Adherence to Definitions of Labor Arrest Influence on Primary Cesarean Delivery Rate.

OBJECTIVE: The cesarean delivery rate in the United States is 31.9%. One of the leading indications for primary cesarean delivery is labor arrest. A modern understanding of the labor curve supports more time prior to the diagnosis of labor arrest. We conducted this study to examine the impact of adherence to the modern criteria for labor arrest and failed induction on rates of primary cesarean delivery and to identify predictors of meeting these criteria. STUDY DESIGN: We analyzed rates of primary cesarean deliveries overall and primary cesarean deliveries occurring due to arrest of dilation,

[The effect of spasmolytics on dilatation of the uterine cervix].

UNLABELLED: Spasmolytics and spasmoanalgetic mixtures are administered to facilitate dilatation of the cervix during delivery and to shorten the first stage of labour. This medication is used in 70% deliveries at the 1st Dept. of Obstetrics and Gynaecology Masaryk University in Brno. The aim of the study was to analyze the spasmolytic effect on the process of cervix dilatation. The study comprises 108 uncomplicated deliveries (52 deliveries with spasmolytics and 56 deliveries without). We analyzed the effect of spasmolytics on the cervix, process of labour and outcome of delivery. Six kinds of

Cardiopulmonary arrest after the administration of magnesium sulphate. A case report.

The death of a patient from cardiopulmonary arrest after the administration of magnesium sulphate for imminent eclampsia is reported. It has long been recognized that magnesium sulphate provides rapid control of convulsions in eclamptic patients with minimal sedative effects on both mother and fetus. However, because of the neuromuscular blocking action of magnesium and the narrow margin between therapeutic and toxic serum levels, the drug must be given with caution. A regimen for magnesium sulphate administration is advocated which allows safe and effective control of convulsions in eclampsia

High-dose versus low-dose oxytocin for augmentation of delayed labour.

BACKGROUND: A major cause of failure to achieve spontaneous vaginal birth is delay in labour due to presumed inefficient uterine action. Oxytocin is given to increase contractions and high-dose regimens may potentially increase the number of spontaneous vaginal births, but as oxytocin can cause hyperstimulation of the uterus, there is a possibility of increased adverse events. OBJECTIVES: To compare starting dose and increment dose of oxytocin for augmentation for women delayed in labour to determine whether augmentation by high-dose regimens of oxytocin improves labour outcomes and to examin

Management of the latent phase of labor.

1. Iatrogenic causes of prolonged latent phase such as sedation, narcotic analgesia, and epidural anesthesia should be avoided during the latent phase. 2. Cesarean delivery is not appropriate management solely for failure to progress in latent phase. 3. Multiparas in latent phase with favorable cervices should be treated with oxytocin stimulation. 4. Multiparas with unfavorable cervices and most nulliparas should be treated with therapeutic rest when they become exhausted during a long latent phase, without regard to an arbitrary time table. 5. Characteristics of latent phase in induced labors

Active-phase labor arrest: oxytocin augmentation for at least 4 hours.

OBJECTIVE: To assess a labor-management protocol that mandated at least 4 hours of oxytocin augmentation before cesarean delivery for active-phase labor arrest. METHODS: We prospectively evaluated term gravidas in spontaneous labor with active-phase labor arrest (cervix at least 4 cm dilated and 1 cm or less of cervical progress in 2 hours). Exclusion criteria included nonvertex presentation, previous cesarean, multiple gestation, and a nonreassuring fetal heart rate tracing or chorioamnionitis at the time of labor arrest. After the diagnosis of active-phase arrest, oxytocin was initiated wit

Risk for complications in the subsequent pregnancy following first versus second-stage cesarean delivery: 25 years follow-up in a large cohort.

BACKGROUND: Cesarean delivery is a prevalent procedure worldwide, and an established risk factor for subsequent pregnancies. AIMS: To examine if a prior cesarean delivery due to first vs. second stage non-progressive labor carries different risk profiles for adverse outcomes in the subsequent pregnancy. MATERIALS AND METHODS: A retrospective cohort study, based on data accumulated over the years 1988-2013 at the Soroka University Medical Center. We compared pregnancy complications and adverse perinatal outcomes in subsequent delivery following a cesarean delivery due to first vs. second stag

Association of Cervical Effacement With the Rate of Cervical Change in Labor Among Nulliparous Women.

OBJECTIVE: To assess the association of cervical effacement with the rate of intrapartum cervical change among nulliparous women. METHODS: We conducted a secondary analysis of a prospective trial of intrapartum fetal pulse oximetry. For women who had vaginal deliveries, interval-censored regression was used to estimate the time to dilate at 1-cm intervals. For each given centimeter of progressive cervical dilation, women were divided into those who had achieved 100% cervical effacement and those who had not. The analysis was performed separately for women in spontaneous labor and those who we

Advanced cervical dilatation and spontaneous preterm labor: a comparison between twin and singleton gestations.

OBJECTIVE: To determine the admission to delivery interval and the rate of immediate delivery in twin versus singleton gestation complicated by spontaneous preterm labor (SPTL). METHODS: A retrospective cohort study of pregnant women presenting with advanced cervical dilatation of 3-5 cm and frequent uterine contractions at 24-34 weeks of gestation was performed. The rate of progression to delivery within 12 h and 24 h, as well as rates of prolonged latency, were compared between twins and singletons gestations. RESULTS: Sixty-nine women were included, of which 25 carried twins and 44 single

Labour management: an appraisal of the role of false labour and latent phase on the delivery mode.

Achieving normal vaginal delivery requires an orderly transition from early labour to an established active labour. We analysed retrospectively the outcome of labour and the delivery mode based on the diagnosis of labour from the first examination of 3,130 parturients. A total of 1,847 (59.1%) delivered mothers had the first vaginal examination performed by the senior house officer (SHO). The majority (74.4%) of the mothers presented in established labour (cervical dilatation > or =4 cm), 25.6% in early labour, while 11.3% in latent phase. Patients who presented in established labour had a sta

[Fetal pulse oximetry: a clinical methodological study].

The aim of the study is to provide preliminary assessment of the possibilities of fetal pulse oximetry as a method for monitoring of fetal pulse rate and oxygen saturation during labour. The study includes 65 pregnant women during active labour at term--all with one fetus and ruptured membranes. A system for fetal pulse oximetry is used, consisting of monitor Nellcor-N 400; oxygen sensor FS-14 and printer P-400 (Nellcor Puritan Bennett incorporation). Sensor is applied by mean dilatation of cervical channel 6.5 +/- 1.9 cm. Mean duration of monitoring of the pregnant woman is 104 +/- 68 minutes

Talking cervixes: How times materialise during the first stage of labour.

The clock occupies a prominent position in many feminist and midwifery critiques of the medicalisation of labour and birth. Concern has long focused on the production of standardised 'progress' during labour via the expectation that once in 'established' labour, birthing people's cervixes should dilate at a particular rate, measurable in centimetres and clock time. In this article we draw on 37 audio- or video-recordings of women labouring in two UK midwife-led units in NHS hospital settings to develop a more nuanced critique of the way in which times materialise during labour. Mobilising insi

Location of placenta and clinical course of labour.

The results of studies on 2396 women are presented. Relations were found between location of placenta and 'ripeness' of uterine cervix, duration of labour, prognostic value of intrauterine pressure, complications of the third stage, fate of the newborn and success of induction of labour. The authors arrived at the conclusion that attachment of the placenta to the fundus uteri may be considered an unfavourable location causing an increase in the number of different deviations from normal labour.

Follistatin serum concentrations during full-term labour in women--significant differences between spontaneous and induced labour.

Follistatin has been isolated from human placenta and has been identified in human foetal membranes and fluids. Serum follistatin levels in women rise during pregnancy particularly near term. In this study, we examined the effect of induction and stage of labour on maternal plasma concentrations of follistatin. Women who gave birth after a normal pregnancy were retrospectively divided into three groups: those who went in labour spontaneously (n = 33), needed induction by amniotomy and IV oxytocin (n = 18) or underwent planned caesarean section (n = 10). Serum was collected at 38-40 weeks of ge

Cervical dilation assessment in simulators compared to a visual tool: A randomized study.

OBJECTIVE: to verify the correct assessment rate when using direct visual comparison in the cervical dilation measures in hard-consistency cervix simulation models. METHOD: an open-label and randomized study conducted with 63 Obstetrics students that were designated either to use direct visual comparison in a dilation guide or not. The students estimated cervical dilation blindly in simulators with different dilations. The primary outcome was the correct assessment rate. RESULTS: the students performed 141 tests. A higher correct assessment rate was found in the Experimental Group than in th

Cervical remodeling during pregnancy and parturition.

Appropriate and timely cervical remodeling is key for successful birth. Premature cervical opening can result in preterm birth which occurs in 12.5% of pregnancies. Research focused on the mechanisms of term and preterm cervical remodeling is essential to prevent prematurity. This review highlights recent findings that better define molecular processes driving progressive disorganization of the cervical extracellular matrix. This includes studies that redefine the role of immune cells and identify diverse functions of the cervical epithelia and hyaluronan in remodeling. New investigations prop
