Dislocation of the calcaneocuboid joint presenting as lateral instability of the ankle.

A 16-year-old professional female ballet student sustained a plantar flexion-inversion injury to her left ankle while dancing. Clinical examination and MRI suggested subluxation of the tibiotalar joint. However, accurate diagnosis was hampered by a transient palsy of the common peroneal nerve. It was subsequently established that she had also sustained a dislocation of her calcaneocuboid joint, a rare injury, which was successfully stabilised by using a hamstring graft. The presentation and management of this rare condition are discussed.

Stabilometry recordings in functional and mechanical instability of the ankle joint.

Functional instability, i.e. recurrent sprains or a feeling of giving way in the joint, is common after ankle sprain. Using stabilometry, an objective and quantitative method for studying postural control, we earlier showed that soccer players with pathological stabilometric results are at risk for ankle joint injury. In the present study we found that the ability to maintain postural equilibrium as demonstrated by stabilometry was reduced among players with functional instability, but was not affected by mechanical instability. The results indicate that coordination and postural control are i

Joint instability in patients with borderline developmental dysplasia of the hip.

BACKGROUND: The treatment strategy for developmental dysplasia of the hip is determined based on the lateral center-edge angle. Nonetheless, an evaluation of joint instability may be important in determining the treatment strategy. This study classified the displacement patterns of the femoral head center during hip abduction. METHODS: Ten patients with borderline developmental dysplasia of the hip, 10 patients with developmental dysplasia of the hip, and 10 patients with normal hips were analyzed. Image matching was performed using X-ray images of hip abduction with a three-dimensional hip m

Dynamic Horizontal Instability of the Acromioclavicular Joint: A Case Report.

CASE: A 19-year-old female patient with a history of shoulder trauma 6 years ago presented with dynamic horizontal instability of the acromioclavicular joint (ACJ). She was treated with open ACJ reconstruction using gracilis allograft and showed a satisfactory clinical result at 1-year follow-up. CONCLUSION: Dynamic pure horizontal instability of the ACJ is a rare entity with only 6 cases reported in the literature. Till now, all reported patients who necessitated surgical treatment failed because of residual instability and/or pain. We present the seventh case of this type with a successful

[Instabilities of the Thumb Carpometacarpal Joint: Our Surgical Outcomes].

PURPOSE OF THE STUDY Carpometacarpal (CMC) instabilities of the thumb joint occur after injuries or due to joint overload in patients with congenital joint hypermobility. They are often undiagnosed and, if left untreated, are the basis for the development of rhizarthrosis in young individuals. The authors present the results of the Eaton-Littler technique. MATERIAL AND METHODS The authors present a set of 53 CMC joints of patients with an average age of 26.8 years (15-43 years) operated on in the years 2005-2017. Post-traumatic conditions were found in 10 patients and in 43 cases instability w

Treatment of Traumatic Elbow Instability With an Internal Joint Stabilizer.

PURPOSE: Current options for treating elbow instability include bony and/or ligamentous fixation with orthosis or cast immobilization, transarticular cross-pinning, temporary bridge plating, and hinged or rigid external fixation. Our purpose was to evaluate the recently developed internal joint stabilizer (IJS), which acts as an internal external fixator of the elbow. Our primary end point was to assess whether use of the device results in a stable and congruent reduction of the ulnohumeral and radiocapitellar joints in patients with acute or chronic elbow instability as a result of trauma. In

Dorsoradial Instability of the Thumb Metacarpophalangeal Joint: A Biomechanical Investigation.

PURPOSE: To define the role of the dorsal capsule and associated dorsal fibrocartilage (DFC) and their interactions with the radial collateral ligament (RCL) as a thumb metacarpophalangeal (MCP) joint stabilizer. METHODS: Eight cadaveric thumbs were mounted onto a custom jig with 20 N of muscle load applied. The thumb position in space was digitized to measure ulnar-radial, pronation-supination, and volar-dorsal laxity at 0°, 30°, and 60° MCP joint flexion. Serial sectioning was performed and measurements were repeated for the intact state, proper RCL insufficiency, proper and accessory (comp

A Technique for Treating Dorsal Instability of the Distal Radioulnar Joint.

We describe a technique for the treatment of chronic dorsal instability of the distal radioulnar joint by creating a strong dorsal radioulnar restraint using opposing flaps from the extensor retinaculum and dorsal capsule of the radioulnar joint. This technique has been used in 18 patients and has proven to be very easy and reliable with all patients demonstrating good stability of the distal radioulnar joint at final follow-up.

[Physiotherapeutic and training therapeutic options for instable patellofemoral joints].

In order to allow an adequate and timely restoration of physical performance with an instable patellofemoral joint within the framework of complex therapy strategies, individual post-traumatic cause-and-effect chains and their influence on the hip-leg axis of both the active and passive musculoskeletal systems must be considered as well as joint-specific aspects. For this reason selected relevant neurophysiological aspects of one and multiple joint movement stereotypes will be firstly presented. Subsequently some selected physiotherapeutic measures will be presented and their indication-specif

Severity of scapholunate instability is related to joint anatomy and congruency.

PURPOSE: To determine whether the bony architecture of the distal radius and proximal scaphoid have a role in stabilizing the scaphoid, and to determine whether a relationship between the bony geometry measurements and the amount of wrist constraint could be determined. METHODS: Eight cadaver wrists were tested in a wrist joint motion simulator. The level of scapholunate instability after sectioning the scapholunate interosseous, radioscaphocapitate, and the scaphotrapezium ligaments was determined and related to radiographic measurements of volar tilt, lateral tilt (ulnar tilt of the radiosc

Posterolateral dislocation of the elbow joint. Relationship to medial instability.

BACKGROUND: Dislocation of the elbow joint is the second most common dislocation in the upper extremity, dislocation of the shoulder being the most common. It has been reported that uncomplicated dislocation of the elbow joint may be associated with a decreased range of motion, degenerative changes in the elbow joint, ectopic calcification, or neurological deficits. As the medial collateral ligament complex can be completely disrupted during dislocation, we evaluated the association between the long-term results of treatment of simple posterolateral dislocation of the elbow and the presence of

Radial instability of the metacarpophalangeal joint of the thumb. A biomechanical investigation.

Human cadaver thumbs were tested to evaluate stability of the radial side of the MP joint. The contributions of the dorsal capsule, radial collateral ligament, accessory collateral ligament, and volar plate were examined with the joint in 0 degrees and 30 degrees of flexion. At 0 degrees flexion, the average joint angulation increased 4 degrees following isolated radial collateral ligament transection and 6 degrees following isolated accessory collateral ligament transection. Release of both the accessory and radial collateral ligaments produced marked instability with joint angulation of at l

[The value of dynamic examination methods in instability of the shoulder joint].

We prospectively examined 25 patients with shoulder instability by clinical examination and by fluoroscopic and sonographic imaging. 14 patients were classified as recurrent, posttraumatic instabilities, 12 of these anterior and two posterior. Clinical examination revealed the direction of instability in 5 patients, fluoroscopy in 5 and sonography in 3 patients. 11 patients had non-traumatic recurrent instabilities. Of these, clinical examination and fluoroscopy demonstrated identical directions of laxity in 8. Sonography concurred with the other techniques in only three cases. We recommend ma

Effects of radioulnar instability on the radiocarpal joint: a biomechanical study.

Five fresh cadaver upper extremities were studied with use of a static positioning frame, pressure-sensitive film and a microcomputer-based videodigitizing system to assess the effect of increasing radioulnar instability on the load distribution within the proximal carpal joint. Three stages of radioulnar instability were studied: (1) an avulsion fracture at the base of the ulna styloid; (2) an avulsion fracture at the base of the ulna styloid plus disruption of the dorsal portion of the distal radioulnar joint capsule; and (3) an avulsion fracture at the base of the ulna styloid, disruption o

Chronic Instability and Pain of the Sternoclavicular Joint: Treatment With Semitendinosus Allograft to Restore Joint Stability.

Chronic instability of the sternoclavicular (SC) joint is a challenging clinical problem, particularly in a patient population for which nonoperative forms of treatment prove ineffective. Patients present after experiencing recurrent subluxation events and subsequent pain, which commonly result in increasing functional limitation. Recurrent SC joint instability of this nature can lead to damage of the SC joint cartilage and bone, and in cases of posterior subluxation or dislocation, damage to mediastinal structures. While the precise treatment algorithm requires tailoring to individual patient

Mild exercise expedites joint clearance and slows joint degradation in a joint instability model of osteoarthritis in male rats.

OBJECTIVE: Exercise remains a hallmark treatment for post-traumatic osteoarthritis (PTOA) and may maintain joint homeostasis in part by clearing inflammatory cytokines, cells, and particles. It remains largely unknown whether exercise-induced joint clearance can provide therapeutic relief of PTOA. In this study, we hypothesized that exercise could slow the progression of preclinical PTOA in part by enhancing knee joint clearance. DESIGN: Surgical medial meniscal transection was used to induce PTOA in 3-month-old male Lewis rats. A sham surgery was used as a control. Mild treadmill walking was
