Ball-Bearing-Inspired Polyampholyte-Modified Microspheres as Bio-Lubricants Attenuate Osteoarthritis.

Osteoarthritis, a lubrication dysfunction related disorder in joint, is characterized by articular cartilage degradation and joint capsule inflammation. Enhancing joint lubrication, combined with anti-inflammatory therapy, is considered as an effective strategy for osteoarthritis treatment. Herein, based on the ball-bearing-inspired superlubricity and the mussel-inspired adhesion, a superlubricated microsphere, i.e., poly (dopamine methacrylamide-to-sulfobetaine methacrylate)-grafted microfluidic gelatin methacrylate sphere (MGS@DMA-SBMA), is developed by fabricating a monodisperse, size-unifo

Osteocapsular Arthroplasty of the Elbow: Surgical Technique.

INTRODUCTION: Arthroscopic osteocapsular arthroplasty of the elbow is a procedure involving three-dimensional reshaping of the bones, removal of any loose bodies, and capsulectomy to restore motion and function as well as to reduce or eliminate pain. STEP 1 GET IN AND ESTABLISH A VIEW: Visualize identifiable articular structures and confirm their anatomic orientation. STEP 2 CREATE A SPACE IN WHICH TO WORK: Remove debris and loose bodies, as well as excise the fat pad and perform a synovectomy as necessary, so that you can see clearly. STEP 3 BONE REMOVAL: Remove osteophytes and restore the

Unilateral Technique for Interposition Arthroplasty of the Elbow-A Technical Note.

Stiffness and pain secondary to end-stage dysfunctional osteoarthritis of the elbow can pose a therapeutic dilemma. Although total elbow arthroplasty is successful in older patients with low functional demand, alternatives to joint replacement must be found in the younger, more active individual. Interposition arthroplasty provides a salvage option for young, high-demand patients who hope to minimize functional restrictions of the affected extremity. The procedure traditionally involves release of both collateral ligaments and the joint capsule, resulting in a higher risk for postoperative ins

Intercondylar Fractures of the Distal Humerus in Patients Younger Than 20 Years: Capsulectomy Results.

Few reports in the literature have been dedicated to young patients with distal fractures of the humerus, and few have addressed subsequent indications to improve range of motion (ROM). This article is a retrospective review of the elbow flexion-extension ROM in 31 patients (age, 12-19 years) who had been treated with open reduction and internal fixation of intercondylar fractures of the distal humerus at a level I trauma center from 1991 through 2013. The ROM of patients who underwent capsulectomy was compared with that of those who did not. Main outcome measures were ROM in degrees and need

Intra-articular injection of triamcinolone acetonide releasing biomaterial microspheres inhibits pain and inflammation in an acute arthritis model.

Inflammation of the synovium and joint capsule is a main driver of pain in an osteoarthritic (OA) joint. Triamcinolone acetonide (TAA) is a classical corticosteroid that reduces synovitis and alleviates pain, albeit transiently. Biomaterial-based local TAA release may prolong the suppression of pain without the need for multiple injections. Polylactic-co-glycolic acid (PLGA) formulations of TAA prolong OA pain relief to a limited extent. A novel polyesteramide (PEA) microsphere platform allows for extended release in the OA joint for over 3 months. To evaluate their effect on pain and inflamma

Medium- and long-term functional behavior evaluations in an experimental focal ischemic stroke mouse model.

BACKGROUND: We describe a patient with tardy ulnar neuropathy and cubitus valgus deformity found to have an intracapsular ulnar nerve. METHODS: An 89-year-old woman presented with severe neuropathic pain in the ulnar digits of the hand, advanced degenerative arthritis of the elbow, and tardy ulnar nerve palsy. Her pain was exacerbated with elbow movement, particularly flexion. She had paralysis of ulnar nerve innervated muscles, hypersensitivity with absence of two-point discrimination in her ulnar 1-1/2 digits, and a fixed ulnar claw deformity. She also had a grossly unstable elbow. RESULTS

Extra-articular arthroscopic lateral elbow release.

We describe a unique extra-articular approach for arthroscopic lateral release for lateral epicondylitis. An arthroscopic extra-articular approach allows better direct visualization of diseased structures with a 30 degrees arthroscope and only requires a small hole in the joint capsule. The camera is placed into the joint through the middle anterolateral portal. The camera is then pulled back through a small rent in the capsule over the lateral radiocapitellar joint to provide an extra-articular view of the diseased structures. The shaver is then placed 1.5 cm proximal to the camera in a proxi

Repression of calcitonin gene-related peptide expression in trigeminal neurons by a Theobroma cacao extract.

ETHNOPHARMACOLOGICAL RELEVANCE: Cocoa bean preparations were first used by the ancient Maya and Aztec civilizations of South America to treat a variety of medical ailments involving the cardiovascular, gastrointestinal, and nervous systems. Diets rich in foods containing abundant polyphenols, as found in cocoa, underlie the protective effects reported in chronic inflammatory diseases. Release of calcitonin gene-related peptide (CGRP) from trigeminal nerves promotes inflammation in peripheral tissues and nociception. AIM OF THE STUDY: To determine whether a methanol extract of Theobroma cacao

Management of the Clenched Fist in Adult Patients with Upper Motor Neuron Syndrome.

The clenched fist deformity (CFD) in adult patients with upper motor neuron syndrome (UMNS), typically due to a brain or spinal cord injury, can develop from an imbalance of the extrinsic finger flexors relative to the extensors. This brings the fingers into a flexed posture across the metacarpophalangeal (MCP) and interphalangeal (IP) joints and places the fingertips close to or in contact with the palm. The intrinsic musculature of the hand also plays a contributing role to the flexion deformity across the MCP joints whose contribution to the deformity can be masked by the extrinsic flexors.

Hallux valgus correction with metatarsal osteotomy: effect of a lateral distal soft tissue procedure.

The purpose of this study was to compare the results of hallux valgus correction using a proximal metatarsal osteotomy with or without a lateral distal metatarsophalangeal soft tissue procedure. This procedure included release of the lateral joint capsule and transfer of the adductor tendon into the neck of the metatarsal. Two groups of patients were reviewed retrospectively. In the first group (17 patients and 25 feet), a proximal closing wedge osteotomy and an Akin osteotomy were performed. In the second group (22 patients and 31 feet), in addition to the osteotomies, the adductor tendon and

Complex foot deformities associated with soft-tissue scarring in children.

Two cases of deformities in scarred feet are presented. One case had an old, well healed forefoot amputation with severe equinovarus deformity, and the other had an equinus deformity following a burn injury 10 months prior. Both the cases were managed by primary release of the contracted joint capsules. The correction of the soft-tissue contractures was achieved by gradual distraction using the Ilizarov apparatus. The clinical presentation and surgical treatment of complex foot deformities, complicated by the presence of scar tissue, are presented. These cases illustrate the benefits of combin

ROS-Sensitive MgOSiO2 Nano Capsules for Effective Against Osteoarthritis in Rat Model.

INTRODUCTION: Magnesium oxide nanoparticles (MgO NPs) as magnesium ionophores have shown potential as a therapeutic strategy for osteoarthritis. However, the rapid absorption and clearance of MgO NPs in the joint cavity and the lack of a clear underlying mechanism may limit their therapeutic efficacy. METHODS: MgO@SiO2 nano capsules were synthesized as a controlled-release nanosystem to mitigate the rapid clearance and potential toxicity of MgO NPs. The physicochemical properties and surface charge of the nano capsules were examined through TEM, EDS, XRD and Zeta potential. The kinetics of na

Arthroscopic Posterior Capsular Releases for Recalcitrant Arthrofibrosis.

BACKGROUND: The origin of arthrofibrosis of the knee is multifactorial; however, it is more commonly seen in patients with arthritis, prolonged immobility, or a history of trauma or surgery to the knee. Arthrofibrosis that results in a flexion contracture of the knee is commonly attributable to scarring and shortening of the posterior joint capsule as seen in the setting of operative intervention, such as anterior cruciate ligament reconstruction (ACLR). INDICATIONS: Arthroscopic posterior capsular releases of the knee are indicated in patients with recalcitrant arthrofibrosis who have (1) fa

Eight-Year Neglected Pediatric Kaplan's MCP Dislocation Following Childhood Trauma Managed by Volar Open Reduction: A Case Report.

INTRODUCTION: Kaplan's dislocation is a rare complex metacarpophalangeal (MCP) joint dislocation in which soft-tissue interposition prevents closed reduction. Chronic neglected cases in children are extremely uncommon and present significant diagnostic and therapeutic challenges. CASE REPORT: We report the case of an 11-year-old left-hand-dominant boy who presented with deformity and restricted motion of the left index finger 8 years after childhood trauma. Radiographs demonstrated a chronic dorsal dislocation of the second MCP joint consistent with Kaplan's lesion. An initial attempt at clos
