Alternative approaches to pterygomaxillary separation.

Pterygoid plate fractures, resulting from the pterygomaxillary separation in a Le Fort I osteotomy, may be associated with untoward fractures that extend to the base of the skull and orbit and which can lead to rare but significant complications. Five alternative approaches to the pterygomaxillary dysjunction were studied in 50 fresh cadavers. The results of this study show that the use of a curved Obwegeser osteotome to achieve the pterygomaxillary dysjunction should be abandoned, as it leads to an unacceptably high incidence of high-level pterygoid plate fractures at, or near, the base of th

A 5-year perspective on a removable overdenture appliance for a patient with osteonecrosis of the jaw, a mandibular resection, and rampant caries.

A 78-year-old man with a history osteonecrosis of the jaw and 2 subsequent surgeries to repair a fractured mandible had a condition that contraindicates most dentoalveolar procedures to restore his failing dentition. A Snap-On-Smile overdenture restored his opportunity for natural speech, occlusal function, and esthetics.

[Posterior cantilevered all-ceramic resin-bonded fixed dental prosthesis: A minimally invasive solution for restoring posterior edentulous spaces illustrated by a chairside clinical case].

INTRODUCTION: While the anterior cantilevered resin-bonded fixed dental prosthesis (RBFDP) is now well established among orthodontists and general practitioners, the posterior cantilevered RBFDP has only more recently gained attention in the international literature. The aim of this article is to provide an overview of this therapeutic option and its rationale, based on an original chairside clinical case performed in a single session. MATERIALS AND METHOD: A clinical case involving a 20-year-old female patient is presented. The patient exhibited a missing mandibular molar due to extraction f

Esthetic and functional rehabilitation of a patient with nonsyndromic oligodontia: a case report from China.

STATEMENT OF THE PROBLEM: Oligodontia (severe partial anodontia) is a developmental dental anomaly. It is either an isolated trait or part of a syndrome. Oligodontia is characterized by the congenital absence of more than six permanent teeth except the third molars. Treatment often calls for facilitated surgical techniques, but less severe cases can be treated conventionally in a normally equipped dental office. CASE PRESENTATION: A case of oligodontia treated with prosthetic rehabilitation is presented in this article to remind clinicians of the anomaly. A medical history was taken and clini

Orthodontic therapy for the restorative patient. Part I: The biomechanic aspects.

The integration of interspecialty treatment before the restorative and prosthodontic efforts of the dentist can eliminate conditions that have the potential to jeopardize successful completion of the proposed treatment. Adult dentitions that have deteriorated because of neglect, fear, caries, extractions, or trauma can be restored to stability, health, function, and esthetics. In "Part I, The biomechanic aspects," the orthodontic paralleling of proposed abutment teeth displays premolar (buccolingual) and molar (mesiodistal) uprighting procedures that achieve improved force distribution to thes

What you don't know can hurt you (and your patients).

The goal of modern dentistry is to return patients to oral health in a predictable fashion. The partial and complete edentulous patient may be unable to recover normal function, aesthetics, comfort, or speech with a traditional removable prosthesis. The aesthetics of the edentulous patient are affected as a result of bone atrophy. Continued resorption leads to irreversible facial changes. An implant stimulates the bone and maintains its dimension in a manner similar to healthy natural teeth. As a result, the facial features are not compromised by lack of support. Implant prostheses often offer

Fractal analysis of dental periapical radiographs: A revised image processing method.

OBJECTIVE: To assess trabecular bone structure as calculated with fractal analysis by 2 binarization processes: White and Rudolph's original method (WR.o) and a revised version (WR.r). Fractal dimension (FD) values calculated with WR.r (FD.r) and a gray-scale-based method (FD.f) were also compared. FD, histogram parameters, and lacunarity were compared by dentate status, jaw location, and sex. STUDY DESIGN: Regions of interest from digital periapical radiographs were defined below the teeth roots and in the edentulous sites of 37 patients. Histograms were assessed for pixel values. Rehabilitation with removable dentures among the dentate population in Finland.

The information available on the rehabilitation with removable dentures among dentate subjects is contradictory. In the present study, the most common type of rehabilitation was a complete maxillary denture with or without a partial one. Nineteen per cent of men and 27% of women belonged to this category. Partial denture(s) without a complete one were worn by 11% of men and 15% of women. The odds ratio of having partial denture(s) was significantly higher among women, among people with a medium level of income, with a regular dental attendance pattern, and with a shorter distance to the neares

Sinus Elevation Through Transcrestal Window Approach and Delayed Implant Placement in 1- to 2-mm Residual Alveolar Bone: A Case Report.

Sinus augmentation with a lateral window technique and delayed implant placement are the traditional treatment schedules when the residual alveolar bone height in atrophic posterior maxilla is less than 5 mm. This article presents a transcrestal window approach (TWA) to replace a missing first molar in an atrophic posterior maxillary ridge with 1 to 2 mm of residual bone height (RBH). Transcrestal sinus augmentation and delayed implant placement were performed. The antral pseudocyst that was located in the nasal wall of the maxillary sinus disappeared in the cone-beam computer tomography image

Removable dental prosthesis splint. An occlusal device for nocturnal bruxing partial denture users.

Nocturnal bruxing is a parafunctional activity of the masticatory system that may create problems for removable dental prosthesis (RDP) users. Such problems may include root fractures, increased mobility of abutment teeth, excessive wear of resin denture teeth, minor connector bending, or denture base cracking. This clinical report presents an occlusal device fabricated for an RDP patient. The device used existing ERA attachments for added retention designed with the intended purpose of protecting the definitive fixed and RDP from damage due to nocturnal bruxing activity and providing for even
