In this report, a unique and bizarre case of complicated suicide is presented. The decedent was found dead in the basin of a porta-potty, wearing women's pantyhose, jewelry, and makeup. The initial investigation was suspect for homicide. This case report describes a 17-month-old child with ingestion of a button battery. The clinical presentation, diagnostic reasoning, national guidelines, hospital course, and follow-up are discussed. Between 1995 and 2015, an estimated 759,074 children aged < 6 years were seen in emergency departments for suspected or confirmed foreign body ingestions, representing a 93.3% increase in cases . The view generally held in the literature is that the frequent occurrence of concomitant nickel and cobalt sensitivity is due to simultaneous sensitization from suspenders, jewelry, etc. In a study population consisting of 168 unselected female twins representing a random sample of the general population, only a few were found with concomitant nickel and cobalt sensitivity, and then mainly among those with hand dermatitis. OBJECTIVE: To describe and evaluate an ear piercing and earring retention method for individuals with metal hypersensitivity.
SETTING: Private facial plastic surgery practice associated with a tertiary care medical center.
METHODS: Thirty-one patients with a history of hypersensitivity to metallic jewelry (62 ears) underwent earlobe piercing with an intravenous catheter.
RESULTS: None of the patients experienced an infection or hypersensitivity reaction. All patients were able to wear nickel-free earrings for short periods without using the shortened catheter.

Allergic contact dermatitis associated with pierced earrings typically presents as erythematous patches and plaques, with prominent spongiosis histologically. The most common inciting metal is nickel. Allergic contact granulomatous reactions to pierced earrings are rare, and are often more nodular in appearance and associated with metals other than nickel. We hereby report the first case of allergic contact granulomas associated with pierced earrings in a child.
