Functional Outcomes of Knee Arthrodesis for Infected Total Knee Arthroplasty.

INTRODUCTION: As the occurrence of total knee arthroplasties (TKAs) is forecasted to continue rising, so too will the frequency of prosthetic joint infections (PJIs) and revision TKAs. Multiple revisions can result in an unreconstructible knee. In such instances, the knee may be salvaged through arthrodesis. We evaluated whether height, BMI, and age impacted patient-reported outcome measures (PROMs) in patients who underwent knee arthrodesis after revision TKA due to PJI. METHODS: We conducted a retrospective review of patients undergoing arthrodesis for an infected TKA at a dedicated orthope

[Open arthrolysis for stiff knee after primary total knee arthroplasty].

OBJECTIVE: To evaluate the effectiveness of open arthrolysis in treatment of the patient with stiff knee in late stage after primary total knee arthroplasty (TKA). METHODS: Between January 2014 and October 2017, 7 female patients (7 knees) with stiff knee in late stage after primary TKA were admitted. The patients were 57-71 years old (mean, 63 years). There were 2 left knees and 5 right knees. All patients underwent TKA because of knee osteoarthritis. The interval between TKA and open arthrolysis was 8-30 months (mean, 13.6 months). There were 4 patients with 10-20° of extension deficit befo

Haemophilic knee arthropathy: long-term outcome after total knee replacement.

PURPOSE: The objective of this study was to evaluate the long-term outcome and prosthetic survival of primary total knee arthroplasty in haemophilic patients. It was hypothesized that the infection and revision rate are higher and the outcome inferior when compared with patients without haemophilia. METHODS: Between 1985 and 2004, forty-three consecutive primary total knee replacements were performed in thirty haemophilic patients. These patients' charts were reviewed retrospectively. Twenty-five patients (34 knees) were available for clinical and radiological follow-up. The outcome was asses

Revision of failed unicompartmental knee replacement to total knee replacement.

Recent reports suggest good outcome results following unicompartmental knee replacement (UKR). However, a number of authors have commented on the problem of osseous defects requiring technically difficult revision surgery. We reviewed clinical outcomes following revision total knee replacement (TKR) for failed UKR and analysed the reasons for failure and the technical aspects of the revision surgery. Between 2001 and 2010 our institute performed 132 UKR's out of which 33 required revision to TKR during a period 6 years. Demographics, details and indications for primary and revision surgery, th

Above-knee amputation after recurrent dislocations of total knee arthroplasty.

Amputation after total knee arthroplasty (TKA) is an extremely rare but disastrous complication. Most of the reported cases resulted from refractory deep joint sepsis or perioperative vascular complications. We present a case of above-knee amputation resulting from vascular complications after recurrent dislocations of a TKA in a patient with underlying end-stage renal disease. The possible association between certain metabolic diseases and instability of TKA is discussed. In contrast to the high incidence of vascular complications after traumatic knee dislocation, popliteal artery injury afte

Primary constrained condylar knee arthroplasty for the arthritic valgus knee.

The purpose of the current study was to review results of primary constrained condylar knee arthroplasty in elderly patients with genu valgum deformity. The hypotheses were: (1) constraint has no adverse effects in elderly patients; (2) treating deformity with a constrained condylar knee prosthesis in lieu of lateral ligament release avoids morbidity, particularly peroneal nerve palsy and flexion instability; and (3) press-fit noncemented stem extensions enhance fixation of the cemented core components and are not prone to loosening. Between 1988 and 1993, 44 consecutive primary Constrained Co

Knee stability in posterior cruciate ligament retaining total knee arthroplasty.

Anteroposterior knee laxity was evaluated in 14 patients (19 knees) who had posterior cruciate ligament retaining total knee arthroplasty using the Miller Galante I prosthesis. The followup ranged from 87 to 118 months (average, 105.9 months), and the measurements were done using the KT-2000 arthrometer. The mean anteroposterior displacement with the knees with Miller Galante I prostheses was 10.1 mm at 30 degrees flexion and 8.1 mm at 75 degrees flexion. In the 15 knees with Miller Galante I prostheses with flexion greater than 90 degrees, seven had less stability at 75 degrees than at 30 deg

[Extra-articular fractures near the knee and concomitant knee damage].

While the prognosis of intra-articular fractures of the knee joint is determined by the reconstruction of the articular surfaces, ligamentous damage and resulting joint instability is a main problem with extra-articular fractures neighbouring the knee. In a follow-up of 43 cases of A 1-3 fractures according to the AO/ASIF classification, 37.2% of the patients revealed concomitant ligamentous damage. Especially supracondylar femoral fractures and the avulsion fractures of the posterior cruciate and lateral collateral ligament are at higher risk.--Ligamentous ruptures accompanying avulsion fract

Functional improvement of unicompartmental knee arthroplasty compared with total knee arthroplasty for subchondral insufficiency fracture of the knee.

Subchondral insufficiency fracture of the knee (SIFK) causes acute knee pain in adults and often requires surgical management. Unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) are the two most common surgical treatments for SIFK. While both UKA and TKA have their advantages, there is no consensus for SIFK localized on the medial compartment. We hypothesized that patients with SIFK treated with UKA would show superior patient-reported outcomes compared to those who underwent TKA. A total of 90 patients with SIFK located medially were included in the TKA (n = 45) and UK

Effect of knee arthroscopic debridement combined with peripatellar denervation on restoration of knee function in patients with knee osteoarthritis.

BACKGROUND: This research examines knee osteoarthritis (OA), a prevalent orthopedic disease marked by cartilage degeneration and chronic synovitis, leading to pain, restricted mobility, and eventual loss of knee function. Notably, patellofemoral osteoarthritis constitutes a significant proportion of knee OA cases. Our study aims to assess the impact of knee arthroscopic debridement coupled with peripatellar denervation on restoring knee function in OA patients and analyze the risk factors affecting treatment outcomes. By doing so, we hope to contribute to the informed selection of clinical tre

The subjective knee value is a valid single-item survey to assess knee function in common knee disorders.

INTRODUCTION: The patient's perspective plays a key role in judging the effect of knee disorders on physical function. We have introduced the Subjective Knee Value (SKV) to simplify the evaluation of individual's knee function by providing one simple question. The purpose of this prospective study was to validate the SKV with accepted multiple-item knee surveys across patients with orthopaedic knee disorders. MATERIALS AND METHODS: Between January through March 2020, consecutive patients (n = 160; mean age 51 ± 18 years, range from 18 to 85 years, 54% women) attending the outpatient clinic fo

Size of Medial Knee Osteophytes Correlates With Knee Alignment But Not With Coronal Laxity in Patients With Medial Knee Osteoarthritis.

The severity of osteoarthritis (OA) has been related to osteophyte size. However, the effects on osteophyte size of repeated and increased loading associated with joint laxity and varus misalignment remain unclear. We investigated these relationships in patients with medial knee OA and compared the performances of computed tomography (CT) and radiography for assessing osteophyte parameters. We examined knee joint alignment on radiographs and knee laxity using arthrometry in 191 patients with medial knee OA who were undergoing total knee arthroplasty. We also measured femur and tibia osteophyte

The association of knee structural pathology with pain at the knee is modified by pain at other sites in those with knee osteoarthritis.

The objective of this study was to investigate the associations of knee structural abnormalities with different patterns of pain. A total of 891 participants (average age 63 years; range 50 to 80 years) participated in this study. Presence of pain at the neck, back, hands, shoulders, hips, knees, and feet was assessed by questionnaire. Participants were categorized as having no pain at any site (no pain), pain only at the knee (KP), pain at other sites but not the knee (OP), and pain at the knee and other sites (KOP). T1-weighted or T2-weighted MRI of the right knee was performed to measure ca

Crosswalk and correlation between the Oxford Knee Score (OKS), Knee Society Score (KSS) and Knee Injury and Osteoarthritis Outcome Score (KOOS-12) in knee osteoarthritis.

BACKGROUND: Significant heterogeneity exists in the patient-reported outcome measures (PROMs) used to assess outcomes in knee osteoarthritis (KOA), with limited evidence exploring the correlation and conversion among the various PROMs. Our study aims to explore the correlation and conversion between the Oxford Knee Score (OKS), Knee Injury and Osteoarthritis Outcome Score (KOOS-12) and Knee Society Score (KSS) in patients with non-operatively treated KOA. METHODOLOGY: This was a prospective cohort study of 131 patients who received non-operative treatment (standardized physical therapy and an

Arthroscopic Arthrolysis of the Knee Joint Following Total Knee Arthroplasty.

Background: Arthrofibrosis, mixed contracture, and patellofemoral impingement are frequent complications following total knee arthroplasty (TKA), potentially leading to chronic pain and poor recovery of range of motion (ROM). The comprehensive management of these complications remains challenging and controversial. Methods: This study analyzed the outcomes of arthroscopic arthrolysis performed in 27 patients diagnosed with arthrofibrosis, mixed contracture, and patellofemoral impingement post-TKA to evaluate the efficacy of this technique in improving knee function, enhancing ROM, and reducing

Knee Joint Distraction for Bicompartmental Knee Osteoarthritis in Asian Patients.

INTRODUCTION: Young active patients with significant pain from knee osteoarthritis are a challenging group of patients to treat. For patients with symptomatic osteoarthritis involving both medial and lateral compartments, total knee arthroplasty (TKA) would traditionally be their only surgical option. Knee joint distraction (KJD) is a novel procedure in Asia that offers a joint preserving alternative for this cohort of patients. This study aims to evaluate patients with knee osteoarthritis treated with knee joint distraction (KJD). MATERIAL AND METHODS: Patients were included in this study if

Robotic-arm assisted total knee replacement in an ankylosed knee.

A patient with rheumatoid arthritis in her 30s who had a left knee that had been ankylosed for 10 years was treated with robotic-arm assisted total knee replacement (RA-TKR) and surgical release of the ankyloses. For RA-TKR, accurate bony registration is essential. It was difficult to record the bone intraoperatively in an ankylosed knee with changed morphology. With careful preparation and implementation, RA-TKR can be an effective option in complicated situations like ankyloses.

Primary total knee arthroplasty for acute fracture around the knee.

Relatively poor results have been reported with open reduction and internal fixation of complex fractures around the knee in elderly osteoporotic patients, and primary total knee arthroplasty (TKA) has been proposed as an alternative solution. While limiting the number of procedures, it meets two prerequisites: (1) to save the patient's life, thanks to early weight-bearing, to limit decubitus complications; and (2) to save knee function and patient autonomy, thanks to early knee mobilization. There are 3 main indications: complex articular fractures in elderly patients with symptomatic osteoar

Spontaneous Knee Ankylosis through Heterotopic Ossification after Total Knee Arthroplasty.

This paper reports on a case of total ankylosis of the knee after a cruciate-sacrificing cemented total knee arthroplasty (TKA). An 82-year-old female patient previously underwent primary TKA for osteoarthritis twenty years ago in our institution. She had recovered uneventfully and returned to her regular activities. There was no history of postsurgical trauma; however, she progressively lost knee range of motion. Radiographs revealed severe bridging heterotopic ossification.
