Laser-Assisted In Situ Keratomileusis in Flat, Normal, and Steep Corneas.

PURPOSE: To compare laser-assisted in situ keratomileusis (LASIK) outcomes in patients with different corneal curvatures. METHODS: This is a retrospective comparative chart review. Patients included in this study underwent wavefront-optimized myopic LASIK between January 2013 and December 2022 at Care-Vision Laser Center, Tel-Aviv, Israel. Patients were divided into 3 groups based on the steepness of the cornea (steep keratometry) (flat: <42 diopters [D], normal: 42-46 D, and steep: >46 D). Case-by-case matching was performed to verify that baseline parameters (age, preoperative visual acuity

Deep anterior lamellar keratoplasty in post-laser in situ keratomileusis keratectasia.

We describe a technique using deep anterior lamellar keratoplasty (DALK) in 5 eyes that developed keratectasia after LASIK. The technique is based on surgical manipulation that allows visualization of the lamellar dissection depth using a posterior approach to reach the predescemetic space. The mirror effect, indentation effect, and folding effect were used to determine proximity to Descemet's membrane. The same diameter donor and recipient buttons were used to correct myopia. The donor button without Descemet's membrane was placed using 10-0 nylon sutures. No intraoperative or postoperative c

Epithelial ingrowth after late traumatic femtosecond laser-assisted laser in situ keratomileusis flap dislocation.

Traumatic flap displacement and epithelial ingrowth after microkeratome-assisted laser in situ keratomileusis (LASIK) is a widely described potentially sight-threatening complication. However, this complication is rare when a femtosecond laser is used to create the LASIK flap. We present a case of epithelial ingrowth after late traumatic femtosecond-assisted LASIK flap dislocation. A 25-year-old woman presented to our emergency department after being scratched in the right eye by her cat. The patient had bilateral myopic femtosecond-assisted LASIK surgery 5 years previously with good postopera

Laser in-situ keratomileusis infection: review and update of current practices.

PURPOSE OF REVIEW: To review new clinically relevant data regarding the prevention cause and treatment of laser in-situ keratomileusis (LASIK) infections. RECENT FINDINGS: Many recent studies of post-LASIK infectious keratitis show the predominance of atypical mycobacteria and Gram-positive cocci and the growing number of other rare pathogens. The American Society of Cataract and Refractive Surgery White Paper treatment paradigm remains the model for initial treatment of LASIK-associated infectious keratitis. SUMMARY: Improved understanding of the risk factor, different causes, along with a

Post-laser in-situ keratomileusis ectasia: current understanding and future directions.

PURPOSE OF REVIEW: The aim of this article is to review the causes, risk factors, management, and future research directions for corneal ectasia after laser in situ keratomileusis. RECENT FINDINGS: Complex corneal biomechanical processes influence the integrity of the normal and postoperative cornea, and developing an understanding of these processes facilitates recognition of risk factors for ectasia after laser in-situ keratomileusis. Currently identified risk factors include keratoconus, high myopia, low residual stromal bed thickness from excessive ablation or thick flap creation, and def

Late-onset idiopathic diffuse lamellar keratitis after laser in situ keratomileusis.

We report a case of late-onset diffuse lamellar keratitis (DLK) in the left eye of a 56-year-old white woman 3 years after uneventful bilateral laser in situ keratomileusis (LASIK). Slitlamp examination revealed stage 3 DLK with diffuse, multifocal, dot-like, and granular haze in the interface. The DLK did not have an obvious causative agent such as trauma or epithelial defects and responded rapidly to topical corticosteroid therapy. Surgery was performed to relift the flap, remove cellular infiltrates, and obtain cultures. The microbiology culture was negative. This case indicates that DLK ca

Ulcerative keratitis caused by Serratia marcescens after laser in situ keratomileusis.

We report 2 cases of severe corneal infections caused by Serratia marcescens after laser in situ keratomileusis (LASIK). Twenty-four hours after LASIK, 2 patients developed infectious keratitis, 1 bilaterally. In each eye, the corneal flap was edematous, ulcerated, and detached from the stromal bed. Treatment included removal of the necrotic flap and aggressive antibiotic therapy. Cultures from corneal exudates were positive for S marcescens. After 1 year, both patients had a loss of best corrected visual acuity (BCVA) ranging from 20/40 to 20/22 because of irregular astigmatism. Overrefractio

Curvularia keratitis after laser in situ keratomileusis from a feline source.

Fungal keratitis following laser in situ keratomileusis (LASIK) is extremely rare. Except in cases that follow trauma, the source of fungal organisms is unknown in published reports. We report a case of keratitis following LASIK that could be directly traced to a fungal skin infection of the patient's pet cat.
