Low-velocity knee dislocation with sports injuries. Treatment principles.

Knowing that the injured MCL and PCL can heal and that the injured ACL and lateral side, predictably, will not heal are the bases for the treatment approach to these injuries. (1) Allow MCL healing nonoperatively. (2) Allow PCL healing to occur as long as PCL laxity is 2+ or less (reconstruct the PCL acutely if posterior drawer is > 2+ initially). (3) Initially delay ACL treatment and reconstruct later, if needed posterior drawer. (4) Perform acute lateral side repair to reattach structures to their distally torn site.

Significance of arthroscopy in children with knee joint injuries.

Clinical and arthroscopic findings after knee joint injury were evaluated in 35 children aged 4-15 years. Arthroscopy revealed a rupture of the anterior cruciate ligament in 9 patients; two lesions were only partial. Eight children suffered from patellar dislocation. Eleven patients showed minimal synovial and cartilaginous lesions as the only cause of hemarthrosis, hydrarthrosis and clinical symptoms. Three had chondral lesions due to a dysplastic patella. The clinical diagnosis was confirmed in only 12 of 35 cases. Arthroscopic therapy was performed in 15 and open surgical intervention in 16

Knee-ligament injuries associated with leg fractures. Prospective study.

It has been proved in 25-35% of the cases that knee-ligament injuries are associated with fracture of the femoral diaphysis. No such association has been confirmed between leg fractures and knee ligaments. In order to find out if this is a coincidence, a prospective study was conducted on 229 patients who had undergone operations for leg fractures at various locations and of variable intensity. It was established in 41 cases (17.34%) that the leg fracture was associated with knee-ligament injuries, resulting in joint instability. A significantly higher percentage of associated ligament lesions

[Open knee joint injuries (including vascular and nerve lesions)].

Immediate definitive reconstruction of all damaged structures leads to better results than delayed operations. If direct closure of the joint is not possible, local rotational flaps, muscle flaps or free skin-fat tissue flaps (i.e. a scapular flap) may be used. In injuries of the popliteal artery, the interposition of a saphenic vein graft is preferable to local reconstruction of the artery. Nerve injuries are the only exception to comprehensive primary repair. Reconstruction in these lesions is usually done later when all wounds are healed. The postoperative treatment includes the use of cont

Prevention of hip and knee injuries in ballet dancers.

Hip problems form about 10% (7.0 to 14.2%) of most published series of ballet injuries. The abnormally large range of external rotation needed for a perfect turnout is primarily due to soft tissue adaptation, more readily achieved in the young dancer. Insufficient range of motion at the hip throws considerable stress on the other lower limb segments. The snapping hip syndrome is common (43.8% of hip problems), with about one-third associated with pain. A tight iliotibial band may contribute to this, and balanced flexibility requires special attention to abductor stretching. The external clicki

Acute knee injury. How to determine if the knee is stable.

Knee injuries are common in patients of all ages. A detailed history concerning the mechanism of injury and symptoms is helpful in establishing a tentative diagnosis, and the physical examination can easily provide confirmation. X-ray films occasionally are helpful, but stress views and arthrograms should not be relied on for definitive diagnosis. Arthroscopy may be required for definitive evaluation of internal derangement of the knee and can be used therapeutically in meniscal injuries and other problems. Proper rehabilitation is mandatory for a good outcome; therefore, physical therapy is e

[Ligamentous and meniscal injuries of the pediatric knee joint].

Intraligamentous injuries to the anterior cruciate ligament (ACL) and meniscus injuries are rare in children and adolescents and often occur as a result of sports injuries. Clinically, they usually present as a hemarthrosis. The diagnosis of choice is the MRI examination. Surgical treatment of intraligamentary ACL injuries using the transphyseal technique is now also the gold standard treatment for children and adolescents, leg axis and length checks are necessary after surgery until growth is complete. Meniscus injuries are also addressed surgically. Postoperative follow-up treatment and reha

Assessing diagnostic challenges in acute soft-tissue knee injuries.

AIMS: This Delphi study assessed the challenges of diagnosing soft-tissue knee injuries (STKIs) in acute settings among orthopaedic healthcare stakeholders. METHODS: This modified e-Delphi study consisted of three rounds and involved 32 orthopaedic healthcare stakeholders, including physiotherapists, emergency nurse practitioners, sports medicine physicians, radiologists, orthopaedic registrars, and orthopaedic consultants. The perceived importance of diagnostic components relevant to STKIs included patient and external risk factors, clinical signs and symptoms, special clinical tests, and di

Knee ligament and meniscus injuries in children and teenagers.

Knee ligament and meniscus injuries in children and teenagers are becoming more numerous because of increased sports participation but also better diagnosis. Meniscus injuries occur either in a normal meniscus or due to a congenital anomaly. The diagnosis is made clinically and confirmed by MRI. Treatment depends on the findings: meniscoplasty for discoid meniscus and primary repair of meniscus tears. Meniscus preservation is the rule. Injuries to the central pivot of the knee typically involve either the anterior cruciate ligament (ACL) or the tibial spine. Age, anatomy and the mechanism of i

The pediatric knee: diagnosis and management of ligament injuries.

In the last few decades, sports injuries in pediatric and adolescent athletes have increased dramatically, with ∼ 38 million young athletes participating in organized sports annually in the United States. Starting at the age of 2 years, an increase of the incidence of knee injuries until adulthood with a peak at 6 and at 13 years of age has been observed. Due to a physiologic laxity of the ligaments, ligament injuries are uncommon in these patients, but they are getting more frequent. In the growing knee, open physis are the points of minor resistance. Therefore apophyseal injuries of the pedi

Occult Intra-articular Knee Injuries in Children With Hemarthrosis.

BACKGROUND: Hemarthrosis after acute knee trauma is a sign of a potentially serious knee injury. Few studies have described the epidemiology and detailed injury spectrum of acute knee injuries in a general pediatric population. PURPOSE: To document the current injury spectrum of acute knee injuries with hemarthrosis in children aged 9 to 14 years and to describe the distribution of sex, age at injury, type of activity, and activity frequency in this population. STUDY DESIGN: Descriptive epidemiology study. METHODS: All patients in the Stockholm County area aged 9 to 14 years who suffered ac

Acute Knee Injuries; On-the-Field and Sideline Evaluation.

An athlete who has an acute knee injury should be assessed rapidly on the field and then more thoroughly on the sideline or in the training room. On-the-field assessment includes questions about the mechanism of injury and any similar previous injuries, a visual check for knee deformities and skin injuries, a neurovascular exam, and, ideally, tests for flexion and hyperextension. On the sideline or in the training room, standard physical tests are likely to reveal any significant injuries. These include the patellar apprehension, Lachman, posterior sag, quadriceps active, posterior drawer, pos

Arthroscopic versus open debridement of penetrating knee joint injuries.

Arthroscopic debridement of penetrating knee joint injuries has become a common treatment method. A comparative study was undertaken to compare this method with open joint debridement. Fourteen penetrating knee joint injuries (fourteen patients) were treated by arthroscopic examination and debridement and were compared to sixteen penetrating knee joint injuries (fifteen patients) treated by open debridement. There were no resultant infections or operative complications in either group. Of note, the arthroscopic debridement group had a shorter postoperative hospital stay [mean of 1.6 days compa

Arterial injuries associated with complete dislocation of the knee.

To determine the frequency and type of arterial injuries associated with complete dislocation of the knee and to correlate the findings at physical examination with those at arteriography, a computerized search for patients with a diagnosis of dislocation of the knee at discharge from an urban trauma hospital was performed. Nineteen patients with complete dislocation of the knee were found. Arterial injuries were seen in six patients (32%). Four of the 19 patients had no pedal pulse at physical examination. In three of these four patients, occlusion of the popliteal artery was seen on arteriog

Epidemiology of acute knee injuries: a prospective hospital investigation.

During a 1-year period, data on all acute injuries treated at emergency departments were entered into a registry; of these injuries, 6% involved the knee joint. The patients were followed until a definite diagnosis was established. The rate of anterior cruciate ligament injuries and meniscus tears was, respectively, 0.3 and 0.7 (injuries per 1,000 inhabitants per year). The rate of dislocations of the patellofemoral joints was equal to the rate of anterior cruciate ligament injuries. Ten percent of the cases involved ligament injuries, and isolated ruptures of the anterior cruciate ligaments w

Arthroscopy in acute knee injuries: a prospective controlled trial.

Eighty-two patients took part in a prospective trial to assess the need and timing for arthroscopy in acute knee injuries. Patients with a suspected ligament injury, a suspected meniscal tear, or a haemarthrosis in the absence of fracture were included in the trial. Patients were entered randomly into two groups. In group I early arthroscopy was performed (within 48 h), in group II arthroscopy was performed within 21 days, but only when it was felt to be clinically indicated. Patients were assessed using the OAK knee score 1 year after the injury. There was no difference in the final result be

[Injuries of the extension apparatus of the knee joint].

The article analyses problems of the diagnosis, the choice of the therapeutic tactics, and the results of operative treatment in 478 patients. Operation undertaken as early as possible after the injury is indicated in fresh fractures of the patella with displacement of the fragments, ruptures of the tendon of the quadriceps femoris muscle and the patellar ligament. The authors prefer stable fixation of patellar fractures by means of a "tightening" wire loop and a "blocking" wire loop in injuries of the patellar ligament. Additional external immobilization is not employed. Study of the late-ter
