Pandemic Demand for SARS-CoV-2 Testing Led to Critical Supply and Workforce Shortages in U.S. Clinical and Public Health Laboratories.

COVID-19 has brought unprecedented challenges to clinical and public health laboratories. While U.S. laboratories have continued striving to provide quality test results during the pandemic, the uncertainty and lack of supplies became a significant hurdle, hindering day-to-day laboratory operations and the ability to increase testing capacity for both SARS-CoV-2 and non-COVID-19 testing. In addition, long-standing laboratory workforce shortages became apparent, hindering the ability of clinical and public health laboratories to rapidly increase testing. The American Society for Microbiology, t

MLO's national CLIA '88 survey, Part 2: Praise versus protest--views from both sides of the fence.

While a recent survey shows that more laboratorians approve of CLIA now than in the past, a considerable enclave remain opposed to this law. Among the key issues that continue to dishearten and incite the clinical lab: the proposed exemption of POLs and diminished personnel standards.

Melioidosis and safety in the clinical laboratory.

Human infection with Pseudomonas pseudomallei, the causative agent of melioidosis, typically produces subclinical disease and an asymptomatic carrier state; occasionally clinical illness, frequently with a fatal outcome, may occur. Consequently, to help protect staff from laboratory-acquired melioidosis, microbiological and biomedical laboratories must have adequate facilities for safe work procedures and laboratory staff must engage in safe work practices. Recommendations from a melioidosis-endemic, diagnostic laboratory for the prevention of laboratory-acquired infection with this bacterium

Nosocomial salmonellosis: implications for microbiologic processing of stools in hospitalized patients.

Many microbiology laboratories are rationalizing their services and are not culturing stool for bacterial and parasitic pathogens from patients hospitalized for more than 3 days. We report a sporadic case of nosocomial salmonellosis that highlights the need for ongoing communication among clinicians, infection control personnel, and laboratories to allow for modification of such a routine laboratory protocol should epidemiologic evidence warrant.

Performance of an enzyme immunoassay test and anaerobic culture for detection of group A streptococci in a pediatric practice versus a hospital laboratory.

The ability of pediatricians and hospital laboratory personnel to detect group A streptococci in patients with suspected streptococcal pharyngitis was evaluated using the TestPack Strep A and anaerobic culture. Duplicate throat specimens (for similar processing by both the pediatricians and laboratory technologists) were simultaneously collected on rayon-tipped swabs from patients with symptoms of pharyngitis. Each swab was first inoculated to a 5% sheep blood agar plate, then tested for group A streptococcus antigen using the TestPack Strep A according to the manufacturer's instructions. Cult

Brucellosis - laboratory workers' nightmare come true: A case study.

INTRODUCTION: Brucella spp. are rarely encountered organisms in the medical microbiology laboratory and, when encountered, can cause concern in laboratory workers. Laboratory personnel may in fact develop serious disease as a result of this exposure. This case highlights shortcomings in recognition of Brucella spp. from a patient presenting atypically as well as the follow-up and management of an infected patient. CASE PRESENTATION: The patient was an 8-year-old boy from a rural area of South Africa who presented to an academic hospital with a bladder mass and history of enuresis in September

Exploring professional-related outcome differences between phlebotomy technicians, medical laboratory technicians, and medical technologists.

As part of a large scale practice analysis on Phlebotomy Technicians (PBTs), Medical Laboratory Technicians (MLTs), and Medical Technologists (MTs), additional data on four "home made measures" of professional-related outcomes, i.e., professional development, quality assurance monitoring, employer reimbursement, and types of continuing education, were also collected. In order to maximize data use from the overall sample of 3,097 respondents (constituting only 18% of the population surveyed), pairwise deletion of data was utilized. No a priori differences between PBTs, MLTs and MTs on these out

Overcoming geographic isolation.

There are a lot of advantages to living in an isolated area, away from the hustle and bustle of the big city. We work hard to provide services more commonly located in a metropolitan area. There is satisfaction in being self-sufficient and able to spread your wings and start a new venture such as an off-campus diagnostic center or a courier route. We enjoy certain advantages in this environment. Changes in medical care delivery systems get to us slowly, giving us time to plan for the impact. Antibiotic-resistant organisms are slower to move in and are easier to control. The blood donor populat

Health and safety of laboratory science students in Ibadan, Nigeria.

Laboratory science students are engaged in laboratory practice under supervision during the course of their training programme. They are exposed to the risk of laboratory-acquired infection and need to be adequately informed and equipped with facilities to protect their health. A questionnaire was administered to laboratory science students to determine their perception of hazards in laboratory practice and the observance of safety codes in their work practices. Of 128 students, 118 completed the questionnaire, a response rate of 92%. Sixty of them (51%) were males and 53 (45%) were females; f

Brucellosis in healthcare settings: research advances in nosocomial infection prevention and control.

Brucellosis, a globally significant zoonosis, poses substantial but often underappreciated risks within healthcare and allied professional environments. This narrative review synthesizes current research advances in the prevention and control of nosocomial and occupational brucellosis, addressing a critical gap in the existing literature that has predominantly focused on community-acquired transmission. We examine seven key dimensions: laboratory-acquired brucellosis and biosafety lapses; occupational exposure risks among healthcare workers, veterinarians, and abattoir personnel; epidemiology

Seroprevalence to polio in personnel at a virology institute.

OBJECTIVES: To determine the level of immunity to polio in adult personnel at the National Institute for Virology (NIV), South Africa. METHODS: Polio neutralizing antibodies results on 776 NIV staff members tested between 1979 and 1999 and seroresponses in seronegative personnel given a booster vaccination were analysed. RESULTS: 613 of the 776 (79%) personnel had neutralizing polio antibodies to all three types, independent of age, gender, race or job category. Types 1 and 2 antibodies were found in 92% and 94%, respectively, but type 3 was less prevalent at 87%. Of the 93 persons seronegat

[Outbreak of Q fever in a municipal abattoir].

The present paper reports on a Q fever outbreak of 149 cases in a municipal slaughter house focus. The diagnosis was established by clinical, epidemiological, radiologic and laboratory investigations. The route of transmission was air-borne from the sick animals (not detected at the veterinary control) to man. A closer cooperation is recommended between the human and veterinary health services, for early detection of the cases, their prevention and control. The authors suggest the need to immunize the personnel exposed to the risk of contamination (zoo technical units, slaughter house personne

Laboratory-acquired vaccinia virus infection in a recently immunized person--Massachusetts, 2013.

On November 26, 2013, the CDC poxvirus laboratory was notified by the Boston Public Health Commission (BPHC) of an inadvertent inoculation of a recently vaccinated (ACAM2000 smallpox vaccine) laboratory worker with wild type vaccinia virus (VACV) Western Reserve. A joint investigation by CDC and BPHC confirmed orthopoxvirus infection in the worker, who had reported a needle stick in his thumb while inoculating a mouse with VACV. He experienced a non-tender, red rash on his arm, diagnosed at a local emergency department as cellulitis. He subsequently developed a necrotic lesion on his thumb, di

Human papilloma virions in the laboratory.

Carcinogenic human papillomaviruses (HPV) can cause cervical, vaginal, vulvar, penile, anal, and oropharyngeal cancers. Non-carcinogenic HPVs can cause anogenital warts and recurrent respiratory papillomatosis. Currently, few research laboratories propagate, isolate or generate papilloma virions. However, there have been questions about potential exposure and risk in this setting. In this brief note, we discuss the use of wild type and laboratory-generated virions in research laboratories, potential routes of laboratory exposure, and considerations for HPV vaccination of laboratory personnel.
