iron deficiency induces PCBP2 expression in macrophages of mice and humans. Mice lacking PCBP2 in macrophages develop features of iron-restricted erythropoiesis, including reduced bone marrow iron, altered peripheral erythrocytes, and suppressed hepcidin levels. Despite medu

Iron Salts, but Not Iron Oxide Nanoparticles, Drive High-Yield Production of Iron-Engineered Extracellular Vesicles.

Efficient production of iron-engineered extracellular vesicles (EVs) is of interest for their use as delivery systems in biotherapy. In this study, we investigated the role of iron in EV biogenesis and evaluated a scalable fluidic strategy for their mass production using turbulence stimulation in bioreactors. Murine mesenchymal stem cells were loaded with iron via ferric quinate or iron oxide nanoparticles (IONPs) and subsequently subjected to turbulence stimulation or serum starvation to induce EV secretion. Using elemental spectroscopy and cryogenic electron microscopy, we found that prolong

Oat protein nanofibril-iron hybrids offer a stable, high-absorption iron delivery platform for iron fortification.

Iron deficiency and anaemia affect two billion people globally. Iron fortification can help to treat anaemia, but most current fortificants are limited by low absorption and/or poor sensory properties. Here we introduce oat protein nanofibrils (OatNF) carrying ultrasmall iron nanoparticles that are engineered to carry iron in ferrous or ferric form. In a prospective cross-over stable-isotope absorption trial in young iron-deficient women (n = 52), OatNF reduced with sodium ascorbate carried mainly ferrous iron and showed high fractional absorption with water and with polyphenol-rich food, show

Salvianolic acid B influences the ferritin function in iron(II)/iron(III) transformation and iron chelation.

BACKGROUND: Ferritin is a cage-like iron storage protein and can regulate the iron balance of life. It can be developed as a new type of iron supplement, while its function may be influenced by certain food bioactive components. To evaluate the effects of the typical food biomolecules, such as phenolic acid, on the physicochemical property of ferritin are of great importance to clarify the ferritin function in maintaining iron balance. RESULTS: In this study, the interaction mechanism of ferritin with iron ions and salvianolic acid B (SalB) was investigated. The complex formed by SalB and iro

Regional brain iron, ferritin and transferrin concentrations during iron deficiency and iron repletion in developing rats.

Iron deficiency in young rats leads to a decrease in brain iron and ferritin concentrations, an increase in transferrin (Tf) concentration, and an increased rate of uptake of iron from the plasma pool. We conducted two experiments to determine whether brain iron, Tf and ferritin respond quickly to iron repletion and to determine whether brain regions respond heterogeneously. Weanling male Sprague-Dawley rats were fed an iron-deficient diet (<5 mg/kg Fe) for 2 wk followed by an iron-adequate diet (REPL group, 35 mg/kg Fe in Experiment 1 and 15 mg/kg Fe in Experiment 2) for 2 or 4 wks, respectiv
