Inferior pole collecting system anatomy: its probable role in extracorporeal shock wave lithotripsy.

In addition to the gravity-dependent position, we believe that other particular anatomical features may be important in the retention of stone debris in the lower calices after extracorporeal shock wave lithotripsy (ESWL). We analyzed the inferior pole collecting system anatomy in 146, 3-dimensional polyester resin corrosion endocasts of the pelviocaliceal system. The inferior pole was drained by multiple calices disposed in 2 rows in 56.8% of the cases and by 1 midline caliceal infundibulum in 43.2%. In 60.3% of the cases there was a lower infundibulum equal to or greater than 4 mm. in diamet

[Flow technic for analysis of the pyelocaliceal system with regard to the genesis of urinary calculi. III. Studies of dynamic models].

Experiments with rubber models of the renal pelvis support the supposition that the state of flow in the pelvi-calyceal system influences the formation of urinary calculi.

[Prenatal diagnosis of malformative uropathies: multicentric retrospective study and a proposed feto-neonatal diagnostic protocol].

We have wanted to appraise the diagnostic reliability of fetal ultrasonography (FU) and its correlation with postnatal ultrasonography (PU). For two years we have studied 20.506 children born in 12 Pediatric Division of Veneto, all provided with FU. Between the 23rd and the 40th week of gestation the FU has individualized 177 fetuses with pelvic dilatation. The PU, performed in the 2nd and 7th day of life and then repeated to 1 month of life, has confirmed the prenatal diagnosis in 150 of the 247 renal unities, we have observed the 39% of false positive and the 8% of false negative. Polycystic disease of the renal sinus: structural characteristics.

Based on a series of 32 identified cases the radiologic, sonographic and anatomic features of polycystic disease of the renal sinus are described. This entity is a bilateral accumulation of thin-walled clear fluid-filled cysts, intertwined with the caliceal infundibula and usually confused radiologically with adult polycystic kidney disease or with renal sinus lipomatosis. Computerized tomography and sonography demonstrate the parenchyma to be free of cystic disease and the hilus to be free of fat accumulation. This newly recognized entity has a completely benign natural history.

Acute adult onset bacterial nephritis: long-term urographic and angiographic followup.

Most adults with acute bacterial renal infection demonstrate no roentgen abnormality in the acute stage and recuperate without significant functional or morphological renal abnormality. We report the initial and long-term findings in a virulent form of acute bacterial nephritis caused by gram-negative organisms and predominantly in patients with diabetes and associated septicemia. The findings of a small, poorly functioning symmetrically scarred kidney with significant caliceal distortion and the associated angiographic abnormalities are presented for the first time in the long-term followup o

[Laparoscopic management of horseshoe kidney stones.].

OBJECTIVE: Horseshoe kidney is the most common renal fusion anomaly. Its peculiar anatomical characteristics increase the risk of lithiasis formation and always entails a surgical challenge for its treatment. MATERIAL AND METHODS: We present the cases of men with horseshoe kidneys diagnosed by CT one case of alarge pyelic lithiasis with others of a smaller size in the calyces, and the other of a big solitary lithiasis in renal pelvis. RESULTS: We performed a laparoscopic pyelolitectomy to remove the pyelic lithiasis and we used a flexible cystoscope through a trocar to extract the calyceal st

Retrograde intrarenal surgery in lateral position for lower pole stone: an initial experience from Single Academic Hospital.

Retrograde intrarenal surgery (RIRS) was generally challenging in management of lower pole stone (LPS) since the unfavorable anatomy. Theoretically, LPS was prone to fall out and down to renal pelvis when patients turned to lateral position, thus to facilitate lithotripsy. The aim of the present study was to report our initial experience of RIRS in lateral position for LPS. From January 2020 to February 2021, 21 patients with LPS received RIRS in lateral position. The intraoperative finding, operation time, complications and stone-free rate (SFR) were recorded and analyzed. The mean stone size

Chronic Unilateral Hematuria: Compound Papillae Are Likely to Bleed.

Objectives: The present study investigated the recent outcomes of chronic unilateral hematuria (CUH), focusing on the detailed distribution of bleeding points based on the anatomical classification of the pelvicaliceal system. Materials and Methods: We evaluated 75 CUH patients treated in the past decade. Bleeding points were recorded separately as five levels of minor calices (Top, Upper, Middle, Lower, and Bottom). Bleeding point detection, immediate success, and recurrence-free rates were assessed. Results: The median age was 47 (range: 13-81) years. Bleeding points were detected in 62 pati

[The Endovision technique in renal percutaneous lithotripsy].

INTRODUCTION: In order to achieve a safer percutaneous access to the kidney, even if not systematically, it is possible to combine the use of eco-fluoro-guided puncture with the endoscopic retrograde vision through flexible ureteroscopy. Our experience has been conducted in order to standardize the technique and highlight advantages and limitations. MATERIALS AND METHODS: 26 patients (15 M-11 F), mean age 46 years, underwent flexible ureterorenoscopy as first percutaneous access for pyelic or pyelocaliceal stones.
20 cases were conducted in the prone and 6 in the supine position.

Efficacy and safety of papillary vs non-papillary access to the calyces in patients undergoing percutaneous lithotripsy: a GRADE-assessed systematic review and meta-analysis.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) access is traditionally obtained via papillary puncture to minimize bleeding, though non-papillary access is frequently utilized in everyday clinical practice due to practical considerations such as anatomical variations and stone location. Because existing comparative studies have yielded inconsistent findings, this systematic review and meta-analysis aimed to synthesize existing evidence evaluating the safety and efficacy of papillary versus non-papillary access during PCNL. METHODS: A comprehensive systematic search of databases, including Pu

Development and validation of a simple stone score to estimate the probability of residual stones prior to percutaneous nephrolithotomy.

BACKGROUND: The aim of the present study was to develop and internally validate a simple stone score (SSS) to estimate the probability of clinically significant residual fragments (CSRF) prior to percutaneous nephrolithotomy (PNL). METHODS: The files of 1170 PNL procedures between January and December 2015 were evaluated. CT-derived stone characteristics were examined. Caliceal stone distribution (CSD) was assigned three grades based on the number of calices involved regardless of the renal pelvis (I = no or single calix; II = more than one calix; and III = more than 2 calices or complete sta

On 50 cases of the dissecting material of the Department of Anatomy and Histology were worked out the kidney's vessels, the basin and the ureters, as the varieties in number and situation of the vessels, the cases of nephric ectopy and comparatively rare case of combined anomaly as well were traced and described. This anomaly is expressed in bilateral nephric ectopy, bilateral double ureter, which begins from independent calices set, and four pairs of polar kidney arteriae, which are situated bilateral as well. In the right they pass before v. cava inf. The vessels are equally calibrated, as t

Successful management of caliceal fistulas following renal transplantation.

Caliceal fistulas are unusual complications of renal transplantation. Segmental renal infarction and distal ureteral obstruction may be contributing factors in their formation. Two cases are reported, the first probably a result of segmental renal infarction from failure to recognize and anastomose an accessory renal artery, the second possibly related to blunt trauma to the area of the graft in an automobile accident and ureteral obstruction resulting from blood clots. Nephrostomy drainage through a healthy area of renal parenchyma was instrumental in the salvage of both kidneys.

[Echography in pyelocaliceal tumors].

In a study of over 32 pyelocaliceal tumors greater than 1 cm in diameter, the authors review retrospectively the reports of the pyelographic and sonographic documents obtained in these patients before the definitive diagnosis was confirmed by the pathological examination. They observed that pyelography has a much greater diagnostic accuracy than does sonography: 31 confirmed (27) or suspected (4) tumors whereas only one tumor was overlooked (silent kidney) on pyelography and 12 diagnosed tumors, 15 overlooked and 5 diagnostic errors on ultrasonography. However, it should be noted that the resu

Unicalyceal single kidney.

A rare congenital malformation of the kidneys, consisting of renal agenesis on the left and a small kidney on the right with only a single calyx, abnormal renal vasculature and impaired medullary function, is described. This was associated with long phocomelia of the right arm. The coexistence of these rare malformations has not been reported before.

A case report of renal calyceal diverticulum with hypertension in children and review of literature.

BACKGROUND: Renal calyx diverticulum refers to a cystic lesion covered with the transitional epithelium in the renal parenchyma. Although there is no clear evidence that calyx diverticulum can cause hypertension, there exists a close association between the two, and there are few related reports. Herein, we reported the case of a child with renal calyx diverticulum complicated with hypertension and summarized the diagnosis and treatment. CASE PRESENTATION: Physical examination of the patient, an 11-year-old child, revealed a left renal cyst with hypertension (155/116 mmHg). There were no rela

[Use of a coagulum for the ablation of renal calculi (author's transl)].

The authors report an experiment using the cadaver kidney with a fibrin coagulum injected into the pelvis and calices in order to extract multiple calculi. They used a mixture of cryoprecipitated plasma and a combination of thrombin and calcium chloride. This mixture was made up immediately before use, and injected into the pyelocaliceal cavities using a small Foley catheter previously inserted into the renal pelvis. This catheter was used to empty completely the pyelocaliceal cavities of urine immediately before injection. Extraction via a pyelotomy incision was used to withdraw the compact m
