Association between maternal and fetal characteristics and levator hiatal dimensions in the second stage of labor.

INTRODUCTION: Little is known about the pelvic floor hiatal dimensions in labor, despite this being the soft tissue complex through which the fetus passes in a vaginal delivery. Intrapartum assessment of pelvic floor hiatal dimensions, particularly in the second stage of labor, may improve understanding of the variation in levator hiatal measurements and their change between maternal and fetal characteristics. STUDY DESIGN: To assess the levator hiatal dimensions at rest during the passive second stage of labor using three-dimensional (3D) transperineal ultrasound, participants were consented

Re-examining Norms of Disrespect and Abuse in the Second Stage of Labor in Tanzanian Maternity Care.

Globally, the widespread occurrence of disrespect and abuse (D&A) on maternity wards is well-documented. Using ethnography and cultural consensus analysis we explore how the practice of midwives hitting women who are in the second stage of labor (pushing) has become a locally accepted form of care in Tanzania if a baby's life appears to be at risk. This analysis interrogates the deep uncertainty of birth outcomes in this setting that may motivate abuse during this time. Seriously engaging with local discourses on abuse and care sheds light on hegemonic norms and power dynamics and is critical

Extending the second stage of labor in nulliparous women with epidural analgesia: a cost-effectiveness analysis.

The objective of this study was to evaluate maternal outcomes with an extended second stage of labor and determine if an extended second stage is cost effective. This theoretical model evaluated expectant management to 4 h compared to delivery at 3 h in the setting of a prolonged second stage of labor in nulliparous women with epidural analgesia. In our theoretical cohort of 165,000 women, we found that an extended second stage resulted in 53,268 more spontaneous vaginal deliveries, 14,163 fewer operative vaginal deliveries, and 39,105 fewer cesarean deliveries. This approach also resulted in

Effects of an intervention program for reducing severe perineal trauma during the second stage of labor.

BACKGROUND: Obstetric anal sphincter injuries lead frequently to short- and long-term consequences for the mother, including perineal pain, genital prolapse, and sexual problems. The aim of the study was to evaluate whether the implementation of an intervention program in the second stage of labor involving altered perineal support techniques reduced severe perineal trauma. METHODS: All women reaching the second stage of labor and giving birth vaginally to singleton babies at Landspítali University Hospital (comprising 76% of births in Iceland in 2013) were enrolled in a cohort study. Data we

The impact of obstetric gel on the second stage of labor and perineal integrity: a randomized controlled trial.

OBJECTIVE: Dianatal® is a bioadhesive gliding film which reduces the opposing force to vaginal childbirth. We aimed to investigate the safety, applicability, and impact of Dianatal® obstetric gel on second stage of labor and perineal integrity. METHODS: Low-risk singleton pregnancies at term were prospectively enrolled. Eligible women were randomly assigned to either labor management without using obstetric gel, or labor management using intermittent application of obstetric gel into the birth canal during vaginal examinations, starting at active phase of labor (≥4 cm dilation). The primary m

Fundal pressure during the second stage of labor in a tertiary obstetric center: a prospective analysis.

AIM: To ascertain whether uterine fundal pressure should have a role in the management of the second stage of labor and to determine its prevalence, benefits and adverse maternal-fetal outcomes. METHODS: This was a prospective observational study set in a tertiary teaching and research obstetric hospital. A total of 8097 women in labor between 37 and 42 gestational weeks with a singleton cephalic presentation were enrolled. Subjects were subdivided into two groups: fundal pressure group (n=1974 women) and control group (n=6123 women). The primary outcome measure was the duration of the second

The use of perineal massage in the second stage of labor and follow-up of postpartum perineal outcomes.

Because perineal trauma causes both short- and long-term problems after labor, the high rate of episiotomies and spontaneous lacerations is an important women's health problem in Turkey. Our aim in this study was to investigate whether perineal massage during labor decreased perineal trauma and trauma-related problems. The study included 396 pregnant women who were giving birth for the first time, between March 2007 and February 2009, in Turkey. It can be concluded that perineal massage decreases the amount of suture material required for episiotomy and thereby the size of the episiotomy and t

The impact of length of second stage of labor on shoulder dystocia outcomes: a retrospective cohort study.

OBJECTIVE: To compare obstetrical and neonatal outcomes of vaginal deliveries complicated by shoulder dystocia, according to the length of second stage of labor. METHODS: We conducted a retrospective cohort study of 177 shoulder dystocia cases that were divided into three categories according to second stage duration (1-20, 21-59, 60-180 min, respectively). The three categories were compared in terms of obstetric characteristics and neonatal outcomes. Statistical analysis utilized the χ2-test and analysis of variance where appropriate. The odds ratios of brachial plexus injury and having a 5-

Duration of the second stage of labor while wearing a dental support device: a pilot study.

AIM: Developing a method of maximizing maternal expulsive effort should be of great value in reducing the number of cesarean sections or instrumental deliveries. Various investigations have shown that use of a dental support device (DSD) increases the isometric strength of different muscle groups. The aim of our study was to investigate the role of a DSD in second stage of pushing. METHODS: Nulliparous women with an uncomplicated singleton pregnancy course were randomly assigned either to a DSD group or to a non-device group. Duration of the second stage of labor was evaluated. Rates of cesar

A National Survey on Current Practices in Management of The Third Stage of Labor in Second Trimester Deliveries.

BACKGROUND: Retained placenta complicates roughly 21-30% of second trimester deliveries. Literature and clinical guidelines regarding management of the third stage of labor are limited, leading to a wide variation in practice. The purpose of this study is to clarify providers' current management practices and their supporting rationale. METHODS: A cross-sectional survey was administered to obstetric providers who have managed retained placenta following second trimester deliveries in the last five years via two national listservs of obstetric providers. Descriptive statistics were used to cha

Effects of Fetal Position on the Loading of the Fetal Brain During the Onset of the Second Stage of Labor.

During vaginal delivery, the delivery requires the fetal head to mold to accommodate the geometric constraints of the birth canal. Excessive molding can produce brain injuries and long-term sequelae. Understanding the loading of the fetal brain during the second stage of labor (fully dilated cervix, active pushing, and expulsion of fetus) could thus help predict the safety of the newborn during vaginal delivery. To this end, this study proposes a finite element model of the fetal head and maternal canal environment that is capable of predicting the stresses experienced by the fetal brain at th

The impact of extending the second stage of labor on repeat cesarean section and maternal and neonatal outcome.

OBJECTIVE: To evaluate the effects of extending the second stage of labor in women attempting a trial of labor after a cesarean section (TOLAC). METHOD: A retrospective cohort study comparing maternal and neonatal outcomes following TOLAC over two periods: period I whose prolonged second stage was considered 2 h, and period II whose prolonged second stage was considered 3 h. The primary outcome was repeat cesarean delivery (CD) rate. RESULTS: Incidence of repeat CD was significantly lower in period II (18.1% vs 29.7%, P < 0.001). Incidence of uterine rupture was significantly higher in perio

Prolonged Second Stage of Labor and Anal Sphincter Injury in a Contemporary Cohort of Term Nulliparas.

OBJECTIVE: This study aimed to assess whether a prolonged second stage of labor is an independent predictor of obstetric anal sphincter injury (OASI) in a contemporary cohort of nulliparous and term parturients, and to evaluate whether predelivery factors can accurately predict OASI. STUDY DESIGN: This was a nested case-control study within a cohort of consecutive nulliparous term parturients with a singleton gestation who underwent a vaginal delivery at a single institution between January 2014 and January 2015. Cases were defined as women with a third- or fourth-degree laceration at the tim

Intrapartum ultrasound at the initiation of the active second stage of labor predicts spontaneous vaginal delivery.

BACKGROUND: Longer duration of active pushing during labor is associated with a higher rate of operative delivery and an increased risk of maternal and neonatal complications. Although immediate pushing at complete dilatation is associated with lower rates of chorioamnionitis and postpartum hemorrhage, it is also associated with a longer duration of pushing. OBJECTIVE: This study aimed to evaluate whether fetal head station and position, as assessed by ultrasound at the beginning of the pushing process, can predict the mode of delivery and duration of pushing in nulliparous women. Management of the third stage of labor in second-trimester deliveries: How long is too long?

BACKGROUND: Retained placenta is the most common second-trimester delivery complication. As the optimal third stage of labor duration remains undefined, complications associated with retained placentas are difficult to study. OBJECTIVE(S): To determine the optimal third stage of labor duration in second-trimester deliveries based on estimates of time-specific probabilities of placental delivery, placental intervention, and postpartum complication. STUDY DESIGN: We retrospectively studied adult women with singleton second-trimester vaginal deliveries. We identified third stage of labor durati

Diagnosis of station and rotation of the fetal head in the second stage of labor with intrapartum translabial ultrasound.

OBJECTIVE: To investigate the ability of intrapartum translabial sonography to diagnose fetal station in the second stage of labor. METHODS: Patients with uncomplicated pregnancies at term gestation with fetuses in vertex presentation in the second stage of labor underwent serial translabial sonography and digital examinations. In a sagittal section of the maternal pelvis, the direction of the head was noted and categorized as downward, horizontal or upward. By rotating the transducer in the transverse plane the cerebral midline echo was also visualized and the rotation of the head was noted.
